The current literature indicates that clinical neuropsychology offers a unique contribution to the assessment, treatment, and rehabilitation of patients with brain dysfunction. This paper provides an overview of the services available and the opportunities for collaborative research, as well as case examples to illustrate diagnosis and treatment.

Technologic advances now make automated anesthesia delivery systems theoretically possible; clinical implementation, however, awaits development and refinement in three areas: (1) adjunctive monitoring, which needs to be extremely sophisticated; (2) automated data collection, which will obviate manual record keeping; and (3) automated therapeutic intervention, which will control drug, fluid, and gas administration. Reduced health care costs and increased safety are the primary goals; there is also the possibility, however, of increased costs and new hazards.

To study the role of formula as a cause of rickets, we randomly assigned 46 very-low-birth-weight (VLBW) infants (≤ 1,500 gm) to one of three groups receiving either Isomil, a soy isolate formula, Similac with Iron, a common milk-based formula, or Similac 24 LBW, a hypercaloric milk-based formula designed for low-birth-weight infants during the first three to four months of life. Postnatal changes in serum calcium, phosphorus, alkaline phosphatase, and albumin were monitored during this study. Radiologic diagnosis of rickets was made in 60% of infants fed Isomil and 5% fed Similac with Iron. Significantly low levels of serum phosphorus and high levels of serum alkaline phosphatase were also seen in infants fed Isomil. The exact cause of the biochemical changes and the high incidence of rickets among infants fed Isomil is not clear. Poor solubility and possibly the decreased bioavailability of minerals in soy isolate formula may be important. We conclude that rickets occurs with high frequency among VLBW infants fed soy isolate, but not milk-based formulas. We suggest that prolonged feeding of soy isolate formulas should be avoided in VLBW infants.

The role of maternal age at birth was investigated to determine its impact on an infant's health status during the first year of life. A sample population of 112 healthy term infants born to young primiparous mothers (≤ 17 years) was compared using a chart audit with a population of 92 infants born to older primiparous mothers (≥18 years). There was no significant difference between the two groups in the number of missed clinic appointments, number of emergency room visits, number of hospitalizations, number of infants fully immunized, or incidence of child abuse. Poor weight gain and fewer outpatient visits for medical problems were more frequent in the infants of the younger population. Education programs designed for pregnant adolescents should stress the proper use of health professionals and appropriate methods of infant feeding.

Of 1,203 patients who received radiation treatment for acne vulgaris between 1940 and 1968, 302 patients were recalled and examined, 121 at Geisinger Medical Center and the remainder by their local physicians. Radiation records were reviewed on all patients. Lead-rubber and cones had been used as shielding. Mean age at the time of exposure was 21 years and mean total exposure was 692 R. Palpable nodular thyroid disease was found in eight patients (2.6%). Of these, thyroid carcinoma was detected in two patients (0.66%). Although the number of patients examined was small, the incidence of carcinomas was unexpectedly high. We conclude that follow-up examination is worthwhile for patients previously treated by irradiation for acne vulgaris.

Carbamazepine is an anticonvulsant drug in widespread use for the treatment of tonic-clonic and temporal lobe seizure disorders. Acute intoxication with this agent results in both neurologic and cardiovascular dysfunction. Neurologic manifestations may range from mild ataxia to profound coma with respiratory failure and are correlated with the plasma drug level. Cardiovascular effects appear primarily as conduction system disturbances. Treatment of carbamazepine overdose consists of aggressive orogastric lavage followed by activated charcoal administration with catharsis. Continuous electrocardiographic monitoring and close observation are necessary until plasma carbamazepine levels fall to therapeutic anticonvulsant levels. In cases of severe intoxication with respiratory failure and prolonged intubation, charcoal hemoperfusion has been reported to result in rapid improvement.

Symptoms of DaCosta's syndrome include effort fatigue and breathlessness, chest pain, palpitation, and dizziness. Considered purely functional and anxiety-related by DaCosta, the syndrome has since been related to the mitral valve prolapse (MVP) syndrome and autonomic hyperreactivity. We studied these specific symptoms in similar cohorts of 68 patients with and without documented MVP from a single practice of internal medicine and found only 6% of patients having MVP without symptoms compared to 25% of control subjects (P < .01). Palpitation was present in 71% of patients with MVP and 33% of controls (P < .001); dyspnea was noted by 50% of those with MVP and 28% of controls (P < .02), and chest pain by 44% of patients with MVP and 25% of controls (P < .01). Our results confirm reports that the symptoms of DaCosta's syndrome are more common in patients with MVP and may call the physician's attention to the proper diagnosis.

Elective coronary artery bypass can now be safely done in elderly patients. Seventy-four patients, aged 70 to 81 years (mean, 73 years), had elective primary coronary artery bypass in a community hospital between August 1978 and November 1982. Two patients (3%) died within 30 days of operation. Late deaths (range, one to 38 months; mean, 19 months) have occurred in three patients (4%). Sixty-five patients (94%) are New York Heart Association classes I or II postoperatively. These results seem to justify continued application of this operation in carefully selected patients.
