Although nurses describe their professional uniqueness as care of the whole person, the spiritual needs of patients often have received little attention. Therefore, this descriptive, cross-sectional survey was designed to investigate the spiritual health of oncology patients and how well oncology nurses assess spiritual health. To achieve these aims, parallel nurse (r = 0.92) and patient (r = 0.89) Spiritual Health Inventories (SHI's) were distributed to a convenience sample of 40 nurse-inpatient pairs from two hospitals. Twenty three patients with primary lung cancer and twenty seven registered nurses responded (n = 50). Analysis of SHI scores of the 21 nurse-patient pairs indicated that these nurses inaccurately assessed their patients' spiritual health (p < 0.05), and that patient and nurse subjects preferred different spiritual caregivers. Patient respondents reported a normatively high level of spiritual health, positively related both to age (p < 0.02) and physical well-being (p < 0.014). Afro-American and Caucasian nurse respondents rated the spiritual health of patients higher than nurses of Asian origin (p < 0.006).

Patients with therapy-related acute nonlymphocytic leukemia present many challenges to health care professionals, yet it is only through the success of cancer management that we have uncovered this rare and unfortunate issue of survivorship. Oncology nurses have been at the forefront of symptom management, pain management, and other important issues in the care of cancer patients, and with a broader under-standing can also emerge as key players in the identification and management of patients with second malignancies.

The purpose of this study was to compare the incidence of infection in patients having venous access ports (VAP) accessed using commercially prepared kits with patients having VAPs accessed using a nursing protocol developed by RNs in the outpatient chemotherapy unit. The RN-developed protocol allowed venous access devices to be approached without sterile gloves and with minimal equipment, a technique which is more time efficient and cost effective. A convenience sample of 26 outpatients was randomly assigned to two groups. Patients in Group I, accessed using the commercially prepared kits, were accessed a total of 69 times. Patients in Group II, accessed using the nursing protocol, were accessed a total of 102 times. The two groups were compared for occurrence of infection as identified by elevated white blood count (WBC); febrile episode (> 100.4°F); or drainage, pain, redness, swelling, or warmth at the port site. Data collected over a period of 6 months indicated no documented infection in either group. Results from this study will be used to support changes in nursing practice.

Many agents used in cancer chemotherapy act on a specific phase of the cell cycle and are classified as cell cycle-specific. Some of these cell cycle-specific drugs, the antimitotics, interact with tubulin, the major protein of mitotic spindles, and cause metaphase arrest, thus halting mitosis. Understanding the dynamic processes involved in tubulin polymerization/ depolymerization and how chemotherapeutic agents disrupt these processes is fundamental to understanding and anticipating common adverse effects and planning nursing interventions.

The world-wide nursing shortage is resulting in institutions all over the world recruiting nurses from other countries with varying nursing skill levels and cultural backgrounds. The purpose of this article is to provide a framework for international institutions to adopt, to ensure continued quality care for oncology patients. At King Faisal Specialist Hospital & Research Centre (KFSH&RC) in Riyadh, Saudi Arabia, nurses have been recruited from over 25 different countries. Many of these nurses have never cared for oncology patients or given chemotherapy. A framework combining the strategies of planning and problem-solving was utilized to operationalize this unit in a multicultural setting. Cultural variants related to caring for Saudi patients by a multinational work force with differing cultural backgrounds as well as differing educational programs for nurses was reviewed in terms of their impact on staffing an oncology unit. An oncology/chemotherapy certification course was developed to document the competency of these multinational nurses to administer chemotherapy and to provide safe quality care to oncology patients.

Over 5 million persons are living today with a diagnosis of cancer. Maintaining employment after a diagnosis of cancer and even during therapy is a major challenge for an increasing number of individuals. The majority of preretirement persons with cancer who have attempted a return to the workplace have confronted a variety of issues and barriers. Social scientists and legal and health care professionals have attempted to identify particular practices and issues that have af fected the return to work experiences of those with a diagnosis of cancer. There has not been a program in nursing research that has systematically tested clinical interventions which effect return-towork outcomes in persons with cancer. Understanding the return-to-work experience is an essential prerequisite to planning clinical nursing therapeutics and assistance to persons with cancer who consider maintaining their worker role. Environmental and personal factors that influence a successful return to the work-place, plus the human responses manifested in the return experience are reviewed in this article. Clinical implications are drawn for nurses who can facilitate the return-to-work for an individual with cancer through interventions that eliminate or alter barriers to the successful return.

We used a needs assessment questionnaire to survey a primarily adult oncology population and their significant others, to gauge their interest in a cancer screening program. The purpose of the study was to determine the feasibility of developing a cancer prevention and detection program for this group of individuals. Although there was overwhelming interest in participating in the program, the subjects held varied opinions about the program's benefits. Differences correlated with personal and familial history of cancer and, in some categories, were quite significant. Additional questionnaire information related to patients' preference in the design of a cancer screening program.

Thirty-nine relapse free testicular cancer patients were interviewed 1–6 years after completion of chemotherapy, in connection with a study of conditioned nausea. Some structured questions about experiences of diagnosis and treatment were included, but this report also summarizes spontaneously given information. The patients' first contact with a treatment setting appears to be an important determinant of the overall treatment experience. If resources are limited, they are used effectively if concentrated during the initial contact with the patient. Support from relatives and friends was reported as good for 90% of the men during treatment. Thirty-two distressing factors during the treatment period were mentioned; the most severe were nausea and vomiting. Other high-ranking factors were “being restricted for the infusion” and “the smell of food.” There is a need for flexibility in ways to prepare and serve food on the ward. Some men argued for more information concerning the disease and chemotherapy; others said they received too much information. Several men suggested that a debriefing talk with the physician and someone from the staff should be arranged 6 months to 1 year after completion of treatment.

This study was designed to identify registered nurses' knowledge and beliefs regarding breast cancer in elderly women, and to identify the practices of registered nurses in performing breast exams and teaching breast self-examination (BSE) to this client group. Correlational analysis using Pearson moment correlation showed a weak inverse relationship between nurses' attitudes toward the elderly and performing and teaching breast exams. As the nurse's negativism about aging increased, the incidence of teaching and performing exams decreased. A strong relationship was found between thorough and regular examination of the nurse's own breasts and performance of the exam on elderly women. As predicted, nurses were more likely to perform than teach exams. Using logistic regression, it was found that nurses who were employed full-time, married nurses, nurses who re-ported encouragement for themselves to do the exam, and nurses who were confident in examining them-selves were more likely to examine the breasts of the elderly women. Nurses were more likely to teach BSE when they had a family history of breast cancer, spent > 50% of their time on direct care, reported belonging to a nursing association, were confident in detecting a lump in their own self-exams, and had a baccalaureate degree or higher.

The purpose of this study was to analyze the opportunities and willingness of women to take care of themselves after an operation for breast cancer. The expectations and experiences which women had about their clinical supervision and support during their care were studied. The sample group consisted of one-sixth of the ≈600 women in southwestern Finland who had developed breast cancer in the previous 3 years. A questionnaire was sent to the subjects. Statistical analysis of the data was based on percentage distributions with correlations, cross-tabulation and log-linear models. The patients reported that they were willing to take part in their own care but that, unfortunately, at all levels they received insufficient professional support and information. We concluded that our health care system insufficiently supports patients' recovery.
