The authors discuss the surgical correction of lower eyelid retraction in dysthyroid, socket, and blepharoplasty patients. Sixty-three patients underwent a lateral canthal tightening combined with a vertical lengthening of the posterior lamella using autogenous posterior auricular cartilage. The average follow-up was for 2 years. We propose this combined procedure as the surgery of choice in the treatment of lower eyelid retraction seen in these three groups of patients.

A case of bilateral transient visual loss following elective four-lid blepharoplasty is described. Postoperative retrobulbar hemorrhages developed sequentially and were immediately treated by decompression, restoring full vision. The critical importance of supervised postoperative observation and immediate availability of medical help is underscored. The etiology of retrobulbar hemorrhage following blepharoplasty, the steps necessary to reduce its incidence, and the treatment of this complication are discussed.

Sebaceous carcinoma of the eyelid and adnexa commonly simulates other entities both clinically and pathologically. Between 1905 and 1981, 43 patients with sebaceous carcinoma of the eyelid were seen at the Mayo Clinic. Prior to establishing the diagnosis of sebaceous carcinoma, patients commonly carried multiple clinical diagnoses (mean = 2.2) and multiple microscopic diagnosis (mean = 1.7). The most common clinical diagnosis was chalazion. The patients' average age at the onset of symptoms was 61.5, with females predominating. The upper eyelid was most commonly involved. The conjunctival surface was involved alone in a statistically greater number of patients with metastatic disease (P< 0.05). Second primary malignancies and/or radiation exposure was found in 11 patients, or 25% of our series. Of the second primary malignancies, malignancies of the skin and breast were most common. Sebaceous carcinoma should be suspect in atypical presentations of common disorders and in cases in which clinical and pathologic findings are not well correlated. If sebaceous carcinoma of the eyelid is a diagnostic consideration, we recommend a frozen section with fat stain together with a full-thickness lid biopsy. Surveillance for second primary malignancies may be warranted.

This is a report of the use of autogenous tarsus as a free graft in 121 eyelid surgical procedures. Seventy-three operations were for reconstruction of full-thickness defects and 30 were for repair of eyelid retraction. The remainder were for repair of cicatricial entropion, symblepharon, distichiasis, and irregularity of the eyelid margin. No grafts failed. There were three complications to a donor eyelid. It is believed that donor eyelid complications are less likely when the graft is harvested so as to include the upper tarsal margin. The free autogenous tarsal graft was found to be effective in repairing a variety of eyelid defects.

We performed silicone intubation for both partial and total nasolacrimal duct obstructions in adults. Twenty-three cases with partial obstructions and nine patients with total observations were untubated. The final results were good (73.9%), fair (8.7%), and poor (17.4%) in the partial obstruction group; and good (22.2%) and poor (77.8%) in the total obstruction group. Complications were minimal and did not affect the outcome. We recommend silicone intubation for partial nasolacrimal duct obstructions and dacryocys-torhinostomy for total obstructions.

Commonly used orbital decompressions are lateral wall, antral-ethmoidal, “three-wall,” and “four-wall” decompressions. Bladders filled with saline were placed in intact bony orbits before and after decompression for comparison of compliance curves by these four methods. The lateral wall decompressions produced the least volume change, while the four-wall produced the most. The three-wall decompression provided next best pressure reduction.

A simple technique of orbital implantation of two hollow spheres at enucleation or evisceration is described. The procedure sufficiently replaces lost volume in the enucleated or eviscerated orbit, facilitates prosthesis motility, and diminishes simulated enophthalmos and superior sulcus depression. The risk of implant migration or extrusion is not increased.
