Fifteen cases of endocarditis in “mainlining” drug addicts are reported. The organism most frequently cultured in this series wasStaphylococcus, and the tricuspid was the valve most frequently affected. Attention is focused on the frequency of pulmonic disease as the presenting symptom of endocarditis in drug addicts. Emphasis is placed on early recognition of characteristic presentation, and early institution of adequate antibiotic therapy. Also mentioned is the wide range of organisms capable of producing infective endocarditis in the drug addict.

Less than two decades ago, it was first shown that progressive instillation of contrast medium into the bladder symmetrically expands the distensible part of this organ. Our experience supports this. With the use of position triangles and restraining bands, we have achieved the immobilization of the patient necessary for effective use of the technic. Correlation of our cystograms with bimanual examination in 40 patients demonstrated that bladder carcinoma at stage T2can be confirmed by cystogram. Stages T3and T4can be distinguished from stage T2, but not from each other. We believe that a fractionated cystogram is a good supplementary diagnostic procedure in clear-cut cases of bladder carcinoma. As we have modified the technic, it is simple to perform and there is no morbidity. Because stage T3and stage T4disease often require the same therapeutic approach, the inability to distinguish between the two stages is of minor importance.

Twenty-four crew members aboard a Brazilian cargo ship developed gastrointestinal and neurologic symptoms 30 minutes to six hours after eating meat from a large barracuda. Initially, the men had abdominal pain followed rapidly by frequent vomiting and profuse diarrhea. Soon after, they noticed a tingling and burning sensation of the tongue, and several had transient blindness or blurred vision. Epidemiologic and laboratory investigations showed barracuda meat responsible. Large fish have been associated with ciguatera poisoning, and although rare, the poisoning is particularly important to populations whose diets consist largely of fish.

A case of gastropericardial fistula and pericardial abscess following subphrenic abscess is presented. This particular complication of subphrenic abscess has not been recorded previously. The frequent association of serious thoracic disease with subphrenic abscess is reviewed. Gastropericardial fistula is usually caused by gastric ulcer or malignancy. A discussion of symptomatic presentation, diagnostic regimen, and proper therapy of this complication is made.

The intent of this report is to summarize our experience in treating acne with tretinoin, antibiotics, and the combination of both agents. For patients with predominantly comedonal acne or in those with inflammatory acne who for one reason or another cannot receive antibiotics, tretinoin alone is used. The majority of patients have both inflammatory and comedonal lesions. These patients are treated with combination of tretinoin plus systemic antibiotics. Full doses are employed for three weeks and then halved. Once inflammatory lesions are suppressed, the antibiotic is withdrawn slowly and treatment continued with tretinoin alone. Occasionally, reintroduction of antibiotics is required. Continuance of tretinoin allows the majority of patients to be withdrawn from antibiotics without relapse. Once antibiotics have been stopped totally, tretinoin usage is titrated until maintenance dose is achieved. Severe inflammatory acne conglobata requires full doses of antibiotics plus tretinoin for a longer period before suppression of inflammatory lesions can be achieved.

Alcoholism generally is considered a disease of people with psychologic disturbance. This article calls attention to the fact that alcohol is used nearly always for its pleasure-inducing effects. Its properties and use as a euphoriant are compared to heroin and the effects of stimulation of the theorized “pleasure center” in the brain by an electrode. The hypothesis is offered that alcohol has the quality of chemically stimulating the so-called pleasure center in most people and is used primarily for that purpose.

During a two-year period, five patients with sickle cell anemia were encountered who had a similar type of recurrent and migratory polyarthritis most commonly involving the elbow joints. The disease was characterized by severe pain of the joints, marked tenderness of the more severely involved joints, with mild to moderate fever and moderate inflammatory joint fluid response. One patient developed aseptic necrosis of femoral heads. Most of the patients had no other associated symptoms at the time of the arthritis episodes. In four of five patients, the clinical findings were limited to the joints without evidence of bone infarction. One patient had thigh tenderness at the time of the arthritis, and three patients showed radiographic evidence of previous bone infarction adjacent to the involved joints.

Case reports of five patients are presented who had obscure massive bleeding from various vascular malformations of the gastrointestinal tract. The small size and vascular nature of these lesions make the diagnosis by routine roentgenographic studies difficult or impossible. They are equally difficult to find even during exploratory laparotomy. Selective mesenteric angiography is probably the best diagnostic aid and may be carried out even if the patient is not actually bleeding at the time. Arteriovenous malformations are more frequent in the right colon. When such a lesion is suspected and all radiographic studies are negative, blind right colectomy is more apt to cure the patient than any other procedure.
