摘要:
Three per cent of patients continuously registered in one general practice during the first 10 years of the National Health Service did not visit the doctor at all during that period. Medical and social characteristics of these people, and those who had visited recently, were studied. Broadly speaking, those who never consult their doctors appeared to be healthy.

Hospitals studied in Sweden and Israel provided far less employee–hours per patient than those studied in the Untied States. Physicians’ services were not included. Adjustment for length of stay, which differs in the three countries, is difficult to make. But even with generous allowances for these differences the variations in employee-hours are striking.

In the United States house calls account for less than 10 per cent of patient–doctor contacts outside the hospital. JVO studies have been done to indicate the best place for providing care—the home or the office. The investigation now reported suggests some of the advantages and disadvantages of each site of care from the standpoints of doctors and patients.

Illness and disability involve social processes as well as biological ones. Patterns of disease have changed, so have patterns of utilisation of services, and the new patient in the affluent society often suffers more from a variety of changes in social organisation and structure. The doctor has to perform more services for people who are not ill in the traditional medical sense. But the responsibility to provide quality care remains.
