Reticulo-endothelial function was studied in liver-grafted pigs using intravenous carbon clearance at 24 h and 7 days postgraft. Flushing of auto- or aliografts with Ringer’s lactate and with or without 4-hour storage caused no change in carbon clearance. To investigate the feasibility of obtaining grafts from living, genetically related adult donors without any complicated harvesting techniques, orthotopic partial liver transplantation (PLT) without cold perfusion of the donor graft was evaluated in beagles by assessing the graft viability using ketone body ratio (KBR). Ten PLT were performed under venovenous bypass. The left half of the donor liver was transected in situ followed by systemic heparinization, and immediately implanted with care taken to leave the recipient’s inferior vena cava intact, and to maintain the normothermic state without cold perfusion. Four often dogs survived for 5 days or longer (longest survival was 9 days) and died of other causes than graft dysfunction. Four others died of thromboembolitic episode, accidental bleeding or technical failure. Two dogs died of graft dysfunction. The changes in KBR in 5 dogs without fatal complications after transplantation were maintained within normal range thereafter. These results suggest that the PLT without cold perfusion of donor graft is possible from the viewpoint of energy metabolis

Changes in the composition of serum bile acids were investigated after orthotopic liver transplantation in piglets. Serum levels of all bile acid components rapidly increased during the anhepatic phase and then quickly decreased, returning to the preoperative state within 6 h of revascularization of the allograft. In the animals in which extrahepatic biliary stenosis was a complication, serum total bile acids (TBA) increased remarkably. A marked increase of hyocholic acid and marked decrease of hyodeoxycholic acid were noted; as percentages of TBA, the average levels per day were 91.3 ± 1.3 and 5.0 ± 1.3%, respectively. No change in chenodeoxycholic acid (CDCA) was observed. In the animals suffering acute rejection without extrahepatic biliary stenosis, serum TBA increased slightly and the percentage of CDCA rose, the average level being 29.6 ± 1.5%. It has recently been suggested that the use of calcium channel-blocking agents may improve the results of renal transplantation either by enhancing the immunosuppressive effects of ciclosporin, or by improving posttransplant acute renal failure. In the present study, unlike others, verapamil was administered to the animals for 10 days posttransplant, alone or in conjunction with ciclosporin and/or solumedrol. In the group in which all three agents were given together, there were minimal changes in plasma urea or creatinine for the 10 days after transplantation. However, survival and the histological appearances of the kidneys at autopsy showed no improvement as compared with those in animals treated with ciclosporin and steroids. There is thus no evidence from this study to suggest that verapamil could advantageously be added to a clinical immunosuppressive regime.

The authors describe an original surgical technique for pancreatic transplantation carried out on an experimental rat model which duplicates as closely as possible the anatomic-functional situation of a pancreatic graft in man (one arterovenous vascular peduncle with one functioning kidney only). The technique entails a microanastomosis between the mesenteric artery and the portal vein of the donor organ with the left renal artery and vein of the recipient. This method appears to be technically encouraging and particularly useful in studying the early phases of ischemic graft injuries, a field in which the authors are particularly interested.

In order to evaluate ventilatory function of the remaining lung after lobectomy, using dogs, we measured the pressure-volume curve of the right lower lobe separately from the other lobes. The right lower lobe bronchus was surgically separated from the other bronchi and the expiratory volume of the right lower lobe was measured independently. The dogs were put in an airtight box. Transpulmonary pressure and expiratory volume in the right lower lobe were measured before lobectomy, after the left upper lobectomy and subsequently after the right upper and middle lobectomy. As a result, the esophageal pressure at endoexpiratory state became more subatmospheric following the lobectomies and the pressure-volume curves of right lower lobe shifted down to the right. Though the right lower lobe was intact after surgery, the pressure-volume curve shifted. This might be caused by the imbalanced postoperative volume of the lung and thoracic cage, and it is one of the problems remaining after lobectomy.

