&NA;The physical environment plays an important role in creating an atmosphere of home. Too seldom, however, are the deeper meanings of home fully incorporated into the design of dementia care settings. In this work, “home” is considered at three levels: as a psychological state, as an expression of self, and as a physical structure. Successful design must address all three levels, and must integrate the physical environment with operational and programmatic aspects of the setting.

&NA;This article describes a pilot research project designed to develop and test the efficacy of place‐based models as a foundation for structuring dementia care settings. Placebased models of care provide a holistic and readily comprehensible context for facilities structuring dementia care settings. A place‐based model of care uses the concept of place (eg, home or resort) as the organizing principle for understanding multiple dimensions of a setting including organizational, social, operational, and physical elements. Although this pilot research was based on a relatively small number of sites, results indicate that there may be a link between quality of care/quality of life and systematically applied place‐based models of care.

&NA;Place‐based models of care provide comprehensive and easily understood foundations for dementia care settings. Place Stories provide narrative descriptions of the models to assist key managers in selecting an appropriate model of care and establishing philosophy, policies, and program evaluation criteria. In this work, a Place Story of Home and the consultant's comments demonstrate the place‐based model's usefulness as a tool to help supervisory and direct care staff to assess and expand congruity of current procedures and daily routines with concepts of home.

&NA;“Connectedness,” not being or experiencing disconnection, calls forth the person with dementia's abilities, when the facilitation of creative care interventions are offered by caregivers. Being, doing, and belonging, the basis for connectedness, require care receivers to be actively involved with meaningful activity. Activity, the core component of all of life, defines the person with dementia's cognitive, physical, psychosocial, and spiritual well‐being. Success, when centered on the care receiver, involves the caregiver's empathic sensitivity; understanding of dementia's affect on the person receiving care; creativity, its implication for innovative approach and care strategies; ways to foster connectedness; enabling of activity participation; safety considerations; and the promotion of self‐satisfaction. By so honoring persons with dementia, meaningful activity upholds their heartfelt sense of connectedness.

&NA;Evaluating the safety of the home environment is an important component of clinical care for persons with dementia. This article discusses safety concerns for persons with dementia living at home alone or with family members, specific modifications to the physical environment to address these issues, and guiding principles for implementing environmental changes. A wide range of environmental strategies can be introduced to maximize home safety. Different adaptations may need to be implemented with progressive memory loss thus necessitating periodic reevaluation of the home.

&NA;The negative impact of family caregiving can be significant. Research demonstrates that caregivers are at risk for considerable burden, depression, and disruption of their social activities. Recent intervention studies, however, have shown that family caregivers can benefit from skill training programs. Elements essential in training family caregivers in behavioral management skills will be highlighted in this article, with special attention given to three areas (1) education in Alzheimer's disease and the basic concepts of behavior management, (2) a curriculum of specific behavioral management techniques, and (3) the development and implementation of individualized behavioral prescriptions. A comprehensive case study is included.
