We studied the distribution of IgAl- and IgA2-containing plasma cells in the jejunum of 5 patients with primary IgA nephropathy and of 2 patients with Henoch-Schönlein purpura. The percentage of IgA-containing cells (62.3. ± 4.6) was below the level found in normal (79.4 ± 5.1) (p < 0.001). The percentage of IgAl-containing plasma cells relative to the total IgA- containing cells was too low in 2, normal in 3 and too high in 2 subjects (mean percentage 68.1 ± 9.6; normal 69.0 ± 3.8). A prospective study evaluated the complications in 154 patients in whom a permanent peritoneal dialysis catheter was inserted percutaneously from April 1982 to June 1986. Obstruction to flow and fluid leakage occurred in 9.2 and 2.6% of the catheters, respectively. Catheter survival was 64.6 and 48.6% at 1- and 2-year follow-up, respectively. The complication rates and survival of percutaneously placed catheters was comparable to those reported for surgically placed catheters. Percutaneous insertion of Tenckhoff catheters is a viable alternative to using the surgical technique.

29 patients with IgA nephropathy whose proteinuria persisted at a level of 2.0 g/day or more and who received prednisolone treatment for 1–3 years were retrospectively evaluated on their clinical courses. 13 of 14 patients with renal dysfunction of less than 70 ml/min in initial creatinine clearance (Ccr) values subsequently entered a progressive course during a follow-up period of 47 months, leading to end-stage renal failure in 8 cases. On the other hand, only 1 of the other 15 patients with preserved renal function of 70 ml/min or more ended up with end-stage renal failure during a follow-up period of 74 months, although 7 underwent a progressive course. Three patients in the latter group experienced a prominent reduction in proteinuria to less than 1.0 g/day and maintained renal function. Meanwhile, the steroid group of moderate proteinuric patients with a creatinine clearance greater than 70 ml/min had a benign course, while the nonsteroid group had an unfavorable one. These results suggest that steroid therapy in IgA nephropathy may be able to stabilize a progressive course, especially in the early stage of the disease, although, because they come from an uncontrolled study, a definite conclusion cannot be draw

The G band characteristic of chromosomes in 15 patients with Balkan endemic nephropathy (BEN), 20 relatives with no features of the disease and 25 healthy controls were investigated by cytogenetic analysis of lymphocyte cultures from peripheral blood. A specific chromosome marker in BEN was established in one No. 3 chromosome homologue characterized by a discordance in the banding patterns of the long arm, shortening the band 3q25 with faster fusion of subbands q26.1 and q26.3, and lack of differentiation of q24. Among the patients there were a mosaic case with a caryotype: 46, XY/46, XY, t(1;3) (q11.2;q25) and another one with a t(2;3)(p11.2; q25) in a tetraploid lymphocyte cell. The evidence of shortening of q25 and postzygotic mutations for 1/3 translocation and 2/3 translocation with the breakpoints on chromosome 3 being in 3q25 suggested that the critical band for BEN is 3q25.

In vitro erythropoiesis inhibitory activity (EIA) was isolated and partially purified from sera obtained from a patient with chronic renal failure (CRF). EIA was purified from human sera by ammonium sulfate precipitation, column chromatography and PAGE electophoresis, and the corresponding biological activity was assayed in erythroid colony (CFU-E) cultures. Ammonium sulfate precipitation revealed that the EIA resided in two high molecular weight fractions which yielded a single peak on G-200 Sephadex. PAGE electrophoresis demonstrated that the EIA fraction separated into 3 bands with a molecular weight range of 12,000–50,000 daltons. Antibody to EIA was prepared in rabbits, and Ouchterlony immunodiffusion assays employing the antibody material revealed that several CRF sera generated intense double preciptin bands when tested in the assay. In contrast, normal sera consistently yielded a weak preciptin band. These results suggest the presence of reduced amounts of EIA-like material in normal humans and elevated levels in CRF sera. Linear calcifications of the abdominal aorta and of the iliac and femoral arteries were measured yearly for 3 years on X rays of 24 patients on chronic hemodialysis taking variable amounts of calcium carbonate and Al(OH)3 but no pharmacological doses of vitamin D or lα-hydroxylated vitamin D derivatives. The speed of their extension appeared exponential and covariant with the male sex, age only for men and, independently of these two factors, with diastolic blood pressure and blood triglycerides. Plasma concentrations of calcium, phosphate and glucose were covariant with the extension of calcinosis only at a borderline level. The doses of calcium carbonate and the levels of plasma alkaline phosphatase were not at all covariant. Conclusions: (1) The effect of high doses of calcium carbonate is possibly harmful only when supraphysiological levels of plasma calcium are induced, whereas plasma phosphate is not adequately decreased. The doses of calcium carbonate per se have no deleterious effect (2). Since alkaline phosphatase is not covariant with the extension of calcinosis, the degree of hyperparathyroidism per se does not seem to play a causative role in vascular calcinosis (3). A 44-year-old man demonstrated proteinuria, microhematuria and renal dysfunction during the course of hyperthyroidism. Neither cryoglobulinemia nor paraproteinemia including light chains was found. No systemic signs suggestive of amyloidosis appeared. Histological findings showed a diffuse mesangial matrix increase with slight mesangial proliferation and diffuse granular depositions of IgG, IgA, C3, C4, Clq, k and λ light chains. Ultrastructurally, micro fibrils of about 20 nm in width were seen to be deposited diffusely in mesangial areas and in glomerular basement membranes. Congo red and thioflavin T staining were negative. These findings support the possible existence of a specific glomerular disease different from amyloidosis

A third case of acquired phosphate diabetes associated with a syndrome of inappropriate secretion of antidiuretic hormone (SIADH), related to a pulmonary tuberculosis, is reported. Renal hypophosphatemia in this patient was caused by the erroneous intake of 1 g doxycycline. It is likely that the selective nephrotoxicity in these 3 patients with SIADH was induced by tetracycline.

A 5-year-old Chinese girl had had absence seizures and received sodium valproate (VPA) treatment which provided good control. Six months later, she developed interstitial nephritis with proteinuria and microhematuria. Renal biopsy revealed interstitial nephritis with granular deposition of immunoglobulin G (IgG) and C3 in the renal tubular basement membrane (TBM). Ultrastructurally, dilated smooth endoreticular cisternae with mitochondrial degeneration in the tubular cells and scattered electron-dense deposits within the TBM were also noted. Serum circulating immune complexes were detectable, ACH50 and properdin factor B increased. Mononuclear cells (MNC) from the patient after in vitro incubation with VPA (100 μg/ml) induced interleukin-2 (IL-2) production and lymphoproliferative response. However, there was no response in controls. The serum VPA level ranged from 84 to 92 μg/ml. After VPA was stopped, the microhematuria and proteinuria disappeared. These observations indicate that VPA-induced interstitial nephritis represents a sequence of interrelationships among multiple immunologic factor
