An important factor in the preoperative evaluation of patients with invasive periorbital carcinomas is the presence or absence of orbital bone involvement. A retrospective study was performed to evaluate the reliability of computed tomography (CT) in detecting orbital bone invasion. Eleven patients with extensive facial-orbital basal cell or squamous cell carcinomas were evaluated preoperatively by CT and the results were compared with the clinical findings and postoperative histopathologic findings. Of the seven cases in which histopathologic examination confirmed bone and/or periosteal invasion, CT was positive in three (43%). Based on these findings, we strongly recommend that CT not be relied upon in excluding orbital bone involvement. Bony invasion may be histologically present, even though destruction is not seen on CT scans. Therefore, a normal CT scan should not deter one from removing clinically abnormal bone or periosteum and submitting it for histologie examination.

There are many reviews of pediatrie orbital tumors in the literature. Most of the series originated from the tumor registries of large pathology referral centers, whereas others are from large medical institutions and pediatrie hospitals. The distribution of types of tumors reported in each series are unavoidably biased by the nature of the evaluating facility. The findings were also affected by the geographic location of the clinical facility. We present an analysis of 141 orbital tumors in children and review 9 other series (totaling 1,229 cases) for an aggregate total of 1,370 cases. Our clinically based series had an incidence of malignant lesions of 10.6%, much lower than the 24% incidence among the combined referral center/pediatric hospital studies. Cystic orbital and vasculogenic tumors were the most common, comprising 58% of our cases and 40% of orbital tumors in children among the combined series.

Contact Nd:YAG laser surgery offers a new method of delivering laser energy to a target tissue site. Nd:YAG, a near-infrared laser, can be transmitted through an optical fiber delivery system to a synthetic sapphire probe. This sapphire probe can be used as a surgical scalpel to cut, vaporize, or coagulate tissues. The contact laser scalpel offers many advantages over conventional steel scalpel surgery and the system overcomes many of the limitations encountered with noncontact Nd: YAG lasers. A review of the history, biophysics, and experimental studies is presented. Clinical oculoplastic applications are reviewed.

Eleven female and 4 male patients presented with 20 documented episodes of acute noninfectious dacryocystic retention. This syndrome complex consists of severe pain and tearing with minimal but tender distention of the lacrimal sac, without signs of inflammation. It is caused by impaction of a dacryolith in the nasolacrimal duct. In our group of patients, the mean age when seen was 39 ± 14 years, and the mean onset of symptoms occurred at age 35 ± 13 years. Six episodes resolved spontaneously, and the rest responded to interventional therapy. In six, irrigation relieved the symptoms. One patient each responded to probing, lacrimal intubation, and primary dacryocystorhinostomy (DCR). In three patients, a new technique of percutaneous nasolacrimal duct dilatation using angiographie techniques relieved symptoms. One patient each failed irrigation and percutaneous dilatation and required secondary DCR. Recognition of this presentation permits differentiation of this syndrome from infectious lacrimal obstruction and allows appropriate therapy. In at least some patients, percutaneous nasolacrimal duct dilatation may offer both a method of accurate diagnosis and an initial alternative to more extensive surgery.

The most common cause of tearing in the pediatrie age group is blockage of the distal end of the nasolacrimal duct at the valve of Hasner. Evaluation with fluorescein using the dye disappearance test and the primary dye test, although not always as accurate as in adults, is possible. Conservative treatment is attempted initially followed by probing and, if necessary, silicone intubation. Less common causes of tearing are punctal atresia and congenital lacrimal sac fistula, which are usually diagnosed later, requiring opening of the atretic membrane and excision of the fistula tract, respectively. Amniotoceles are usually seen within a week of birth and are treated aggressively with medication and/or probing to prevent acute dacryocystitis.

In 516 consecutive dacryocystorhinostomies, a soft stent of 0.25 in. Penrose drain tubing was utilized. The Penrose drain remained in place for approximately 1 month to enhance ostium patency. Ostium patency and alleviation of epiphora were achieved in 99.4% of these cases. Patient comfort, simplicity of follow-up, success rate of ostium patency, and the low incidence of complications suggest that Penrose drain tubing is an excellent material to use when the surgeon feels that an ostium stent is indicated.

One thousand five hundred twenty-one patients (2,038 eyes) with congenital nasolacrimal duct obstruction were reviewed at The Children's Hospital of Philadelphia. Of these, 174 patients with congenital dacryostenosis involving 192 eyes underwent polymeric silicone (Silastic) intubation to treat the dacryostenosis. All patients had undergone at least several months of conservative management consisting of topical antibiotics and massage. All had undergone at least one probing and irrigation. Congenital dacryostenosis as well as resolution of symptoms were confirmed by clinical examination and use of a modified dye disappearance test. In 192 eyes the overall success rate was 83.33%. When broken down into age group, progression of success rates were from 100% in the 6–13-month age group to 79.6% in the over 24-month age group. Success rates differed significantly according to how long the Silastic tubing remained in position, especially in older age groups. The data suggest that Silastic intubation is a successful means of treating congenital dacryostenosis and that one should consider leaving the silastic in place for 6 months when possible.

Early recognition and identification of aquired immune deficiency syndrome (AIDS) ocular manifestations are mandatory if proper precautions are to be taken. Kaposi's sarcoma is a well-known manifestation of the disease; however, the initial presentation of the lesion on the eyelid is rare. Such a case is presented in order to alert ophthalmologists to this. It is suggested that all eyelid lesions in suspect patients should be regarded as potentially related to AIDS.

A 55-year-old woman presented with a mass of the optic nerve sheath suggestive of a meningioma. On excision, this proved to be amyloid. No systemic involvement could be found. Although amyloid may arise as a solitary lesion of the orbit or extraocular muscle, this “pseudomeningiomatous” pattern has not been previously noted. A magnetic resonance scan was helpful because the amyloid imaged at a density similar to vitreous and cerebral white matter, unlike meningiomas, which appear more like bone. The diagnosis of localized amyloidosis, however, still remains dependent on histopathology and cannot be made on a clinical basis.

A case of metastatic basal cell carcinoma is reported that presented as a tearing problem caused by a paralytic ectropion from a metastasis to the parotid gland affecting the seventh cranial nerve. Features of metastatic basal cell carcinoma are dicussed.
