The term acute interstitial nephritis (AIN) has generally been used to describe the pathologic changes observed in the kidney after a variety of ‘insults’, but many authorities have questioned whether AIN constitutes a distinct clinico-pathological entity. This report describes 5 patients in whom the diagnosis of AIN was established after ruling out other processes which could have produced similar clinical or pathologic findings. All patients presented with severe renal failure but improved both clinically and biochemically within several months. Five cases of acute post-partum renal failure with microangiopathic haemolytic anaemia are reported. In 4 patients the pregnancy had been complicated by preeclampsia or frank eclampsia, and in 1 patient the symptoms appeared some days after caesarean section performed beyond the term of a normal pregnancy. The presence of intravascular coagulation was indicated by thrombocytopenia, increase in fibrin degradation products in the serum, and by the finding on renal biopsy of intra-arteriolar and intraglomerular thrombosis. All the patients were given anticoagulant therapy (heparin alone or with dipyridamole). One patient died after 40 days with no improvement in the haematological or renal symptoms. The remaining 4 cases showed an improvement in renal function, and the haemolytic anaemia resolved. Control renal biopsy performed in 3 cases after varying intervals showed fibrous thickening of the intima of the arterioles and interlobular arteries. Anticoagulant therapy was continued over a prolonged period with the object of preventing further deposition of fibrin and platelet aggregation on the arteriolar lesions. The surviving patients have remained in good health for 38, 27, 20 and 6 months, respectively, after the delivery. These results should confirm the utility of anticoagulant therapy in microangiopathic haemolytic anaemia with renal failure, occurring during or after pregnancy.

To study aspects of protein homeostasis and proteinuria, experiments were done on normal rats and on rats 1–6 days after a single intravenous injection of the aminonucleoside of puromycin. Albumin excretion of normal rats, measured by a sensitive radioimmunoassay, was 0.139 mg/24 h. Following the injection of aminonucleoside, albumin excretion first increased on the second or third day, and increased thereafter exponentially with time. In 0.5 µm frozen substituted sections of kidney tissue there were a few droplets of albumin, but not of γ-globulin, in the proximal tubules of control rats. There was little change one day after aminonucleoside. Large numbers of protein reabsorption droplets, containing albumin and γ-globulin, were seen in both proximal and distal tubular cells of rats 4–6 days after aminonucleoside. This finding is consistent with increased reabsorption from the lumen by both proximal and distal tubules and, inferentially, with increased glomerular permeability to both proteins. Six days after aminonucleoside, two populations of tubules were observed; one relatively normal in appearance and containing an increased number of droplets; the second dilated, and containing massive amounts of both proteins in lumen and cells. Iodine metabolism has been studied in 20 patients with advanced chronic renal insufficiency due to primary renal disease, and thyroxine turnover in another 5 similar patients. Comparatively to 18 controls, the uremic patients had a lower urinary iodine excretion, and a much lower renal iodide clearance, which, however, was not as much decreased as the creatinine clearance. The normal relation between the renal iodide and creatinine clearances was disturbed when the latter was 6.3 ml/min or lower, and in these cases the renal iodide clearance exceeded the corresponding creatinine value. The plasma inorganic iodine (PII) was increased because of the iodide retention to (mean ± SE) 0.84 ± 0.17 µg/100 ml, compared to 0.12 ± 0.01 in the controls. The thyroidal iodide clearance (Th. Cl.) rate was decreased (22.8 ± 4.44 ml/min vs. 36.7 ± 6.48), but in spite of this decrease, the absolute iodine uptake (AIU) by the thyroid, which is calculated as AIU = Th. Cl. X PII, was increased (7.1 ± 1.37 vs. 1.9 ± 0.30 µg/h). The serum protein-bound iodine was normal. The PII, Th. Cl. and AIU values and the relation among them in renal insufficiency were comparable to those observed after chronic administration of small amounts of iodine. Convective ultrafiltration using commercially available artificial membranes with selective sieving properties is a widely used method of molecular separation. We had previously reported an ultrafilterable inhibitor of toad bladder sodium transport in whole canine plasma using this technique. In the present study, two variables which have large effects on ultrafiltration of the transport inhibitor were identified. A relationship between stirring speed during ultrafiltration and recovery of the inhibitor was found, such that detectable recovery of the inhibitor was shown only at high stirring speeds. These results interpreted in terms of the gel layer model of the concentration polarization phenomenon indicate a profound effect of concentration polarization on the sieving properties of the membrane. In addition, it was found that recovery of the transport inhibitor was reduced by the polypropylene membrane support supplied by the manufacturer of the ultrafiltration cell. The evidence indicates that the inhibitor adsorbs to polypropylene. The net effect of these two variables on ultrafiltration can result in both false positive and false negative bioassays for the humoral sodium transport inhibitor.
