The purpose of this study was to determine the relationships among measures of knee extensor and flexor strength, walking efficiency, and gross motor abilities in adolescents with cerebral palsy. Seventeen adolescents (mean age 16 ± 3 years) completed isokinetic tests for knee extension and flexion (concentric–no pause–eccentric muscle action cycles at 90-degrees per second angular velocity) and walking efficiency tests (Energy Expenditure Index or EEI) on two occasions, 1 week apart. Gross motor ability was assessed using components of the Gross Motor Function Measure (GMFM) test. Although strength and walking efficiency scores tended to be reliable on one test occasion (reliability coefficients > 0.75), data averaged over two occasions were used to maximize reliability (reliability coefficients > 0.84). The EEI scores were modestly related to knee extensor strength (peak torque and work done during concentric and eccentric muscle actions), more during fast walking (r = −0.61 to −0.65; p < 0.01) than during comfortable walking (r = −0.44 to −0.50; p < 0.05). Knee flexor strength did not correlate significantly with either EEI (r = −0.24 to −0.43; p > 0.05) or gross motor ability (r = 0.37 to 0.42; p > 0.05). Gross motor ability was modestly related to knee extensor strength (r = 0.57 to 0.69; p < 0.01) and highly related to comfortable (r = −0.82; p < 0.01) and fast (r = −0.79; p < 0.01) EEI scores. Adolescents with mild cerebral palsy can produce reliable strength and walking efficiency scores. Knee extensor strength is modestly related to walking efficiency and gross motor ability in these adolescents

摘要:
The purpose of this single-subject study was to examine the effect of foot orthoses on the balance skills of a child with a learning disability who presented with abnormal lower extremity alignment and biomechanics. The experimental design was an ABAB format: baseline, intervention, withdrawal, and reinstatement. The subject was fitted with foot orthoses, and his balance skills were tested using a subtest of the Bruininks-Oseretsky Test of Motor Proficiency. A celeration line and trend analysis were used to analyze all data. Results demonstrated a significant treatment effect during both treatment conditions. The authors concluded that the improvements demonstrated by this child support the theory that the use of foot orthoses will improve the balance skills of a child with a learning disability who also has abnormal lower extremity alignment and biomechanics

This study was undertaken to gain current information on the practice of physical therapy in educational settings serving school-age children and adolescents with disabilities. Questionnaires were mailed to 259 pediatric physical therapists throughout the United States. Responses received from 155 therapists working in school systems were used to collect the data reported here. The final sample represented all 50 states and the District of Columbia. Respondents were asked their opinions concerning actual practices related to the delivery of therapy services to students in their schools and what they believe to be ideal practices in the following categories: evaluation, development of the Individualized Education Plan (IEP), service delivery, team dynamics, and administrative support. Results of the survey show the majority of school-based therapists in this sample are women, between the ages of 31 to 50 years of age, with an average of 17 years experience in pediatrics, and 8 years working in school systems. Significant differences were found between respondents' statements of actual practice and their opinions of ideal practice. The results of this survey provide preliminary information for pediatric therapists to develop policy and guidelines for the delivery of services to students with disabilities
