Linkage studies provide significant evidence for loci conferring liability to schizophrenia on chromosomes 6p, 8p, and 22q with partial replication by others.There is suggestive evidence for loci on the short arms of chromosomes 3, 9, and 20. Multigenic inheritance is likely to underlie susceptibility to schizotypal personality traits and related psychoses.

Over the years, a large number of psychophysiological deficits have been demonstrated in schizophrenia.The greatest challenge is to progress beyond studies of phenomenology to investigations of etiology. The ability to combine psychophysiology with neuroimaging, along with the development of animal models, now permits psychophysiology to test specific etiological hypotheses in schizophrenia.

The science of neuroimaging has added to our knowledge about the biological substrate for schizophrenia.Despite this, the daunting complexity of neuroimaging findings makes interpretation difficult and firm conclusions about the cerebral structural and functional deficits in schizophrenia seem as far away as ever. Nevertheless, the past year's literature holds tantalizing clues to future breakthroughs and increasing evidence supporting dominant hemispheric dysfunction localized in the temporal lobes. The use of larger, well characterized, sample populations has allowed further analysis of the relationship between imaging parameters and symptomatology. The differentiation of neural tracts connecting the brain regions thought to be involved has enabled experimenters to generate more sophisticated hypotheses to drive their research. In addition to this, the advent of more powerful image analysis algorithms offered the opportunity of automated analysis of large high-resolution data sets and the ability to combine easily images from different modalities. These developments may allow more consensus over image acquisition and analysis between groups and thereby results that are easier to replicate. The conclusions that can be drawn from the past year's literature are few. The case for abnormalities in the neural pathways connecting subcortical to cortical and cortical to cortical structures is stronger. The understanding of the neurochemical basis of the aetiology and treatment of schizophrenia is burgeoning.

Recently published research on the neuropsychology of patients with schizophrenia has improved our understanding of their memory, attention and executive function deficits, as well as possible ways to remediate those deficits.We have also seen a shift towards symptom-specific neuropsychological research to account for the heterogeneity of schizophrenia. Throughout this review we will comment on progress in the development of neuroanatomical models which account for neuropsychological deficits in schizophrenia.

Studies confirm the negative syndrome in schizophrenia as a distinct dimension of psychopathology on both empirical and theoretical grounds.Current interest in negative symptoms centres on their possible prognostic significance; claims are made for a modest but direct effect on persistent negative symptoms with the newer antipsychotic drugs. Studies exploring the cognitive abnormalities underlying particular negative symptoms continue to implicate dysfunction of the prefrontal cortex and its subcortical connections.

Research into psychosocial management in schizophrenia is witnessing a new phase in family approaches with studies sufficiently differentiated to permit distinction of the effective ingredients and yet sufficiently similar to allow for meta-analysis. Individual patient work has begun to benefit from cognitive behaviour therapy approaches and the centrality of medication compliance is increasingly clear. The challenge is to devise and research interventions sustainable in routine practice.

Outcome studies attempt to improve understanding of the evolution of schizophrenia over the lifespan.Vulnerability markers often predate illness onset and, along with negative symptoms, appear to endure across illness epochs. Disabilities and treatment resistance may accrue early during the course of illness. Suggestions that timely pharmacological or psychosocial interventions can alter the natural history of schizophrenia have rekindled interest in early detection and prevention.