We examined the pulmonary hemodynamics and morphology after injection of a sclerosing solution of 5% ethanolamine oleate (EO) into 24 normal dogs. EO of 0.5 ml/kg (n = 5), 1.0 ml/kg (n = 6), and 3.0 ml/kg (n = 7) was injected through the jugular vein into the right atrium for pathological examination and gravimetric study of the lung, while monitoring the pulmonary hemodynamics for 12 h. Normal saline of 3.0 ml/kg was injected into the remaining 6 control dogs, using the same method. Cardiac output significantly decreased immediately after injection of the sclerosant in all dogs given 0.5 ml/kg, 1.0 ml/kg and 3.0 ml/kg injections of EO; however, there was a tendency toward recovery from 6 h after injection in dogs given 0.5 ml/kg and from 9 h in dogs given 1.0 ml/kg. Pulmonary hypertension just after injection and hypoxia at 9-12 h occurred only when 3.0 ml/kg was injected. Irreversible pulmonary hemorrhage was present in the excised lungs in 4 of 7 dogs given 3.0 ml/kg, while there were no significant lesions in the other dogs. The lung water content in cases of 1.0 and 3.0 ml/kg injections was significantly higher than that in the controls, while there was no significant difference between those given 0.5 ml/kg and of the controls. The findings obtained in this study suggest that EO less than 0.5 ml/kg used for sclerosing esophageal varices seems to have little untoward influence on pulmonary hemodynamics and morphology.

Dispensing subcutaneously 1,2-dimethylhydrazine, intestinal carcinogenesis was investigated in male Fisher rats with different surgical colonic anastomoses, producing blind gut loops of isoperistaltic high (fecal stasis) and anisoperistaltic low fecal contact. One hundred and eight rats, except 1 rat of the control group, developed colonic neoplasms. In contrast to the control group, mainly in anastomotic areas and in isoperistaltic blind gut loops with intensive fecal contact huge adenocarcinomas of exophytic growth appeared with a mean tumor diameter (MTD) of 1.9 ± 0.7 to 2.2 ± 0.8 cm. Even in the control group, where rats only underwent laparotomy, we observed small polypoid adenocarcinomas mainly located in the distal colon (MTD: 0.7 ± 0.3 cm). The H2-blockers, cimetidine and ranitidine, are reported to inhibit liver regeneration. In this work, the effects of a new powerful H2-receptor antagonist, famotidine, on liver regeneration were studied in rats. The animals were divided into three groups: group I in which a standard two-thirds hepatectomy was performed, group II in which the rats were treated with famotidine (intramuscular dose of 0.8 mg/kg body weight) on the day of operation and 24 and 48 h after hepatectomy, and group III in which the animals were intramuscularly injected with a larger dose of famotidine (1.2 mg/kg body weight) in the same way as group II. The histology and mitotic index of remnant livers and serum levels of aminotransferase and albumin were examined from 24 h to 10 days after the operation. The treatment with famotidine (groups II and III) did not inhibit hepatocyte mitosis but, on the contrary, raised the index on day 3 after hepatectomy when compared with the controls (group I). The albumin synthesis was well preserved in the famotidine-treated animals. The noninhibitory effects of famotidine on liver regeneration are discussed.

The early effects of 4’-epidoxorubicin (Epi-Adriamycin) on liver regeneration and late effects on liver function were studied after a standard partial hepatectomy in rats. 4’-epidoxorubicin was given intravenously immediately preoperatively. Neither suppression nor delay in DNA synthesis (analyzed by incorporation of 3H-thymidine) during the first 72 h postoperatively was registered after a dose of 5 mg/kg body weight. Four weeks following the dose of 5 mg/kg a slight decrease was found in liver weight (8%), body weight (8%) and plasma albumin values (9 %) compared with the partial hepatectomized controls. Total protein, alkaline phosphatase, total bilirubin and alanine aminotransferase were not changed. A dose of 2 mg/kg did not alter any of the parameters. No histological signs of liver and kidney damage were seen. The results indicate that preoperative treatment with 4’-epidoxorubicin combined with partial hepatectomy may possibly be used in humans without clinically significant impairment of liver regeneration and function of the remaining liver and without histological detectable nephrotoxicity.
