The immediate post-operative course was investigated in 70 patients who had undergone coronary artery bypass grafting under normovolaemic moderate and subsequent extreme haemodilution, and in 70 control patients who underwent a similar operation with a haemic prime in the heart-lung machine. Altogether 8 patients (5.7%), 4 from the haemodilution group and 4 from the controls required re-exploration for excessive post-operative bleeding and were examined as their own group. There were no marked differences in bleeding through chest tubes or in blood haemoglobin concentration between patients who had undergone intra-operative haemodilution and controls. Post-operatively both patient groups required approximately analogous amounts of infused blood, colloids and crystalloids. Central venous pressure was consistently lower in haemodiluted patients. Opening of peripheral circulation as indicated by a rise in skin temperature occurred significantly faster in haemodiluted patients than in controls. Before and after weaning from the respirator, arterial blood oxygen tension was slightly lower in patients who had undergone intra-operative haemodilution. Serum creatinine levels were lower and blood platelet counts significantly higher in haemodiluted patients than in controls. Rates of peri-operative myocardial infarctions showed no essential difference between the groups.

The sugar and nitrogen content of the gastric juice was assessed in 27 patients with duodenal ulceration, 17 patients with dyspepsia, and 27 normal volunteers. Comparing the data for each constituent for each patient using a discriminant function analysis, it was shown that the normal group differed significantly from both the duodenal ulcer group and the dyspeptic group, but that there was only a degree of variation between the two groups of patients.

A randomized controlled trial to assess the effect of intravenously administered Trasylol on the healing of experimental colonic anastomoses in the rabbit has been performed. Before undergoing a standard left colonic resection and anastomosis, the animals received intravenous Trasylol (80,000 KIU). Postoperatively 160,000 KIU were given in divided doses per day for 3 days. Control animals received saline placebo. The mean bursting pressure of the anastomoses in the Trasylol-treated group was 47.7 ± 3.0 mm Hg compared with 37.5 ± 3.3 mm Hg for the control group (p < 0.05). The mean difference in collagen content of the anastomosis compared to the resected specimen was +1.25 ± 0.50µg/mg and -1.02 ± 0.47 µg/mg for Trasylol and placebo, respectively (p < 0.005). In this model intravenous Trasylol produces a significant elevation of the bursting pressure and a significant improvement in the collagen content of the anastomosis. In 12 patients with suspected acute appendicitis undergoing appendectomy the bowel sounds were registrated pre- and postoperatively with a microphone placed on the abdominal wall. The results show that auscultation and registration of bowel sounds may aid in the diagnosis of acute appendicitis. However, these results cannot be applied to the clinical situation. Further evaluation of this method is essential.

Acute hemorrhagic pancreatitis was induced in rats by injecting sodium taurocholate into the common biliopancreatic duct. The extent of pancreatic necrosis was quantified in histological sections during the course of the disease. The proportion of necrotic acini was low, although the amount of necrosis increased from 3.3% of pancreatic parenchyma at 15 min to 10.5% at 12 h. The degree of ischemia in the inflamed pancreas was estimated by extracting intravenously injected toluidine blue from the gland. The amount of the dye in the gland decreased progressively during 12 h to 58.8% of the amount in normal pancreas. The development of pancreatic edema was studied by recording the water content of the gland. The edema was maximal at 3 h and resolved partly in 12 h after the induction of the disease. Necrosis and ischemia become progressively more pronounced in the edematous pancreas during sodium taurocholate-induced acute hemorrhagic pancreatitis. This kind of pathophysiologic course is also thought to characterize human pancreatitis. The present simple model of acute hemorrhagic pancreatitis in the rat is suitable for quantitative observations on the development of pancreatic damage under various experimental conditions.

Sliced rat liver was autologously transplanted to subcutaneous tissue. Morphology of the hepatocytes in transplants was investigated for up to 1 year after auto-transplantation; the effect of partial hepatectomy on the autografts was studied. Autografts with approximately one-tenth of the original weight were found alive in subcutaneous tissue for as much as 1 year after transplantation. Partial hepatectomy did not lead to greater weight.

In 7 patients biopsies of the extensor hallucis or digitorum brevis muscles were obtained before and 2 weeks after surgical denervation. Ultrastructural morphology showed convincing phenomena of denervation, but not of incipient regeneration. No increase (or decrease) of single or ‘twin’ myonuclei could be demonstrated in light microscopic examination of semithin sections. Histochemical examination did not show significant changes, but revealed type grouping in all biopsies, presumably due to previous denervation-reinnervation processes. Biochemical studies revealed decreased cytochrome c oxidase activities in the biopsies after denervation.
