Moderate to severe papulopustular acne requires systemic and long-term treatment, the first choice therapy most often being tetracycline. In this double-blind randomized parallel-group study a new erythromycin derivative, erythromycin acistrate (EA), a 2′-acetyl ester prodrug of erythromycin, was compared with routine tetracycline treatment. The study group comprised 60 patients with moderate or severe acne who were treated for up to 6 months. One patient in each treatment group was completely cured within the first 3 months. After 6 months' treatment a further patient in the tetracycline group was completely cured. There were no significant changes in laboratory test values. Both drugs were well tolerated: one patient in each treatment group discontinued treatment due to diarrhoea during the first week. EA is a safe and well-tolerated alternative to tetracycline for the systemic treatment of acne.

In this double-blind clinical trial we compared the effect of clobetasol propionate ointment (Dermovate) in Orabase (50/50 v/v) and triamcinolone acetonide (Kenacort-T) in the treatment of erosive oral lichen planus (OLPe). All patients (n= 40) presented with uni-or bilateral atrophic or erosive lesions on the buccal mucosa or on the tongue. The ointments were applied during a 9-week period: the first 3 weeks twice a day, the following 3 weeks once a day, and during the final 3 weeks once every second day. After each 3-week period, the patients were asked to rate their symptoms on a visual analogue scale (VAS). In addition, the lesions were scored by a clinician using a four-point scale. Following treatment for 3 weeks, clobetasol propionate was found to be more effective than triamcinolone acetonide with respect to clinical improvement (P<0.05). No significant difference was observed following 6 and 9 weeks of treatment. Immunohistochemical analysis of biopsy specimens taken prior to and after the 9-week period revealed no change in the distribution of Langerhans cells irrespective of the treatment. The results demonstrate that clobetasol propionate compared with triamcinolone acetonide provides a more immediate clinical response in OLPe.

A study was performed to assess the value of a topical corticosteroid after cryotherapy of hand warts. The study group comprised 25 patients who had a pair of warts suitable for comparison. The warts were treated with liquid nitrogen cryotherapy using a standardized method. Clobetasol propionate was applied to one of each pair of warts and placebo to the other under double-blind conditions. Using a visual analogue scale the patients recorded the severity of pain, duration of pain and the severity of blistering in each lesion. Analysis of the results showed no significant difference between lesions treated with corticosteroid and those treated with placebo. We conclude that a single appb'cation of clobetasol propionate is ineffective in relieving symptoms after cryotherapy of hand warts.

Long-term acyclovir therapy does not seem to reduce the rate of post-treatment herpetic recurrences. We report the results of long-term acyclovir followed by thymopentin, a synthetic polypeptide derived from the thymic hormone, for the treatment of highly recurrent genital and orofacial herpes simplex. We tried a 3-month regimen with acyclovir followed by 18 weeks of thymopentin in patients suffering from either orofacial or genital herpes. We obtained complete or partial responses with significant reduction in the recurrence rate in 88.9% of patients with orofacial herpes and in 84.6% of patients with genital herpes. We believe that the proposed treatment is of clinical value and is also to be recommended in terms of cost/benefit.

The aim of this multicentre controlled study was to analyse the effect of the timing of antibiotic administration on the preoperative prophylaxis of wound infections in contamination-prone areas of the skin and to verify the utility of a single dose of prophylactic antibiotic compared with other multiple dose schemes. A total of 527 patients were surgically treated for skin neoplasms arising in contamination-prone areas. The four prophylactic programmes, to which the patients were randomly allocated, were as follows: (A) no antibacterial prophylaxis; (B) intramuscular cephazolin, 1 g every 12 h beginning 48 h prior to surgery and continuing for 48 h after surgery; (C) intramuscular cephazolin, 1 g every 12 h beginning 2 h before surgery and continuing 24 h after surgery; and (D) intramuscular cephazolin, 1 g single dose 2 h before surgery. The rate of postoperative infections was: group A 12%, group B 4.6%, group C 0.77%, group D 2.96%. Our study confirms the utility of antibiotic prophylaxis in prevention of postoperative infections and demonstrates that brief regimens are superior to more prolonged regimens. In particular, a single dose antibiotic schedule significantly reduces the infection rate, is cheaper, and is better tolerated by the patients.

The purpose of this study was to determine the effects of daily bathing with emollient in children with atopic dermatitis. A group of nine children aged 5 months to 13 years were studied at weekly intervals for 1 month. The parents were asked to bathe the child once per week in a bath with emollient (Oilatum) at a standard concentration. Additionally, the parents were asked to select one of the child's arms for a 15 min daily soak in a basin of water with emollient. The children were objectively assessed by an observer who had no knowledge of which arm was receiving the daily soaks. After 2 weeks, in eight of the nine children the arm that was soaked daily was visibly worse than the unsoaked arm. Mean clinical scores for the nine patients were determined at weekly intervals and in the two-tailed pairedt-test thePvalues at weeks 2 and 3 revealed a statistically significant difference between the arms. There is thus no evidence from this study that routine daily bathing in emollient is helpful as has been claimed. Indeed the results suggest that soaking for 15 min daily is harmful in chronic stable atopic dermatitis in children.

Eight different formulations for the oral administration of 8-methoxypsoralen (8-MOP) were compared in rabbits. Plasma levels of 8-MOP were measured using a high-performance liquid chromatography method. Partly dissolved 8-MOP resulted in higher plasma levels than the solid form. Addition of corn oil to a semisolid matrix (SSM) increased AUC values and peak plasma concentrations indicating that the oil component has an important influence on the absorption of 8-MOP in the rabbit. No marked differences were found using oils with differing fatty acid composition.

We compared the effectiveness of Q-switched Nd:YAG and alexandrite lasers in the treatment of 31 patients with monochromatic black amateur tattoos. The effects of different fluences and varying energy deposition patterns were also investigated. Improvement was determined by clinical scoring. In all patient groups superior results were obtained with the Q-switched Nd:YAG laser. Adverse side-effects other than transient textural changes were not noted. Our observation showed that the homogeneity of energy deposition rather than high fluence is crucial for successful treatment of monochromatic amateur tattoos.

The treatment of 90 cases of generalized pustular psoriasis (including impetigo herpetiformis and acrodermatitis continua) in the period from 1982 to 1992 was evaluated. A combination of Lei-Gong-Teng (Tripterygium wilfordiiHook F), dapsone and antibiotic therapy is recommended for the treatment of mild cases of this disease, and etretinate, hydroxyurea, methotrexate and chloromycetin were effective for severe cases. Thiamphenicol produced poor results. It is recommended that corticosteroids be avoided except when routine methods are ineffective and the condition of the patient is life-threatening.
