Studies of midwifery programs and coronary care unit manpower use in the United Kingdom and general feldshers in two urban areas in the Soviet Union were carried out to gain insights relevant to possible revision of state health personnel licensure laws in the United States. Such revisions are under consideration because existing laws could impede innovations in manpower use. Although no specific guidelines for revision were discovered, much significant information was obtained.

This study is concerned with the correlates of two indices of practice orientations based on data obtained from a national sample of general practitioners in England and Wales. The doctor's scientific orientation is measured by his reported use of 19 diagnostic procedures during the previous two weeks. The doctor's social orientation to medicine is measured by questions which define his views on the proper scope of medical practice. Moderns (high on both measures) and technicians (high on diagnostic use) are overrepresented in smaller communities, are more likely to have bed access, to be in partnership or group practice, to have an appointment system, to take course work, and to have professional contact with other doctors. Many other findings are reported and discussed.

Within the context of a larger study of Medicare in five Midwestern communities, this report considers data from interviews with 188 physicians regarding their attitudes toward the program and their perceptions of its impact on the doctor-patient relationship and the practice of medicine. Responses to the open-ended questions revealed that most respondents (40 per cent) felt that Medicare had had no effect at all on their relationships with their patients. About 32 per cent thought the relationship had been harmed, while almost 19 per cent thought Medicare had helped the relationship. The long-term impact of Medicare on the practice of medicine was viewed by the majority (55 per cent) to be harmful, although 22 per cent thought Medicare would be beneficial and an equal proportion thought there would be no effect. Responses to the first question were based more on personal experience with patients after Medicare had begun while the latter attitude was based more on ideological arguments related to fee-for-service medicine. Generally, internists practicing in the larger cities were more favorable toward Medicare than surgical specialists or general practitioners.

The delegation of ambulatory patient care tasks by pediatricians is directly related to the types and numbers of health workers per doctor employed in solo and two-man practice settings. Physician productivity rate is directly related to patient care task delegation in settings of all sizes. Larger size settings, especially multispecialty groups, compare unfavorably with those solo and small specialty group settings which employ two or more health workers per physician both in patient care task delegation frequencies and in physician productivity. Physician productivity in smaller practice settings may be a reflection of the community demand for care in areas where medical manpower is in short supply and where the physician, motivated to respond to their demand, employs more ancillary help as a method of discharging his responsibilities. Whether similar factors influence the productivity of physicians in multispecialty groups cannot be answered from the data at hand.

Clinical departments rarely use community-wide surveys in their research programs. Yet, to study the determinants and consequences of the uneven distribution of medical care, to identify barriers to utilization and gaps in services, to plan and test the effectiveness of innovations in the organization of health care delivery, a clinical department such as pediatrics may find value in survey research studies. Such studies also provide a stimulating teaching laboratory for social pediatrics. The problems encountered in the establishment of a survey research program to study child health in Rochester, New York are reported: adaptations of sampling procedures and measurement techniques to fit the characteristics of child populations, the required organization of a field work staff, and the necessary relations to the computing center for data processing.

Frequently, one reads in the literature about “the” emergency room, as if all emergency rooms were similar and a single model would suffice to describe them all. In this interview-survey of 1,113 patients attending the emergency rooms of four hospitals in New York City, however, it was found that each emergency room served very different populations and played very different roles in the medical-care patterns of those patients. As a result of this study and of a review of the previous literature, the authors suggest that there are three major roles for emergency rooms: 1. trauma treatment centers; 2. physician-substitutes when a private practitioner or out-patient clinic are not available; and 3. “family physician” to the urban poor. These roles are not mutually exclusive and all can be present to some degree in any one emergency room.

A pilot program of comprehensive ambulatory patient care has been introduced in a typical, short-term, voluntary hospital located in an urban setting. Services, organized on a team basis, were provided to 1,200 patients during the demonstration phase. As a result of the profession and patient satisfaction with the reorganized service, the entire ambulatory patient care program is being reorganized into a series of comprehensive care units with concurrent elimination of “clinics.”
