Nitrogen balance studies during oral and complete intravenous feeding of small premature infants. Nitrogen balance studies have been performed on 15 premature infants with birth weights less than 1,200 g, who were fed by naso‐gastric tube or by total intravenous nutrition. Each regime was found to be capable of producing growth and positive nitrogen balance. The balance data obtained from this study have been used in combination with nitrogen retention data for foetuses of similar weight and gestational age, reported elsewhere, to predict the intravenous nitrogen requirement of the small premature infan

99mTc pertechnetate for detection of ectopic dysgenesis of the thyroid in childhood hypothyroidism.99mTc pertechnetate scans of the neck and pharyngeal region were performed on 29 hypothyroid children. Eleven were found to have lingual or sublingual thyroid tissue. No child with persisting ectopic thyroid tissue was intellectually impaired, but 5 of 14 athyreotic children were of subnormal intelligence. Early thyroid scanning of hypothyroid infants can be of prognostic value. In addition, all children who had become hypothyroid between the ages of 6. and 11 years were found to have ectopic glands.

Serum 170H‐progesterone estimations in the diagnosis and management of congenital adrenal hyperplasia. The results of serum 170H‐progesterone estimations performed during a 12 month period on 9 children with congenital adrenal hyperplasia due to deficiency of 21 hydroxylase are reviewed and correlated with measurements of urinary 17‐oxosteroids, pregnanetriol and 11‐oxygenation index. It is concluded that this investigation is not reliable as a sole index of control but is useful in the diagnosis of congenital adrenal hyperplasia and in its management when used in conjunction with the traditional controls. Idiopathic transient normocytic, normochromic anaemia of childhood. The paediatric literature recognises an uncommon, self‐limiting condition characterised by a severe normocytic, normochromic anaemia occurring in otherwise healthy children. Previous descriptions report erythroblastopenia as the characteristic bone marrow finding. Fifteen children are described with the essential features of this condition. Of these, erythroblastopenia was present in only seven; four showed an erythroid hyperplasia and in four the bone marrow was only marginally abnormal. The latter two groups of patients demonstrated a “shift to the left” in their erythroid precursors. Right diaphragmatic hernia in the newborn. It is uncommon for herniation of the liver through a right diaphragmatic defect to present in the neonatal period. Four infants are described who developed respiratory distress within four hours of birth and were thought to have a Group B beta haemolytic streptococcal infection. In all four the diaphragm seemed normal on the first X‐rays. Progressive herniation of the liver, which occurred during the first week after birth, initially resulted in confusing clinical and radiological findings which were ascribed to worsening pneumonia and pleural effusion. The importance of serial X‐rays of the chest and abdomen and the presence of loops of gas‐containing bowel in the right upper quadrant of the abdomen, as indicators of right diaphragmatic herniation, are emphasized. Two diagnostic techniques are described: in two infants, a right pneumothorax after oxygen‐induced pneumoperitoneum confirmed a communication between the peritoneal and right pleural cavities; in the other two infants, comparison of X‐rays taken during spontaneous breathing and forced inspiration revealed in the latter substantial clearing of the right lung field and descent of the liv

Hypercalcaemia: An unusual complication of tuberculosis. A case report. Severe hypercalcaemia was observed in a seven year old Noumean boy with mediastinal lymph node tuberculosis. Many therapeutic manoeuvres were employed initially to obtain control of the serum calcium level, which finally responded to a course of indomethacin and remained stable on prednisone and antituberculous drug therapy. It has been postulated that the hypercalcaemia is due to increased sensitivity to Vitamin D, as seen in patients with sarcoidosis, but it is suggested that prostaglandin secretion from the inflammatory tissue may contribute to the hypercalcaemia.
