Cognitive referencing predicts that children with significant motor delays commensurate with their cognitive abilities will benefit from physical therapy and/or occupational therapy to a lesser degree than children with cognitive abilities, which are significantly higher than their motor abilities. This paper examines the key prediction of the cognitive referencing triage model: that IQ is a strong predictor of response to treatment. Thirty-nine children (three to six years old) with motor delays were divided into two groups (high IQ and low IQ) based on the McCarthy General Cognitive Index. All children received occupational therapy or physical therapy for nine months. Participants were posttested using the Peabody Developmental Motor Scales at the end of the school year to determine motor gains. Between-group comparisons, using repeated measures analysis of variance with time as a within-group factor and group (low IQ, high IQ) as a between-subjects factor, revealed no significant differences in Peabody Developmental Motor Scales z scores between groups. These results do not support the cognitive referencing model. Implications, including the potential for the cognitive referencing model to inappropriately exclude motor-delayed children from occupational therapy/physical therapy services, are discussed.

The purposes of this paper are twofold: 1) to familiarize clinicians with kinematic terminology, and 2) to illustrate the use of computerized motion analysis videography to examine the microstructure of movement. Three subject videotapes were chosen based on performance data from a previous study: Subject 1, accurate and slow; Subject 2, inaccurate and fast; and Subject 3, accurate and fast. The task required the child to tap reciprocally between two targets. Kinematic comparisons among the three subjects revealed differences in how the children approached the task and how they monitored the action. Discussion includes three clinically relevant postulates: 1) Individuals 'privilege' speed or accuracy; 2) Mode of control is related to the performer's cognitive strategy and resources; and 3) Use of sensory information affects the level of efficiency in performing a task.

The purpose of this study was threefold: 1) to determine the feasibility of the use of computerized sensory organization testing (SOT) with children three to seven and a half years of age, 2) examine the stability of scores obtained from this test, and 3) to determine if scores obtained delineate the maturational changes in postural control as previously reported. Twenty-three children three to seven and a half years of age, and 11 adults 21 to 31 years of age with no history of developmental, orthopedic, or neurological deficit participated in two trials of each SOT condition. Stability and strategy scores were found to be stable between trials (a ≥ 0.58). Results concurred with previous reports: 1) children were less stable and used different movement strategies than adults (p≤ 0.0001 andp≤ 0.04, respectively); 2) evidence of a transitional period between 3 and 7 years of age was noted; and 3) although somatosensory effectiveness in postural control is adult-like between 4 and 6 years of age, measures of visual and vestibular effectiveness in postural control are not adult-like by seven and a half years of age. The authors conclude that SOT provides stable, useful measures of sensory system effectiveness in, and maturational changes of postural control in young children.

In this descriptive study, the Wee-Functional Independence Measure (WeeFIM) was used to document functional change in children with cerebral palsy after orthopedic surgery and physical therapy. The study sample consisted of 20 children between the ages of four and 12 years with a diagnosis of spastic cerebral palsy. The subjects' WeeFIM scores collected after surgery were compared with WeeFIM scores obtained by a phone interview with a parent at follow-up. Despite differences in subjects' age, time since surgery, physical development, and therapeutic intervention, children with spastic diplegia showed a larger trend toward improvement in functional mobility than children with spastic quadriplegia. A trend toward improvement seemed to be greater in the WeeFIM domains of mobility and sphincter control than in the domain of self-care. The results of the study indicate that the WeeFIM may be an effective test to document change in functional independence for children with cerebral palsy after orthopedic surgery and physical therapy.
