A vigorous effort is underway to detect and map genes that increase susceptibility to bipolar disorder.Improvements have been made to genetic technology and diagnostic assessment to increase statistical power and the likelihood of replicative findings. Three reports of linkage are reviewed along with a preliminary report of 'anticipation'.

This year several notable studies have advanced the treatment of mood disorders.In addition, some of the older drugs have been approved for new indications and some new drugs have been approved for use. It is clear that treatment of mood disorders must be considered in different phases: treatment of acute episodes and maintenance treatment for recurrent disorders.

Publication of the DSM-IV highlights the renewed importance of negative symptoms in the diagnosis and phenomenology of schizophrenia. Future efforts should combine longitudinal studies of the lives of afflicted individuals with a dimensional approach to symptomatology to identify various components of the disorder before linking symptoms with brain pathology.

Two advances have been achieved in molecular genetic approches to schizophrenia.The first is progress in the search for linkage across all the human chromosomes. The second advance is the discovery of a new kind of DNA mutation, unstable trinucleotide repeats, that is responsible for several neuropsychiatric disorders. Significant evidence suggests that these unstable genes may also be related to psychotic diseases such as schizophrenia. A number of candidate loci, including neurodevelopmental genes, have been tested and several regions appear promising.

In addition to replicated findings of cerebrospinal fluid space enlargement and limbic system pathology in schizophrenia, evidence is increasing of a subtle volume reduction in more widespread frontal and perisylvian cortical association areas.Moreover, in chronic patients, an increase in basal ganglia volumes, possibly caused by long-term neuroleptic treatment, has consistently been found. Pathomorphology has gained some prognostic significance for the course of schizophrenia, although the clinical usefulness of recently reported biological, neuropsychological, and psychopathological correlates of the anatomical findings remains to be established.

Distinctive profiles of neuropsychological impairment and neurophysiological abnormalities exist in schizophrenia.At the cognitive level, these impairments appear to involve various aspects of working, semantic, and secondary memory. Some of these deficits may be unresponsive to neuroleptic medication. Functional neuroimaging has indicated a pattern of cerebral frontal hypoactivity and temporal hyperactivity consistent with the cognitive findings. Taken together, these results may begin to account for the frequent and often severe morbidity associated with the disorder.

Several important developments in the pharmacotherapy of treatment-resistant schizophrenia have occurred over the past year. These developments include the resolution of questions related to the clinical application and safety of clozapine, the widespread introduction of risperidone, and new approaches to the treatment of negative symptoms and cognitive impairment. These developments will be critically examined and future challenges for the management of treatment-resistant schizophrenia will be discussed.
