Twenty-seven children who survived total correction of Fallot's tetralogy are reviewed and 14 with suspected complications were reinvestigated by cardiac catheterisation and contrast studies. There were three late deaths. Of the remaining 24, 16 are well and have no evidence of complications other than pulmonary incompetence. One child has moderate residual pulmonary stenosis; there has been some reopening of the ventricular septal defect in six, all with a residual right ventricular/pulmonary artery systolic gradient of between 20 and 53 mm Hg (one has since had further surgical repair). One child developed marked pulmonary incompetence.The plain X-ray and angiocardiographic appearances are described. Dilatation of the infundibulum of the right ventricle was an important feature. It varied from a slight convexity to aneurysm formation. It gave a reliable indication of persistently raised right ventricular pressure, indicating persistent outflow obstruction, reopening of the ventricular septal defect, or both. A rapid increase in size was followed by death in one, and by successful re-operation in another child.The appearance of the pulmonary valves on angiocardiography was unreliable in the diagnosis of persistent pulmonary stenosis following operation unless there was distinct doming of the valves. Recatheterisation is the only certain method of detecting significant residual stenosis.Distortion of the appearance of the left ventricular outflow tract in systole appeared to be without functional significance.

The rapid injection of 50 c.c. of 76% Urografin in the investigation of 58 patients with hypertension of suspected renovascular origin was seen to cause, on the average, an increase in the size of the kidneys measured on the intravenous pyelogram. This increase was approximately 0·5 cm at its maximum. An explanation for this size variation is offered.

We have described three cases of crico-pharyngeal web not associated with iron deficiency, discovered on routine barium swallows in the thyroid clinic.It is suggested that there is an association between cricopharyngeal web and thyroid disease. An association which, as far as we know, has not previously been commented upon. We are unable, at the present time, to explain this association.

A radiological classification of diseases must cope with a number of exacting demands which make conventional clinical classifications impractical. The present system of classification is designed to meet these demands and facilitate indexing of teaching and research material. It also serves for immediate communication of the radiological interpretation, thus obviating repetitious and often unnecessarily conflicting review of films. The classification remains brief and yet comprehensive by separating anatomical structures, aetiological agents and radiological forms, the components of all disease entities. Each component may be specified by a readily memorised letter and number. Appropriate components are combined to compose each specific diagnosis.Cases may be coded in greater or lesser detail according to departmental policy or the requirements of the individual radiologist. In the majority of cases anatomy and aetiology suffice to define a disease. Thus a colon polyp is A8g1 and pneumonia is R5b1. Many special radiological features may be coded in addition to the disease itself. The code may be recorded on the film envelope for immediate communication purposes or on index cards for permanent record and future recall.The classification fulfils a practical need that has long been felt by academically inclined radiologists. It should lead to more effective recall of valuable clinical material which might otherwise be lost. It should also facilitate the efficient day-to-day operation of the Department of Radiology.

A technique is described for the irradiation of the whole brain and spinal cord in the treatment of medulloblastoma and ependymoblastoma. The necessity for shielding part of the posterior X-ray field for a proportion of the treatments, as in earlier irradiation schemes, has been eliminated by modifying the field using a specially designed wedged copper filter. The filter also enables the isodoses to follow approximately the contour of the spinal cord, and to bring the isodoses in the head into line with those arising from an angled cobalt field applied anteriorly through the forehead thus producing uniform dosage throughout the brain. Two such filters, of adjustable length, which can together accommodate a wide range of patient sizes are described in detail. Isodoses for the filters and for a typical patient are included and a method for quickly providing isodoses for any patient is outlined. A special “setting-up” jig for the posterior field facilitates quick and accurately reproducible positioning of the patient and provides immobilisation during irradiation. Clinical reports of the seven patients treated so far are included and, although the follow-up is too short to produce survival figures, only one of the seven has died, this patient being a case of ependymoma.

The spectral distributions of electrons ejected from plane slabs of compact human bone and Lucite have been measured by a magnetic electron spectrometer at various emerging angles less than 70 deg. for 150–300 kVp X rays. Calculations based on the measurements have provided data for the mean energy and the effective range of secondary electrons. The results have a bearing on the problem of the absorbed dose in soft tissue at a bone-tissue interface.
