It is with great pleasure that I introduce this special issue ofPediatric Physical Therapydevoted to pediatric cardiopulmonary physical therapy. Joan Darbee, the guest editor of this issue, has pulled together an excellent group of articles covering an array of topics in this area. I'm sure this issue will be used as a resource by both cardiopulmonary therapists and pediatric therapists. Each article is an in-depth presentation of clinically significant information on cardiopulmonary conditions seen in the pediatric population. It will be apparent from your readings that the authors have given enough basic information to allow thoughtful questioning and discussion of the advanced information presented. All levels of therapists will find these articles useful. Again I thank each author for her contribution to this issue and Joan Darbee for her leadership in bringing this group together.

摘要:
The primary purpose of this paper is to describe normal musculoskeletal development of the thorax within the context of normal motor development of the newborn through 12 months of age. The second purpose is to illuminate the critical need for pediatric physical therapists to identify and treat pulmonary/chest dysfunction. Examples of diagnoses and situations that may lead to adverse trunk/pulmonary development are presented. The final purpose is to present a case illustration of a C5 quadriplegic child followed from 3¾ to 14 years of age with a successful reversal of severe chest wall dysfunction.

The pediatric physical therapist may encounter infants, toddlers, children, and adolescents with acute respiratory illnesses or chronic pulmonary dysfunction. Evaluation of these patients should include assessment for signs of respiratory distress, assessment of breathing pattern and cough, auscultation, percussion and palpation of the chest, as well as evaluation of the development status of the infant and toddler, and assessment of the strength, posture, and exercise capacity of the child and adolescent. A complete cardiopulmonary evaluation is essential in determining an appropriate treatment plan and identifying improvement or decline in the child's condition.

Cardiac surgery is successfully performed on numerous pediatric patients with many different congenital heart defects. Children have postoperative pulmonary risks similar to adults. These can be minimized with postoperative physical therapy. The purpose of this paper is twofold: (1) to review congenital heart defects and the surgical repairs, both palliative and corrective, and (2) to describe postoperative physical therapy, emphasizing the importance of early mobilization and ambulation. The physical therapy postoperative management of the pediatric heart transplant patient is also reviewed.

Asthma is the most common of all lung diseases in children, affecting 8–10% of children under 17 years of age. Children with asthma often experience school absences and restrictions in physical activities. Physical therapists have expertise in specific evaluation and treatment techniques and knowledge of respiratory function that can contribute to the overall management of the child with asthma. Asthma education for the child, family, and school personnel should be a part of the physical therapist's treatment plan. This article reviews medical treatment of asthma and discusses physical therapy evaluation and treatment, including home and school asthma education.

The purpose of this study was to describe current practice of cardiopulmonary physical therapy for pediatric patients. An investigator-designed questionnaire was published in the Spring 1990 issue ofPediatric Physical Therapy. Seventy-six respondents returned questionnaires. Sixty-two (89%) of the respondents indicated that they treat pediatric patients with cardiopulmonary dysfunction. Assessments of breathing pattern, respiratory rate, and cough function were utilized by the majority (63%-89%) of them in their physical therapy evaluations. Eight-five percent indicated that they used positioning as a cardiopulmonary treatment technique. The other treatment techniques in use by a majority of the therapists (71–76%) included breathing exercises, postural drainage, percussion, and vibration. Seventy-seven percent of all the respondents indicated that they need more training in evaluation and treatment of cardiopulmonary dysfunction. Overall, the questionnaire results conveyed that pediatric physical therapists should be acutely aware of the signs and symptoms of cardiopulmonary distress in children and be able to intervene as necessary.
