摘要:
The majority of reactions that occur during blood transfusion are stimulated when the recipient receives antigen from donor blood to which he has developed antibody. The reaction may be immediate or insidious in its onset. Side effects range from mild discomfort to death. The transfusionist needs to be able to distinguish symptoms of a reaction and be prepared to treat them immediately.

摘要:
The administration of any blood component carries with it the potential of an adverse reaction. Many times, these reactions can be blamed on the biological aspect of the blood. Because the antigen profile of blood varies among individuals, incompatabilities which are known as immune responses, may occur during transfusion. However, even when donor blood is a perfect antegenic match, it is possible for transfusion reactions to occur. These reactions are considered nonimmune, and may be linked to the collection, preparation or administration of the blood. Certain predisposing factors, such as liver or renal failure, patient size, or pre-existing disease may affect a patient's tolerance to donor blood. Nonimmune transfusion reactions involve not only refrigerated blood components, but also commercially prepared products. Below, nine reactions involving a nonimmune response are discussed.

Catheter-related hypersensitivity-type adverse reactions have been associated with insertion of the LANDMARK Midline Catheter (Menlo Care Inc., Menlo Park, CA). The authors' retrospective study showed an incidence for these reactions of 0.3% (7/2169) as well as an incidence of 3.3% (2/61) with insertion of CENTERMARK PICC (Menlo Care Inc., Menlo Park, CA), another aquavene-based catheter. The mechanism causing these reactions remains unknown, but the authors think that there is a small but significant risk of potentially life-threatening adverse reactions associated with insertion of these catheters. The health care professional who uses these catheters should be aware of these hypersensitivity reactions and should report any cases that occur to the Food and Drug Administration's MedWatch immediately.

Health care has been given a mandate by an educated public, managed care reimbursement sources, and the need to constantly improve patient care: to measure the quality of the outcomes of care. Multidisciplinary outcome indicators are basic to that measurement. A tool to assist with that measurement is a multidisciplinary clinical pathway.

Infusion pumps are used in the hospital setting, nursing home, and increasingly, in the home. Medical Device Reports to the Food and Drug Administration of adverse events during the use of infusion pumps for a 10-year period are described. Examples of cases reported to the Food and Drug Administration are provided. The problems reported by medical facilities (hospitals, medical centers, hospital pharmacies, or nursing homes) are compared with those that occur in the home or reported by home health care agencies. Overall, there were no differences in the types of adverse events reported by medical facilities when compared with reports from home health care agencies. However, there were differences in the location of use of some of the infusion pumps studied, which could reflect the trend toward home care over the past decade.

This research evaluates the Venoscope® for its ability to detect infiltrations when present (sensitivity) and to detect the absence of infiltrations when not present (specificity), and compares these findings with those obtained via ultrasonography. Healthy adult volunteers were randomly assigned to receive or not receive an intentionally made 5-ml normal-saline infiltration. The Venoscope® had a sensitivity of .92-.93 and a specificity of .89–1.0. Ultrasound had a sensitivity of .92-.93 and a specificity of .22–25. The Venoscope®, which is simple and easy to use, is a valid indicator of the presence and absence of IV infiltrations.
