Research in the past year on psychosocial antecedents of depression has highlighted the importance of interpersonal factors, among which distal interpersonal experiences in childhood are increasingly becoming a focus of attention.It is worth noting that the social meanings of these early experiences are often paralleled by more recent experiences immediately before depressive onset.

Premenstrual dysphoric disorder has been categorized under mood disorders in the DSM-IV as depression, not otherwise specified, although the research diagnostic criteria for premenstrual dysphoric disorder are listed in the appendix.Postpartum depression and psychosis have been categorized under mood disorders in the DSM-IV as course modifiers. Support for the categorization of these illnesses as mood disorders is increasing.

Advances have been made in several areas of second messenger and signal transduction research in mood disorders.Evidence from postmortem brain and clinical studies which implicates disturbances in signal-transducing guanine nucleotide binding proteins in bipolar disorder is discussed as well as results of studies suggesting that mood stabilizing agents, particularly lithium, act on intracellular signaling targets. Findings supporting disturbances in inositol phospholipid and intracellular calcium signaling systems in affective disorders are also described.

Service-delivery research is reviewed with specific emphasis on the economic impact of depression on the health system, nonclinical factors influencing treatment, current management practices in the primary care and speciality sectors, and efforts to improve depression management in primary care. Special emphasis is given to randomized trials of new service-delivery programs.

Treatment optimization has focused on methods for the determination of electrical stimulus dosage and comparison of different schedules of treatment frequency.Seizure duration is less likely to be used as a general measure of treatment adequacy. Substantial progress has been made in the management of cardiac side effects and the delineation of risk factors for persistent retrograde memory deficits.

Genetic studies have provided suggestive evidence for loci conferring susceptibility to schizophrenia on chromosomes 6p and 22q.The possible involvement of unstable DNA sequences (dynamic mutations) has also created interest. Studies of in-utero exposure to influenza epidemics as a risk factor have produced conflicting results. The evidence for a relationship between obstetric complications and schizophrenia has been strengthened, but its meaning remains unclear.

Schizophrenia clearly involves changes in the structure and functioning of the brain, particularly in the cerebal cortex.This has been established by many computed tomography, magnetic resonance imaging, single photon emission computed tomography and positron emission tomography studies. Earlier studies have typically employed small sample sizes and the selection of controls is important in determining whether significant differences are found. For this reason it remains tantalizingly unclear which parts of the brain are structurally and functionally abnormal. This year's publications are making significant inroads into these issues by employing large sample sizes with careful matching of patients and control individuals, together with increasingly sophisticated methods of image analysis. The evidence looks increasingly strong that schizophrenia involves abnormalities of fronto-temporal connections involving amino acid neurotransmitters and that some of these changes may arise from impaired development of normal brain asymmetries.
