AbstractThe spectrum of the sexually transmitted diseases (STDs) has expanded over the years. As they occur in the sexually active during reproductive years, they can affect pregnancy and its outcome, maternal health and the neonate. While morbidity affects both mother and child, maternal mortality is rare, but death can result in the baby. Those providing care to pregnant women should be aware of the presence and risks of these infections in pregnancy and consider screening those perceived as being at high risk.

AbstractAimTo compare the efficacy of a new application form of hydrocortisone 17‐butyrate (H17‐B), 0.1% topical emulsion (Locoid Crelo®) with that of betamelhasone 17‐valerate (BMV) 0.1% scalp lotion (Betnovate ® lotion).MethodsErythema, induration, scaling, pruritus and overall severity of the disease were assessed in 136 patients with moderately severe psoriasis of the scalp at the start and after two and four weeks of therapy. In addition, the effect of the Crelo® steroid‐free vehicle was assessed in 12 other patients by dry dermoscopy and squumometry.ResultsAlthough both treatments induced a prominent overall improvement, this improvement was significantly better in the H17‐B‐treated group. H17‐B emulsion was superior in the reduction of scaling and overall severity after two weeks, and reduction of scaling, induration, overall severity and total symptom score after four weeks. Cosmetic acceptability also favoured H17‐B emulsion. This is probably related to the emollient effect which was shown after a one‐week treatment with the steroid free emulsion alone.ConclusionThe innovative H17‐B emuslion is an efficient addition to topical corticosteroid therapy of psoriasis; it is especially suitable for the treatment of the scalp and other h

Serological tests for Lyme borreliosis were performed by immunofluorescence assay and flagellin enzyme‐linked immunosorbent assay. Skin biopsy specimens were taken from the periphery of morphea lesions for histological examination and PCR.ResultsAntibodies toB. burgdorferiwere detected in 3 patients andB. burgdorferiDNA was demonstrated in 5 patients.ConclusionsThe amplification of DNA with PCR analysis seems to open new prospects for the detection of Borrelia genome in tissues, in the present study we were able to demonstrate the presence ofB. burgdorferiDNA in patients with morphea, even in seronegative patients. AbstractBackgroundThe reasons for the apparent increased risk of skin cancer in Celts are unclear, but it is possible that Celts possess more risk factors than non‐Celts.AimsTo determine whether Celts differ from non‐Celts with respect of cutaneous response to UVR, and tests of cell mediated immunity.MethodsThe MED of UVB, degree of clinical photodamage, pigmentation, numbers of peripheral blood T‐lymphocyte sub‐sets, and response to a battery of infra‐dermal antigens were determined in 18 Celts with a history of BCC and 18 age, sex, and skin type matched non‐Celts with a history of BCC. Each was then exposed to 2MED, and biopsies taken of irradiated and non‐irradiated sites at 24 h.ResultsCelts tended to have a thinner epidermis than the non‐Celts. Following irradiation the epidermal thymidine labelling index in Celts had almost doubled, whereas in non‐Celts it fell slightly. AbstractBackgroundDermatological patients often suffer from psycho‐social problems in relation to their disease. These problems may affect the outcome of the treatment and should therefore be dealt with in an appropriate way. A framework suitable for the analysis of psycho‐social problems of disease exists in psychological literature, and can be applied to dermatological patients in particular.MethodsExamples of qualitative analysis are presented. The examples are based on focused, semi‐structured interviews transcribed into case stories. Four patients with long‐standing psoriasis vulgaris were studied.ResultsThe coping strategies used by patients are identified using the proposed framework. The cases show that minimising is a much used strategy by patients with psoriasis. The underlying mechanisms are analysed. The analysis can serve as background for additional psycho‐social intervention on the key points ofAppraisal and Sense of Coherence, which are the processes of recognising the personal significance of situations and the ability to understand the significance of a given situation in a meaningful way.ConclusionThe presented model of coping is simple and allows rational and explicit identification of the required mode of psychological intervention necessary in order to offer the best possible help to

AbstractAimTo investigate the outcome of therapeutic ovarian hormones on concurrent aphthous stomatitis.BackgroundStudies of therapeutic hormones in recurrent aphthous stomatitis have shown conflicting results, mostly not amenable to statistical analysis.Methods114 clients attending a well woman clinic were evaluated by means of a structured interview.ResultsWomen with menstrual cycle associated aphthae were more likely to have complete or partial remission when receiving therapeutic ovarian hormones; women with menstrual cycle unassociated aphthae were more likely to have no change or exacerbation. These differences were statistically significant (p<0.001).ConclusionsThe findings are consistent with the postulated role of ovarian hormones in the aetiology of menstrual cycle associated aphthous stomatitis.

AbstractObjectivesTo study in detail the characteristics of patients attending a screening campaign for detection of malignant melanoma.BackgroundMalignant melanoma is the malignant tumour exhibiting the greatest increase in incidence in most countries.DesignOn September 15 and 16 1990 an “open house” for evaluation of skin tumours was performed mainly at dermatologic departments throughout Sweden. In the city of Malmö such patients were investigated by dermatologists and oncologists and documented in a more detailed manner than in the national campaign.Subjects380 patients.MethodsOcular whole body examination for premalignant and malignant skin tumours. Registration of patient characteristics concerning sex, age, reason for attending. Registration of therapeutic action and possible differences between dermatologists and oncologists.ResultsThe median age of attending persons was 67 years probably reflecting the higher prevalence of skin tumours among the elderly. Three quarters of patients sought advice for a specific skin change, in most cases verruca seborrhoica. Several premalignant and malignant non melanoma skin changes were found. Altogether two malignant melanoma were found, both of the thin type and. thus, potentially curable. Dermatologists referred 10.4% of specific skin changes to surgery compared to 30.8% for oncologists.ConclusionsThe great number of attending patients reveals a great concern about skin tumours among the public. AbstractBackgroundCutaneous lymphadenoma is a recently recognized unusual tumor. It presents as an indistinct solitary non‐ulcerating dermal nodule occuring mostly in the head and neck region of middle aged persons. Its histogenetic definition is controversial and recently it has been proposed that it might represent an unusual inflammatory basal cell carcinoma. We present clinical, hislological and immunohistochemistry data of two cases. For the first time lectin‐imrnunohistochemisiry of cutaneous lymphadenoma is reported on.ObservationsOne tumor was located on the eyebrow of a 37 years old, the other on the nose of a 75 years old male patient. Slow growth over years occured. Histology showed biphasic pattern of elongated and angulated lobules with peripheral rim of basaloid cells and central network of lymphohisltocytic cells with preponderance of protein S 100‐positive cells. Peanut agglutinin (PNA) did not show peripheral staining around tumor lobules as seen in basal cell carcinoma.ConclusionsClinical and histological features as well as absence of peanut agglutinin staining argue in favor of the distinction of cutaneous lymphadenoma from inflammatory basal cell carcinoma. AbstractThis is a report of a case of multinodular keratoacanthoma (MNKA), a rare variety of solitary keratoacanthoma (KA) characterized by a collection of multiple KA nodules on the margins of a necrotic ulcerative lesion spreading centrifugally for 3 weeks in a patient with malignant non‐Hodgkin's T‐cell lymphoma.
