The results of seven laboratory tests of liver function, including spleen/liver activity ratios obtained by densitometric analysis of scans, are analyzed in 50 patients with proven Laennec's cirrhosis. In this series, the liver scan not only disclosed the liver gross anatomy and structural abnormality and established the best possible site for biopsy examination, but also, the increased splenic activity served as a useful diagnostic indication of Laennec's cirrhosis. Of 50 proven cases of Laennec's cirrhosis, 41 (82%) had abnormal spleen/liver ratios. An abnormal spleen/liver ratio in combination with abnormal results from any one or two other tests was relatively effective in the detection of cirrhosis. The accuracy is improved if the other laboratory tests are chosen from among tests for serum albumin, serum bilirubin, and SGOT. (Liver abnormalities other than cirrhosis can also present an abnormal spleen/liver ratio.) This simple determination extends the value of the liver scan commonly requested in search of metastases, primary lesions, or inflammatory processes, or in preparation for needle biopsy examination.

The development of the Doppler ultrasonic flow velocity detector has improved diagnostic accuracy in peripheral arterial occlusive disease. Survey of the peripheral vessels with the Doppler ultrasonic flow velocity detector, measurement of systolic arterial blood pressure at the ankle and arm, and exercise testing are three easily done tests which may be readily carried out in the doctor's office and which provide useful information. Noninvasive vascular testing should be in the armamentarium of all primary care physicians.

Toxic epidermal necrolysis consists of two distinct clinical entities. The Ritter type, seen mostly in children under age 10, is the severest manifestation of staphylococcal disease. A toxin has been isolated which is postulated to be the cause for clinical lesions. High epidermal necrosis occurs. Treatment is with one of the penicillinase-resistant pencillins, and prognosis is good. The Lyell type is seen mostly in adults, is drug-related, and may be the severest form of the Stevens-Johnson syndrome. Full-thickness epidermal necrosis occurs. Treatment consists of withdrawing the offending drug. Good nursing care is essential. Prognosis is guarded.

A simplified classification of the porphyrias is given which is thought to be advantageous to the anesthesiologist in determining those patients who are predisposed to acute attacks. These acute attacks may be precipitated by the administration of barbiturates, but may also be spontaneous. The current theory for the precipitation of the acute attack is described, with the probable mechanism being a decrease in uroporphyrinogen synthetase levels and the resultant interference in heme production. Increased formation of cytochrome P-450 with barbiturates also produces increased levels of delta aminolevulinic acid, which may be a cause of the acute attack. The significance in anesthesia and suggested means of anesthetic management are discussed.

In a retrospective study at the University of Louisville Affiliated Hospitals, 42 patients with carcinoid tumors not arising in the anorectal area were identified in ten years (1962–1972). The ileum was the organ most frequently involved with primary tumor (28%). The nonappendiceal gastrointestinal tumors were multiple in 28%, metastatic in 66%, and associated with a second malignancy in 25%. Of the symptomatic small-bowel tumors, 83% were metastatic at the time of diagnosis. Carcinoid syndrome was observed in only two patients, both of whom had liver metastases and elevated urinary 5-HIAA levels. Resections for cure were done on 25 patients, palliative resections on six, and biopsy on six. Six tumors were from autopsy material. Among the 24 patients treated and followed up for five years, the survival rate was only 16%. In those patients having resection for cure, the five-year survival rate was 39%, excluding appendiceal tumors. The advanced stage of disease at time of discovery and the dismal prognosis for invasive carcinoids are contrary to many clinicians' impressions of the nature of carcinoid tumors but entirely consistent with several other recent reports (James Ewing Society meeting, April 19,73).

In a single-blind clinical study of mebendazole, a new, broad-spectrum anthelmintic, 70 patients with light to severe infections of Trichuris trichiura were treated. Ten subjects received placebos, and 60 received 100 mg mebendazole twice a day for three and four days. The drug was well tolerated by the patients, and no toxic side effects were observed. Egg count comparisons using the McMaster method on feces collected before and one month after treatment indicated a cure rate for trichuriasis of 65.6% and 89.3% and a total egg reduction of 69.5% and 95.9% for three-and four-day regimens, respectively. Patients not cleared showed an egg reduction rate of 31.6% and 76.8% on the three-and four-day regimens, respectively.
