&NA;When a comatose man requiring ventilatory support for acute ingestion of phenobarbital was admitted to the University of Utah Hospital, he was managed by a multispecialty team, which included a nephrologist and a clinical pharmacologist. Since the patient could not be moved to the dialysis unit, a mobile dialysis machine was used. A negative‐pressure adapter was used in conjunction with a batch‐type dialysis machine to create a single‐pressure dialysate delivery system. Blood was delivered to the dialyzer and returned to the patient through bilateral femoral catheters that had been placed percutaneously. Pharmacokinetic studies were performed and compared to the disappearance of phenobarbital by conservative measures alone after the patient had awakened and dialysis was discontinued.

&NA;Many gastrostomy tube complications can be diagnosed with readily available radiologic technics. A review of four cases seen at one institution over a period of 3½ years demonstrates the role of this approach in establishing the presence of intraperitoneal leakage, tube migration, and catheter‐induced intussusception. It is suggested that a more frequent use of radiology with or without oral contrast agents may allow earlier diagnosis, more expeditious therapy, and decreased morbidity from tube gastrostomy complications.

&NA;Measurements of ambient oxygen concentrations were made in 22 commercial flights (707 and DC‐9 aircraft) starting at an altitude of 1,500 meters (5,000 feet) with two oxygen analyzers, one calibrated at sea level (158.8 torr) and a second for Denver's altitude. Peak “pressurized cabin” altitudes ranged between 6,050 and 8,450 feet when “cruising altitudes” varied between 29,000 and 37,000 feet. In the former analyzer, the lowest oxygen concentrations noted were about 15.2% (115.5 torr), while in the latter, they were 17.6% (133.7 torr). The possible dangers of temporary hypoxia to patients with cardiac, pulmonary, and hematologic diseases, while flying under these circumstances, are discussed.

&NA;Laboratory culture results, dating back to 1960, were reviewed at the Veterans Administration Medical Center in Oklahoma City. Nontuberculous mycobacterial incidence increased substantially during the 21‐year period as tuberculosis incidence decreased. Nontuberculous mycobacteria were found to represent 13.7% of all mycobacterial isolates, withMycobacterium kansasiiandM intracellulare‐aviumcomplex the predominant species. When the dates of first positive cultures were evaluated, a significant month‐to‐month variation was found inM tuberculosisbut not nontuberculous mycobacteria, suggesting a different mode of transmission for the two. There was no significant difference in the racial distribution or age of individuals with nontuberculous mycobacteria.

&NA;We have recently developed a Data Bank for Digestive Disorders, with primary data derived from the performance of upper gastrointestinal endoscopy. By using this system to analyze the clinical characteristics of patients with atypical chest pain, we found information from the patient's history (reflux, dysphagia, abdominal pain) useful in determining final diagnosis. Further development of the system may allow more selective use of diagnostic procedures for patients with digestive disorders.

&NA;Percutaneous transluminal angioplasty (PTA) of the abdominal aortic bifurcation was performed in three patients with atherosclerotic disease. All three patients had experienced severe claudication on exertion. The dilatations were performed with Gruntzig balloon catheters after determination of a significant pressure gradient (greater than 20 mm Hg) across the stenotic lesions. Follow‐up at 9, 12, and 23 months revealed no recurrent symptoms. Balloon dilatation of the distal abdominal aorta is safe and effective and eliminates the risk of impotence that is common after vascular reconstruction.

&NA;Five cases of paraesophageal hernia recently came under the care of the Surgical Service of the University of Maryland. Major morbidity and mortality resulted only when early repair was postponed. In contrast, elective repair was uniformly simple and safe.

&NA;Eighty‐one cases of mechanical small bowel obstruction were seen at our hospital in the three year period 1978 to 1980. Four (5%) were due to gallstone ileus, three of them in women. All four patients were more than 75 years of age. Because most patients with biliary‐enteric fistulas and gallstone ileus are elderly and in poor general condition, simple removal of the obstructing gallstone is usually adequate, but interval cholecystectomy is recommended in patients with a long life expectancy, because recurrence of calculi and symptoms increases with time.
