The purpose of this study was to explore reaching in the early neonatal period. The subjects were 23 full-term, normal newborns (mean age = 28.7 hours), delivered vaginally with little or no perinatal medication. The infants were seated and videotaped from overhead and frontally placed cameras. Arm movements made when a stimulus was in the infant's visual field were compared to those made when no stimulus was present. Overall movement path length was found to be longer in both horizontal and vertical planes when a stimulus was present (dependentttest; horizontal path,t= 2.06,p= 0.05,df= 22; vertical path,t= 3.48,p= 0.005,df= 12). Using univariate statistics (dependent (test), the mean aim of arm movements made when the stimulus was present was not found to be different from movements made during the no-stimulus trials at time of visualization of stimulus (t= −0.26, p = NS,df= 19) or in five seconds following stimulus visualization (t= −1.21, p = NS,df= 22). Stepwise multiple regression analyses, however, suggested that older infants and infants whose mothers had previous live births showed more laterally directed arm movements (toward the stimulus) in the five seconds following visualization of the stimulus than when no stimulus was present (r= 18.37,p= 0.0002,df= 2,13).

Seven children with meningomyelocele and sacral level paralysis were selected for careful study of their early motor development. The Bayley Scales of Infant Development (BSID) and the Movement Assessment of Infants (MAI) were administered at regular intervals from 2 to 18 months of age. While the mean BSID Mental Scale score tended to remain consistent and above —1 SD, the mean Motor Scale score showed a steady decline with age. Mean performance was —1 SD by 6 months and −2.5 SD by 18 months of age. MAI scores reflected hypotonia and delayed development of automatic reactions. Correlations between BSID Motor Scale scores and MAI automatic reaction scores ranged fromr= —40 tor= —77 between 6 and 12 months of age. Comparison of BSID scores of study children with other children of similar age and lesion level suggested that performance of this small sample was typical of infants with meningomyelocele and sacral level motor function.

This retrospective, longitudinal study was designed to compare the developmental outcome of 19 preterm infants who were classified as small for gestational age (SCA) with 69 preterm infants appropriate for gestational age (ACA). The subjects were infants with gestational ages of less than 37 weeks and for whom complete test profiles had been obtained from their regularly scheduled follow-up visits at 4 months, 1 year, and 4½ years corrected age at the Neonatal Intensive Care Unit Follow-Up Clinic.At both the 4-month and 1-year visits, the Mental and Motor Scales of the Bayley Scales of Infant Development were administered. A research version of the Peabody Developmental Gross Motor Scale was administered at the 4½-year visit.Statistical analyses revealed no significant differences between group performance on any of the measures taken at the 4-month, 1-year, and 4½-year clinic visits. These findings suggest that in this sample of preterm infants, the SCA group did not perform significantly differently than their ACA peers on those motor and mental measures used in this study. Interpretation of these results should be made with caution in light of several limitations of the study, including small sample size, subject selection, and assignment of gestational age.

摘要:
The focus for many years in trauma services and in childhood injury control and prevention programs has been concentrated on the reduction of mortality. Recently, a greater emphasis has been placed on the prevention and reduction of morbidity. As an integrated component of the pediatric trauma care system, the rehabilitation team, particularly the physical therapist, plays a vital role in the reduction of secondary physical morbidity due to childhood trauma. The purpose of this article is to discuss issues regarding physical recovery of injured children, the description of motor recovery, and to review physical therapy assessment models and intervention practices that are pro-ported to enhance physical and functional recovery.
