Kraepelin's unitary concept of affective illness was challenged in the 1950s by a group of researchers proposing the subdivision of depression into a series of binary classifications. Endogenous/reaction, psychotic/neurotic and unipolar/bipolar, thus became the most widely used diagnostic categories. Recently published research, however, has tended to expand the bipolar spectrum to include most forms of depression with regard to variables such as course, family loading of mood disorders, temperament, and response to treatment. Nevertheless, it is important to distinguish between the symptoms of various subtypes of disorder in order to facilitate genetic and biological research, as well as treatment planning.

Recent research on seasonal affective disorder (SAD) has refined the diagnostic criteria and highlighted the heterogeneous nature of the disorder. Recent light-therapy studies have shown improved methodology, but some negative studies have added to the controversy over non-specific (placebo) effects of light exposure. Several psychobiologic studies have generated interesting data, but the pathophysiology of SAD remains elusive and unconfirmed.

摘要:

Child and adolescent depression research is still at an early stage of development. Current strategies owe much to insight derived from studies of adults, but important differences between depressed children and adolescents and depressed adults are emerging. These differences are increasingly identified in studies of neurobiology, course and treatment outcome. Future research should continue to borrow the best from adult studies but should, in addition, address our understanding to the developmental^ specific aspects of depression in the younger population.

Depression, whether syndromal or subsyndromal, is a common but treatable source of excess disability in late life. It is important to take a long-term view of the treatment of depression in the elderly, particularly with respect to maintenance therapy, because of the high risk for relapse, recurrence, and chronic invalidism. Special populations of elderly people, such as those with medical and neurological illness, or those with bereavement-related depression, also merit greater research attention.

Several predictors of time to (or rate of) recovery from a depressive episode and of time to (or rate of) recurrence of depressive illness have been discussed in the literature. This article summarizes the most recent research in this area, and those studies fulfilling a minimum set of methadological requirements.

摘要:

Recent studies pertaining to psychological models of depression, vulnerability to depression and psychotherapeutic outcome studies are reviewed. Points of convergence are emphasized and it is concluded that these three areas selected for review cannot progress without more theoretical development and more collaboration and communication across disciplines.
