摘要:
On February 11,1982 The British Institute of Radiology received its greatest honour, when Her Majesty the Queen graciously visited us. She came, as Patron, to open our new home at 36 Portland Place, but her visit was far mbre than the unveiling of a plaque: with characteristic generosity of interest, she examined and commented on almost every part of the refurbished house and commended it. It certainly did look elegantly splendid as well as functional and that was the main theme of our President's remarks in showing her around the building. The readiness with which Her Majesty discussed the modern scientific and historical exhibits prepared for her was evidence of her wide understanding of the aims of science in medicine and her informal and relaxed conversation during tea showed us how well she appreciates the financial pressures under which the Health Service and medical research try now to operate. We all felt that our Patron was our appreciative and encouraging leader. A letter (reproduced here) received the next day from Sir Philip Moore, her Private Secretary, will be treasured by succeeding generations.

AbstractWith the aid of cineradiography the pharyngeal stage of deglutition was examined at a speed of 50 and 100 frames per second among 150 individuals without dysphagia. In 124 (83%) the act of swallowing had a symmetrical and synchronous pattern. Different types of aberrations from this pattern were registered in 26 individuals (17%). In seven individuals the epiglottis did not tilt down properly but remained in a transverse position during deglutition. In eight individuals, contrast medium passed into the laryngeal vestibule. Seven individuals had a dysfunction of the cricopharyngeal muscle which did not relax properly. One individual had a web in the upper oesophagus. In all but two individuals the aberrations were isolated and not concomitant with other abnormalities of the act of swallowing.

AbstractIn order to define the diagnostic outcome of cineradiographic examination of the swallowing act (pharyngeal stage) in patients with dysphagia, 250 such patients were consecutively examined at filmspeeds of 50 and 100 frames per second. Synchronous and symmetrical swallowing was observed in 20% of the patients while 80% exhibited functional abnormalitiesi.e., epiglottic dysfunction (33%), misdirected swallowing and aspiration (41%), cricopharyngeal dysfunction (22%), paresis of the pharyngeal constrictor musculature (10%), webs (15%) and Zenker diverticula (2%). These results clearly indicate that cineradiography can frequently disclose functional abnormalities in the act of swallowing among patients with dysphagia and explain the symptoms of these patients. Cineradiography is clearly superior to conventional single-film technique in revealing functional abnormalities in the act of swallowing.

AbstractThe aim of this study was to establish the effects of two iodine-containing radiological contrast media (ioxaglate and diatrizoate) on plasma iodide, thyroid hormones and TSH in a group of 20 subjects (8 females, 12 males). The following were evaluated two, eight and 30 days after administration of the drugs: plasma iodide, T4, T3, rT3, FT4I, TSH before and after TRH (250 μg IV).131I uptake was measured before the media were given and 30 days afterwards. The results were compared at the different times during the investigation using Wilcoxon's signed ranking test for non-parametric variables. Plasma iodide was found to be above the normal range two to eight days after ioxaglate and two to eight and 30 days after diatrizoate. After ioxaglate, rT3 was increased at eight days and T3 at 30 days.131I uptake remained low after the 30th day. After diatrizoate, T4 was decreased on the second day and FT4I was low on the eighth and 30th day. All figures represent statistically significant but nevertheless minor variations of the initial value of each parameter.In conclusion, although ioxaglate and diatrizoate lead to variations in plasma thyroid hormone levels, they are of little importance.

摘要:
This excellent book is part of the series “Tutorials in Postgraduate Medicine”. With an ever-increasing number of reviews in gastroenterological sujects, it is refreshing to find a compact book which should provide a complete reference work for a number of years to come. Its authors are all well-known authorities and, as the book is edited by an anatomist, this aspect is particularly well covered.

AbstractHypertonic solutions, such as contrast media (meglumine/sodium diatrizoate-76%; Renografin-76), induce vasoconstriction in the renal vascular bed via an unknown mechanism. We assessed a possible role of two vasoconstrictor hormones known to be released by the kidney, angiotensin II (AII) and the prostaglandin, thromboxane. Specific inhibition of AII at the smooth muscle receptor level by saralasin (0.33–0.75 μg/kg/min, IA) did not attenuate the response. Inhibition of prostaglandin formation by indomethacin (4 mg/kg, IV), likewise, did not prevent the decrease in renal perfusion. We conclude that neither All nor the prostaglandin system mediate the contrast medium-induced renal vasoconstriction.

摘要:
This book consists of 8 chapters, each being the edited version of invited papers given at a symposium organized by the HPA. The first 4 are devoted to instrumentation, and cover the accuracy required in surveys, the calibration of monitors for dose, dose-rate and contamination and a short evaluation of available health physics instruments. The remaining 4 deal with the practical application of monitoring in real-life situations: with X- and γ-ray installations, environmental monitoring when unsealed sources are used, wipe testing and leak testing and the monitoring of airborne contaminants.

AbstractIt has been previously suggested that glucagon improves the bile duct and gall-bladder opacification obtained by infusion cholangiography.One hundred and ninety consecutive patients referred for intravenous cholangiography were entered into a double-blind study designed to see if glucagon would in fact significantly improve bile duct opacification. All patients received a one hour intravenous infusion of iotroxamide at a rate of 3.15 mg/kg body weight/min.Glucagon did not enhance bile duct opacification, the mean post-injection scores for the two groups being 2.47±1.11 and 2.29±1.15 respectively. On the basis of the present study we cannot recommend the routine use of glucagon following infusion cholangiography.
