Myofascial pain dysfunction (MPD) patients, pain-free control subjects, and low back patients from a previous study were compared for visual analogue scale (VAS) responses to experimental pain (43–51°C temperatures) and clinical pain. MPD, low back pain, and control groups' nociceptive temperature-VAS response functions were all power functions with exponents of 2.1 for VAS-sensory-intensity responses, and 2.4–2.7 for VAS-affective responses. MPD patients' VAS-sensory responses to clinical pain, direct temperature matches to clinical pain, and temperature-VAS response function were internally consistent. The similar nociceptive stimulus-VAS response functions and consistent use of VAS to rate experimental and clinical pain provide a standardized procedure, whereby intensities of different types of pain can be accurately compared.

Eighteen children and young adults underwent intensive cancer therapy resulting in severe oral mucositis. Plasma and erythrocyte β-endorphin-like immunoreactivity (β-end ir) were followed serially from baseline state through the onset of painful mucositis, treatment with narcotics, and eventually resolution of the painful process. With the onset of pain and before institution of narcotic therapy, erythrocyte β-end ir showed significant reduction, with a further decrease during narcotic therapy and return toward baseline with resolution of the mucositis. Plasma β-end ir showed no significant changes. Evaluation of the erythrocyte-bound component of circulating β-end ir appears to be important when assessing changes in this endogenous opiate during pain.

Over a 2-year period, six patients with lung cancer and severe, intractable upper extremity pain were evaluated for sympathetic dystrophy. Of these patients, three had symptoms and signs consistent with this diagnosis. All three of these patients obtained complete and long-lasting relief of pain after stellate ganglion block. This suggests that sympathetic dystrophy should be considered in the differential diagnosis of intractable extremity pain due to cancer.

To determine the effectiveness of a single treatment with acupuncture-like transcutaneous electrical nerve stimulation (TENS) for relief of primary dysmenorrheic pain, 20 women received either one placebo sugar pill or 30 min of acupuncture-like TENS at four acupuncture points bilaterally. Pain levels were measured using the visual analogue scale and McGill pain rating index for up to 150 min following treatment. Results showed that the experimental groups attained a statistically significant (p < 0.05) amount of pain relief and the control group did not. The results of this study indicate that acupuncture-like TENS could be an alternative for relief of menstrual pain.

This article describes the tissue compliance meter (TCM), a simple handheld instrument for objective quantitative documentation of soft tissue consistency (firmness or softness). A rubber disc with 1 cm2surface attached to a force gauge is pressed into the examined tissue. The tissue compliance is expressed in mm of depth of penetration of the rubber disc in relation to the force employed. The depth of penetration is indicated by a ring sliding on the shaft of the force gauge. TCM can be employed as part of the physical examination and can be performed in 1–2 min. The TCM can document soft tissue pathology such as tumors, scars, edema, inflammation, spasm, spasticity, rigidity, flaccidity, taut bands of myofascial trigger points, lumps, and tumors. In addition to documenting existing pathology, the TCM provides an objective and convenient method of evaluating the natural course of the condition, improvement or impairment. The immediate as well as long-term effect of therapeutic interventions including injections, medications, physical therapy, and manipulation can be documented objectively and quantitatively.

Formal observation of the interactions between chronic pain patients and their treating physicians has been infrequently reported. Four chronic pain patients with Crohn's disease are reviewed during their course of treatment with the Mount Sinai Pain Management Service. Clinically and behaviorally descriptive findings, supplemented by markedly elevated Minnesota Multiphasic Personality Inventory (MMPl) profiles, indicate a noncompliant, manipulative, and acting-out mode of interaction between the patients and their doctors. Results are discussed in light of prior research and treatment implications for the pain specialist. Identification of and sensitivity to such potentially dysfunctional doctor–patient relationships are important in the evaluation and treatment of chronic pain itself.

Ten patients with chronic pain syndromes participated in a doubleblind, placebo-controlled, trial of L-tryptophan (4 g daily) in a cross-over experimental design. Patients were maintained on a low-protein, low-fat, highcarbohydrate diet throughout the 8-week study to insure adequate entry of L-tryptophan into the brain for conversion to serotonin. All patients reported less pain by the Pain Rating Index of the McGill Pain Questionnnaire while taking L-tryptophan. However, the degree of pain reduction was substantial in only two patients; only these two elected to continue with the treatment regimen. It is concluded that the combination of L-tryptophan and diet may be a useful intervention in selected chronic pain patients.

A multidisciplinary study was conducted at the Orofacial Pain Clinic, The Royal Dental Hospital of Melbourne, to investigate the relationship between independent and dependent (outcome) variables in patients with chronic orofacial pain and discomfort. Data were collected from a study population of 334 patients over an 8-year period. There were 251 females (75%) and 83 males (25%), with a mean age of 48 years and a mean duration of symptoms of 79 months. The mean follow-up period was 27 months. The main presenting complaints were temporomandibular joint (TMJ) disorders, atypical facial pain, glossodynia, glossopyrosis, stomatopyrosis, dental causalgia, and headaches. The correlations between psychosocial variables and outcome were modest. There was a substantial relationship between pain history variables and outcome. The correlation between outcome with number of doctor/dentists consulted was −0.54 (p < 0.005), with number of surgical interventions −0.58 (p < 0.005), and with number of dentures −0.61 (p < 0.005). The contribution of patient health-seeking behaviour and doctor/dentist behaviour to the maintenance of the chronicity of orofacial pain problems has been elucidated in this study.

Human subjects received transcutaneous irradiation on the skin overlying peripheral nerves with a low-power helium–neon laser (1 MW, 632.5 nm, 20 Hz) for 20 s to each site. This treatment was accompanied by irradiation of the skin overlying painful joints for 4–8 min according to a predetermined protocol. Control group (n = 34) received treatment by an apparatus that looked identical to the laser apparatus but emitted no radiation. Laser or placebo therapy was repeated 3 times a week for 10 weeks. Subjects in the experimental group exhibited a highly significant reduction in pain intensity. Low-power laser may represent an adjunct in the management of the pain of rheumatoid arthritis.
