摘要:
A case of generalized pustular psoriasis in pregnancy treated effectively with systemic corticosteroids is reported. A live, healthy infant was born at 36 weeks gestation. A further severe episode of pustular psoriasis occurred after cessation of corticosteroids, but was treated successfully with methotrexate and isotretinoin. The treatment of generalized pustular psoriasis in pregnancy is discussed.

摘要:
Lichen myxoedematosus (papular mucinosis) is characterized by papular lesions (and sometimes by internal involvement) caused by a dermal infiltrate of acid mucopolysaccharides which induce extensive thickening and hardening of the skin. The aetiopathogenesis of the disease is unknown. The therapy is generally by immunosuppression. We report the case of a 63-year-old man with lichen myxoedematosus successfully treated with cyclosporin A.

摘要:
We present seven patients with clinical and histological features typical of confluent and reticulated papillomatosis (CRP). Treatment with minocycline and/or erythromycin resulted in complete clearing of this skin condition, raising the possibility of CRP being triggered by a bacterial infection.

摘要:
Flat warts, mainly caused by human papillomavirus types 3 and 10, have a high rate of spontaneous remission. Those warts that do not disappear by themselves can be very difficult to treat. In our study we compared the efficacy of topical tretinoin, topical interferon beta and the combination of both substances in the treatment of flat warts. The combination of both substances proved to be considerably more effective than either alone.

摘要:
As calcipotriol has been reported to increase mitotic activity, to have a role in regulating immune response and to increase the contact hypersensitivity response when applied topically at the elicitation site we sought to determine whether topical calcipotriol is able to potentiate the response to squaric acid dibutylester (SADBE) in severe alopecia areata. A group of 28 subjects with severe alopecia areata were sensitized with a 2% SADBE solution applied to the vertex. After 2 weeks and then weekly, a 0.001% SADBE solution was applied to the whole scalp. Every day, except the day of SADBE application, an ointment containing 50μg/g calcipotriol was applied to the left side of the scalp. Treatment with calcipotriol and SADBE did not significantly potentiate hair growth (P= 0.133). This study suggests that calcipotriol has an inhibiting action on cell multiplication.

Hairless mice, effective as a model for the prevention of steroid atrophy by tretinoin, were used to examine the effectiveness of tretinoin as a therapeutic agent for the reversal of steroid atrophy. Skh-1 mice were treated topicaUy with 0.05% clobetasol propionate for 3 weeks, followed by either 0.05% tretinoin or vehicle for 3 weeks. Appropriate controls were included. Striking tretinoin effects were a marked reversal of epidermal and subcutaneous muscle atrophy and an increase in the steroid-reduced glycosaminoglycan levels that significantly exceeded vehicle effects. Rapid recovery of fibronectin, collagen and non-collagenous protein levels with vehicle treatment confounded tretinoin results. Although the hairless mouse is not an ideal model for the treatment of steroid atrophy, the results suggest a possible therapeutic value for tretinoin.

In an open, prospective multicentre trial in 89 dermatological practices in Germany, 433 patients with subacute and chronic forms of psoriasis were treated withMahonia aquifoliumointment (Rubisan).M. aquifoliumointment is used as a homoeopathic preparation (according to the German Homoeopathic Pharmacopeia). Of 433 patients entered into the study, 375 were treated over the planned period of 12 weeks, or dropped out of the study early because of healing. A modified PASI score fell significantly from 5.5±4.0 to 2.3±2.6. According to an overall evaluation by dermatologists, symptoms improved or disappeared in 81.1% of patients. According to a subjective evaluation by the patients, 79.7% improved or healed completely. The tolerability ofM. aquifoliumointment was evaluated as good or very good by 82.4% of patients. There was a significant improvement in the quality of life, which was used as a subjective parameter throughout the study. While 30.1% of patients had significant or severe symptoms at the start, this was the case for only 5.6% after 12 weeks.M. aquifoliumointment is thus a well-tolerated preparation which should find a place in the treatment of psoriasis.

Itraconazole is an oral triazole antifungal drug with an extended spectrum of activity and a favourable phar-macokinetic profile, which results in prolonged activity in skin and nail tissues. It is active against most human fungal pathogens and has been conventionally used at a dose of 100 mg once daily for 15 days in cases of tinea corporis/cruris and, at the same dose for 30 days in cases of tinea manuum/pedis. Studies in onychomycosis have indicated that 200 mg itraconazole once daily results in proportionately higher drug levels in keratinous tissues than a dose of 100 mg. In this open multicentre study, 33 patients with tinea (16 with tinea corporis and/or cruris and 17 with tinea manuum and/or pedis) received itraconazole at a higher dose (200 mg once daily) but for a shorter time (7 days in the case of tinea corporis/cruris and 15 days in the case of tinea manuum/pedis). Overall, 82.7% of the patients (66% of those with tinea corporis/cruris and 94% of those with tinea manuum/pedis) were cured or markedly improved 8 weeks after the end of treatment. These findings were confirmed by the patients' opinion of the treatment and the grading of each clinical symptom of infection. Itraconazole was well tolerated. The results suggest that shorter treatment schedules with higher doses may be at least as effective as conventional schedules.
