Linkage studies aimed at identifying the chromosomal loci for genes involved in schizophrenia continue to yield conflicting results. However, investigation of the optimal size and structure of pedigrees, more consistent ways of using diagnostic schema, newer highly informative probes and an international collaboration between researchers should enhance the ability to detect linkage. Important studies have also emerged that use methods complementary to linkage, such as the direct examination of candidate genes and the investigation of cytogenetic abnormalities that cosegregate with illness.

The past year's work on brain imaging in schizophrenia is reviewed. Structural studies still dominate. There is now a greater emphasis on methodology, with simple area ratios giving way to assessment of approximate volumes. Multi-structure morphometry continues to expand with particular focus on temporal lobe grey matter components and basal ganglia. There remains no clear concensus on the pattern of abnormality associated with the schizophrenic brain, the clinical correlates of structural change or the theoretical implications. There is some concensus that ventricular enlargement may antedate the onset of illness in some patients at least, though the question of progression remains open. The first reports using magnetic resonance spectroscopy have now been published, offering an exciting new dimension to the investigation of brain chemistry in health and disease.

Cocaine, alcohol, and marijuana are the most commonly abused illicit substances. This pattern holds true in schizophrenia, which appears to be a risk factor for substance abuse. Psychosis as a direct result from these substances is examined in terms of nosology and phenomenology.

Research effort in schizoaffective disorder and other functional psychoses remains directed at refining phenomenologic descriptions of these conditions. Several outcome studies provide new information on the natural course of schizoaffective disorder. Other studies provide clinical descriptions of postpartum psychosis in the developing world, and also of primary erotomania and late-onset psychosis. Leukoencephalopathy, as recorded by magnetic resonance imaging is reported as a pervasive feature of late-onset psychosis. Finally, the putative relationship between the reproductive hormones and brain dopamine is addressed in a series of case reports on postpartum psychosis.

The tension between two fundamentally different approaches to characterizing and treating psychopathology has increased during the last year. These are the traditional diagnostic-category approach and the symptom dimensional approach. Progress made in the symptom-dimensional approach has reached the point where it may now challenge the diagnostic-category approach. This challenge has been bolstered by reports that pharmacological agents have greater specificity in their cognitive effects than previously recognized. This evidence is reviewed.

Research in the neuropsychology of psychosis has advanced from the identification of differential cognitive deficits in patient groups to theoretical attempts to explain psychotic phenomena in terms of dysfunctional information processing systems. Differing levels of neuropsychological analysis in this area are outlined and a recently proposed theoretical model of the neuropsychology of schizophrenia is presented. Memory dysfuction and frontal lobe' impairments in schizophrenia are also discussed and finally, single-case study approaches, neuropsychological findings in chronic schizophrenia, cognitive analyses of psychotic phenomena and twin research are briefly reviewed.

This article reviews recent issues pertinent to the treatment of affective disorders. Clinical and research topic are considered in relation to prediction of response, treatment resistance, and prophylaxis and maintenance therapy. The status of anticonvulsant therapies in affective disorder is evaluated, and optimization of response to electroconvulsive therapy in special patient groups discussed.
