This paper summarizes the results of a study among 17-year-old students. The following questions were addressed in the study: How common is the problem of pain in the musculoskeletal system among this age group? Is there a relationship between pain and restriction or asymmetry of movement in a student's hip and/or sacroiliac joints? Can pain be decreased by use of physical therapy and an independent exercise program? Methods included a questionnaire administered to 500 students, of which 68 volunteered for physical examination and 33 participated in a physical therapy program. The treatment group was then compared with 18 students with similar complaints who were not treated. Data from the questionnaire showed that 66% of the students were experiencing pain at the time and eight percent noted daily pain in the neck, shoulders, back, hips, or knees. Of the 68 students examined, 57 complained of pain and 51 demonstrated restricted movement in one of the sacroiliac joints. Thirty-three students with complaints of pain were treated using Observational Movement Analysis. In comparison with 18 students with similar complaints, the students treated showed a significant decrease in pain measured by a Visual Analogue Scale (P< 0.01). In conclusion, it is possible to decrease musculoskeletal pain among these young people.

A critical appraisal of the literature on the effects of progressive resisted muscle strengthening as a management strategy for children and adolescents with cerebral palsy (CP) was initiated as a group consensus project by 1 7 clinical physical therapists. Seven articles were reviewed and graded using Sackett's framework. All studies reported an improvement in the performance of individual muscles or groups of muscles after a program of resisted exercise, regardless of the type of exercise or muscle group targeted. Three studies that evaluated muscle spasticity of the antagonistic muscle groups found no deleterious effects. Only one study was classified as level 1 evidence; the remaining six studies were level 5. Caution must be used in interpreting their results because of the limitations in design rigor. The group concluded that progressive resisted exercise of isolated muscle groups increased muscle performance in clients with CP, but the relationship between strength training and functional abilities remains unclear.

The purpose of this study was to determine if the severity of osteogenesis imperfecta (Ol) in childhood might have any influence on functional outcome. In a cross-sectional study, the functional outcome of 61 children with Ol was related to the three subtypes of the disease. Functional outcome was measured with the Dutch translation of the Pediatric Evaluation of Disability Inventory (PEDI). The PEDI measures capability and performance in three specific categories: “self-care, ” “mobility, ” and “social function.” We concluded that functional abilities, especially in the mobility category, were related to Ol subtype. The more severe the disease, the lower the score on mobility. In the self-care area, the score of all children ≤7.5 years of age, was within two standard deviations of the median. Despite severe disablement, children with Ol type III scored within the normal ranges. In older children with Ol, the relationship between Ol subtypes and functional ability became more pronounced. In children with the most severe disablement, there was a tendency, although not significant, for social function to be better developed. In children with a disease such as Ol, which affects posture, alignment, and growth, a severity-related functional ability profile seems to exist. Future multicenter research could further define a disease-related functional ability profile in Ol.

Provision of culturally competent early intervention services to young children and their families from diverse backgrounds has become a major concern among pediatric physical therapists. A critical step in providing competent services is to use culturally valid standardized developmental screening instruments. In this study we examined the difference between the Denver II screening results of Alaska Native children and those of the Denver II normative group. One hundred two typically developing Alaska Native children, two weeks to six years of age, were tested using the Denver II. Denver II administration and scoring procedures were followed. A convenience sample was enlisted from well-child clinics at the Alaska Native Medical Center and Women, Infants, and Children clinics in Anchorage, Alaska. Data were analyzed using logistic regression. Analysis showed that some developmental skills emerged at significantly different ages between the two groups. This suggests that the Denver II should be used with caution when screening for developmental delay in Alaska Native children.
