Global economic changes, managed care and insurance issues, media forces, and new career choices for women have impacted the profession of nursing. A loss of trust underscores decreased loyalty to hospital organizations and disillusionment with nursing practice. Nursing shortages have ensued. This article reports data from a grounded theory study of nursing in hospital organizations. Strategies of respecting, communicating, maintaining visibility, and engaging in participative decision making are the transformative processes leading to relational self-organization—a shared, creative response to a continuously changing and interconnected work environment. Organizations that foster ethical choices, respect, and co-creativity rather than focusing only on fiscal and material assets become the successful organizations of the future.

摘要:
Empowering self and others promises to be a way to heal broken trust and fractured relationships with nurses. A relational theory of power development proposes ways to create empowering rather than overpowering relationships between and among nurses. This theory is based in the reflective work done through qualitative phenomenological inquiry, among other reflective work. Recognizing that all we do in health care is situated in a context, nurse administrators are encouraged to examine their assumptions (grounded in context) about health, people and work as a base for creating different scenarios to address different work situations.

A major nursing shortage is threatening health care organizations, and as the supply of available nurses is shrinking, one hospital is examining the composite of the care team and determining the personnel options for the future. This article examines how Exempla Saint Joseph Hospital approaches the redesign of a critical care team. Through clarification of roles and job prioritizing, a group of alternate care providers was developed. The ability of the registered nurse to utilize the newly clarified alternate care providers is discussed as well as the limited care management skills of the RN that may effect her success of a new care team.

The Commission on Graduates of Foreign Nursing Schools (CGFNS) is a not-for-profit, immigration neutral, internationally recognized authority on education, registration, and licensure of nurses worldwide. CGFNS was established to protect the public trust by ensuring that nurses educated in other countries who wish to practice nursing in the United States are eligible and qualified to meet licensure and other practice requirements. This article provides an overview of CGFNS certificate holders and describes the CGFNS Certification Program, which is comprised of a credentials review, a one-day qualifying exam of nursing knowledge, and an English language proficiency exam. Characteristics of foreign-educated nurses who are CGFNS certificate holders, such as age, country of education, type of education program, and English language proficiency, are also discussed.

Nursing is a rich profession with many diverse roles in many settings. The nurse consultant is often a recognized leader/expert who shares knowledge and expertise with clients who request assistance with organizational or personal challenges. Consultants report tremendous satisfaction in their roles, because of the autonomy, independence, and excitement of seeing the client benefit from the consultation. Consultants also face many personal challenges with constant travel, separation from their families, and the responsibility to keep on the “cutting edge.” The opportunities to develop a successful consulting business grow as health care evolves and needs experienced professionals to assist with difficult decisions.

Nurse leaders live in a world of intense chaos. Now more than ever, the workplace environment is undergoing impressive and radical change. Forces have left the nurse leader with more tasks at hand, more meetings to attend, and more relationships to cultivate with less time to accomplish everything than ever before. How is one to achieve a sense of balance or harmony? How can a nurse leader succeed in this environment of increasing demands? Nursing leaders are not only part of, but they are role models for today's changing work force. In order to build an engaged, healthy and productive work force, nurse leaders must first emulate these same characteristics. This article will consider consequences of increasing pressure of changing health care roles and job stress followed by a review of the relationship between the mind and the body. An examination of the influence of culture and gender follows. A strategy of prioritizing the “self” first while creating a healthy lifestyle will be discussed. Lastly, specific approaches and alternatives intended to help nurse leaders achieve this balance will be recommended.

Connectivity is the buzzword of the new millennium. We are connected to the Internet, to local area networks, and to paging systems and voice mailboxes. But are we forgetting the goal of emotional connectivity? Technology's impact on us has been twofold: along with increased efficiency comes an increasing sense of isolation and alienation among the workforce. Administrators can employ emotional intelligence to keep groups of people cohesive, through reduced dependence on electronic forms of communication and an understanding of the dynamics of the old-fashioned meeting.
