The function of the kidney in adult subjects with Cooley’s disease has been hitherto inadequately investigated. In 10 patients suffering from thalassemia major, glomerular filtration rate, renal plasma flow, renal blood flow have been studied, together with urine concentration capacity. The authors emphasize certain pathological changes, such as the increase in renal plasma flow and the failure of the urine concentration ability. On the basis of renal biopsies, two pathogenetic theories are discussed: the functional impairment of the countercurrent system and fibrosis of the medullary tissu

We recently showed in dogs that there is a close relationship between bicarbonate reabsorption and glomerular filtration rate (GFR). The present studies were designed to confirm these findings in five normal men. Bicarbonate titrations were done after ingesting a low protein diet for one week and were repeated following elevation of GFR 11–29% by high protein diet and dexamethazone ingestion. In every case tubular reabsorption of bicarbonate increased in proportion to the increases in GFR. We conclude that glomerular-tubular balance for bicarbonate reabsorption is a characteristic of human, as well as canine kidney

Renal nerves were stimulated either directly (loop electrodes) or indirectly (bilateral carotid occlusion with maintenance of control blood pressure) in anesthetized dogs to determine the effects on renal blood flow (RBF), glomerular filtration rate (GFR), sodium excretion (UNaV), and intrarenal distribution of blood flow (microsphere method). Direct nerve stimulation decreased RBF 20% in 9 of 10 dogs; but GFR, UNaV, and intrarenal distribution of blood flow did not change significantly. Carotid occlusion decreased RBF in 9 of 10 dogs. There was a 22% fall in RBF for the whole group, GFR decreased 33%, and UNaV decreased from 0.39 to 0.17 µEq/min/g (p < 0.05). A small (8%) decrease in midcortical fractional flow (p < 0.05) after carotid occlusion was the only distributional flow change observed. The results disclosed no relationship between changes in sodium excretion and changes in total RBF, GFR, or intrarenal distribution of blood flow following nerve stimulation. These studies also indicate that the renal effects of carotid occlusion are probably not mediated by renal nerve stimulation alone

Serial hemodynamic studies have become increasingly important in hypertension research. These studies have been accomplished using the A-V shunt. The progressive decline in the number of A-V shunts in favor of the A-V fistula has limited the number of patients available for study. Although the A-V fistula can be used to measure cardiac output, the intra-arterial blood pressure cannot be measured through it. The present paper describes a technique formeasuring both the cardiac output and direct intra-arterial blood pressure using the bovineartegraft.

Prompted by a series of hepatitis-like cases in patients on regular haemodialysis treatment, the sets of polyvinylchloride (PVC) test tubings used were submitted to perfusion experiments. The perfusates from four commercially available tubings were also investigated. Diethylphthalate (DEP) could be identified by IR and PMR spectrometry in the perfusate from the test tubings. UV spectrometry proved the presence of other non-identified substancesin the perfusate from both the test tubings and from some of the other tubings.

Three unusual cases are described which demonstrated hypomagnesemia and symptoms resembling magnesium deficiency syndrome. The hypomagnesemia was most likely secondary to urinary losses from diuresis following release of post-renal obstruction and renal homotransplantation. Heretofore, this association has not been described. Case I was unique because of the post-obstructive diuresis in the face of marked renal impairment. Case 2 is noteworthy because of the profound hypomagnesemia, 0.4 mEq/l, and because of the development of congestive heart failure. The rapid improvement noted in cardiac function with magnesium replacement suggests a relationship to magnesium deficiency.

Since the use of NH4CI – the standard agent for short duration acid loading – may be hazardous in patients with hepatic disease, the acute renal response to another acidifying agent, orally administered CaCl2 (2 mEq/kg body weight), was compared to that obtained with NH4CI (1.9 mEq/kg body weight) in normal subjects during a 5- to 6-hour test. Urine pH decreased in all subjects to less than 5.20, and there were no significant differences in the maximal renal responses to either agent. Both in terms of acidification of the blood and as a stimulus to lowering of urine pH and excretion of acid, CaCl2 can adequately replace NH4CI as an acidifying agent in a short duration test and can be used in clinical settings in which NH4CI is contraindica

The concomitant administration of Indomethacin reduced the biochemical signs of the nephrotic syndrome induced in rats by aminonucleoside; in the rats treated with both drugs a morphological picture suggesting activated function of podocytes was found at the electron microscopic examination of the renal glomerular structures, together with the lesions characteristic of the aminonucleoside nephrosis. The possibility is discussed that Indomethacin could modify the permeability of the glomerular capillary wall by stimulating thepodocytes to synthesize basement membrane material.

Distinct glomerular ultrastructural lesions in the form of nodular subepithelial and intramembranous, as well as discrete and band-like subendothelial deposits are described in a patient suffering from mixed IgG-IgM essential cryoglobulinemic glomerulonephritis. It is suggested that the deposits may represent immune complexes which are believed to play a central role in the pathogenesis of the glomerulonephritis.
