LH pulsatility and response to a single s.c. injection of buserelin in polycystic ovary syndrome |

The present study was undertaken in order to determine whether patients with polycystic ovary syndrome (PCOS) have LH pulse frequency and/or amplitude higher than those in normal cycling women during the follicular phase, and, if so, to establish possible factors which might influence LH secretion in PCOS.The study was conducted on 14 PCO patients (aged 19–30 years), who were subdivided according to the data on their cycle abnormality into 2 groups: amenorrheic (Am-PCOS,n= 9) and oligomenorrheic (O-PCOS,n =5).LH pulsatility was assessed in the early follicular phase in controls (n= 5) and O-PCOS and at any time in Am-PCOS. Blood samples were taken every 10 minutes for 4 hours. Pulse analyses of LH data were performed using the Munro program. The buserelin test was performed on the same day by injection of 40μgof buserelin (blood samples were taken every 60 minutes for the following 10 hours). Eleven PCO patients and 12 control subjects had an oral glucose tolerance test (oGTT) (blood samples were taken every 60 minutes for glucose, insulin and C-peptide measurements).Both mean LH pulse frequency and mean pulse intervals were not distinguishably different in PCO women (Am and O) and controls. In contrast, the mean pulse amplitude was significantly higher in the Am-PCOS group than in O-PCOS women and controls (p<0.02 andp<0.001, respectively). A significant positive correlation was established between nadir LH concentrations and LH pulse amplitude(r =+0.966,p<0.001). The LH response to buserelin stimulation was significantly higher in Am-PCOS than in O-PCOS (p<0.004).A highly significant positive correlation was observed between LH pulse amplitude and insulin response during oGTT (p<0.001) in PCO subjects. Basal (prebuserelin) LH concentrations correlated significantly with fasting insulin levels (p<0.008) and insulin and C-peptide responses to oGTT.These results allow us to conclude the following:1. An increased LH pulse amplitude and an exaggerated LH response to buserelin observed in amenorrheic PCO subjects compared to those in oligomenorrheic PCO subjects fail to support the hypothesis of an intrinsic hypothalamo-pituitary abnormality.2. The relationship between fasting and glucose-stimulated insulin levels with LH nadir concentrations, pulse amplitude and response to buserelin suggests an etiological role of insulin in the pathogenesis ofPCOS.

We have previously reported1that human peripheral blood mononuclear cells (PBMC) contain type II estrogen binding sites (type II EBS). In this study, the fluctuations of type IIEBS during the menstrual cycle were analyzed in 6 normally menstruating women.Approximately 3 times higher levels of type II EBS were found in the period periovulatory with respect to both follicular and luteal phases. In postmenopausal women the mean type II EBS levels were similar to those observed in the follicular phase of the cycle. However, in 3 postmenopausal patients a short course of estrogen or tamoxifen resulted in a marked increase of type II EBS levels. Tamoxifen was also found to compete with 17β-estradiol for type II EBS in PBMC, although to a lesser extent than diethylstilbestrol.

摘要:
Plasma met-enkephalin (MET-ENK) levels are increased in type 1 diabetic women and in pregnant diabetic women in comparison with normal women. Plasma MET-ENK levels further increase in the peripartum period both in diabetic and non-diabetic females, probably due to the analgesic and behavioural properties of the opioid system.

摘要:
In leiomyoma and normal myometrium estrogen receptors act independently at low or high levels of the normal serum steroid range in the menstrual cycle. It might be an inherent characteristic of leiomyomas, which results in their progressive growth in the absence of any abnormal stimulation.In the secretory phase of the menstrual cycle, serum progesterone suppresses estrogen receptor concentrations in leiomyoma.In the present study serum follicle-stimulating hormone (FSH) and luteinizing hormone (LH) showed direct as well as inverse correlations with estrogen and progesterone receptors in different phases of the menstrual cycle.

To investigate the role of epidermal growth factor (EGF) in fetoplacental development, we measured the urinary and amniotic fluid EGF levels throughout pregnancy. Thirty urinary samples of nonpregnant women, 85 of normal pregnant women, 21 of women with toxemic pregnancy, 17 of postpartum women and 30 of newborns, and 55 amniotic fluid samples of pregnant women with a variety of conditions necessitating amniotomy and amniocentesis at 25–39 weeks of gestation were collected. EGF concentrations were measured by double-antibody radioimmunoassay.Urinary EGF levels of pregnant women reached their peak (24.6±6.7 ng/mg creatinine) at 19–22 gestational weeks; after that, they slightly decreased. Although there is no significant difference between the urinary EGF levels of non-pregnant women (19.0±5.1) and those of pregnant women (18.1±3.2), the EGF levels of toxemic women (12.2±1.5) were lower than those of normal pregnant women. The levels in puerperium women were similar to those found during pregnancy. However, the neonates had higher urinary EGF concentrations than those in pregnant women. On the other hand, EGF levels in amniotic fluid were higher according to gestational weeks and the levels of intrauterine growth retardation (IUGR) cases lower compared with normal pregnancy. Furthermore, EGF concentrations in amniotic fluid have a significant correlation with the creatinine levels in amniotic fluid.These data suggest that EGF plays an important role in fetoplacental development and it is possible that the measurement of amniotic fluid EGF might become available for the clinical assessment of fetal maturation.
