摘要:
Diagnostic thermography is a useful test for assessment and management of low back pain syndrome patients. This paper will update the major advances in thermography since Albert et al. first reported its use in diagnosis of lumbar disc disease in 1964. This complex patient population will be discussed, with particular reference to diagnostic thermography in differential diagnosis of low back pain syndromes and specifically in lumbar disc herniation, facetitis, lateral recess syndromes, and reflex sympathetic dystrophy by specific thermal pattern recognition. Medical thermography has taken its place beside such tests as the radiograph, electromyograph, and tomography scan in diagnosis and management of patients with low back pain syndromes and is clearly beyond clinical investigation, as is supported by numerous studies in the literature.

摘要:
Despite the availability of adequate analgesics, relief from both acute and chronic pain often remains far from optimal. Physician reluctance to prescribe adequate doses of analgesics, although related to many factors, is primarily due to a fear of initiating or perpetuating narcotic dependency. Pain requiring pharmacologic intervention can usually be adequately controlled with the use of narcotic and nonnarcotic analgesics, either alone or in combination with other drugs. In management of the person in chronic pain, the recognition of the multiple factors affecting this pain is essential. Most important is the development of a healthy, nonconfrontational physician/patient relationship to allow formulation of a realistic approach to pain control agreed upon by both physician and patient.

摘要:
Three classic works in the area of ethnocultural factors on the human pain response are discussed in detail. Their results indicate that differences appear to exist in pain behavior and response between various ethnocultural groups involving pain tolerance, verbal complaints of pain, attitudes toward pain, and illness, and orientations toward the future. It is concluded that we still lack experimental studies with good algesimetric and anthropological sophistication but that there is sufficient evidence to suggest cultural differences, including religious and minority group distinctions, rather than racial differences.
