摘要:
&NA;Nineteen children whose sibling participated in a home care program for dying children and 17 children whose sibling died in the hospital were interviewed 1 year after the death. Questions were designed to assess three time periods: the weeks immediately preceding their sibling's death, the time of the death itself, and the year following the death. Children who participated in home care described a significantly different experience than those whose sibling died in the hospital. The majority of home care children reported that they were prepared for the impending death, received consistent information and support from their parents, were involved in most activities concerning the dying child, were present for the death, and viewed their own involvement as the most important aspect of the experience. Non‐home care children generally described themselves as having been inadequately prepared for the death, isolated from the dying child and their parents, unable to use their parents for support or information, unclear as to the circumstances of the death, and useless in terms of their own involvement. The relationship of these perceptions to subsequent adaptation is discussed and provides insights into the factors which facilitate more positive adjustment for children following their sibling's death at home.

摘要:
&NA;Persons with precancerous Dysplastic Nevus Syndrome (DNS) who are members of hereditary cutaneous malignant melanoma (HCMM) families have an increased risk of developing malignant melanoma, a uniformly fatal cancer if not detected early. This article is the second of two papers which present the results of a study conducted to: 1) describe the impact of DNS/HCMM on certain aspects of family life as reported by family members, 2) determine family members' health practices related to prevention/early detection, and 3) determine the educational needs of family members in relation to DNS/HCMM. This paper also describes the health practices and educational needs of individuals in families with DNS and HCMM. The sample consisted of 48 family members who participated in telephone interviews. The majority of respondents had accurate knowledge about DNS and HCMM, but educational needs were identified related to methods to avoid sun exposure, differences between dysplastic and normal moles, melanoma danger signs, and hereditary aspects of DNS/HCMM. Although the majority of individuals were engaging in recommended prevention/early detection practices, there appeared to be deficiencies and confusion about skin examination techniques and methods to decrease sun exposure. Significance for nursing includes an awareness of relationships between perceived susceptibility to disease and health‐seeking actions, recognition of the value of an individualized teaching program stressing self‐responsibility, and the potential use of such a program as a model for other groups at high risk of cancer.

摘要:
&NA;Two areas of importance to the delivery of health care which have received inadequate attention are the influence of social support on health outcomes and the social support characteristics of culturally diverse populations. The Norbeck Social Support Questionnaire was used to determine the nature and perceived quality of social support of cancer patients living in Egypt. A sample of 40 hospitalized subjects were interviewed by a health care professional. The amount and sources of support for this sample are described and these findings are compared to the normative data reported previously for American employed adults. The mean age of the Egyptian cancer patients was 45 years, approximately 57% were female, and 90% of the subjects were married. The mean number of persons identified as providing social support was 14.78; the reported frequency of contact and duration of relationship of these network members suggest a stable support network. Family members/relatives was the category of persons providing the greatest proportion of support. The amount of functional support (affect, affirmation, and aid) reported suggests a relatively high perception of social support available to these Egyptian cancer patients, particularly when compared to the normative data reported for the United States sample of employed adults.

摘要:
&NA;Implementation of the hospice philosophy of care is in its infancy in the United States. The success of the concept relies heavily on the quality of individual patient care. It is for this reason that a descriptive study was undertaken to determine the strengths and needs of registered nurses, licensed practical nurses, and nurse's aides working with the home hospice patient on aper diembasis. The findings of this study showed that the registered nurses were more comfortable than the licensed practical nurse and nurse's aides regarding physical care, but they lost this lead in the psychological area. The three groups had a fairly good understanding of the hospice philosophy, although aides needed more education in the area of medication and pain management. Registered nurses and licensed practical nurses also showed a need for improvement in attitude toward family participation in patient care.

摘要:
&NA;The fear of breast cancer and subsequent mastectomy is overwhelming in our society today. Today's women are grapsing for an alternative to mastectomy with the same chance of survival. A lumpectomy followed by radiation therapy may be the answer to preserving the breast, as well as curing the cancer. The popularity of this treatment for early breast cancer is growing and the nurse must be knowledgeable in the procedure with its nursing interventions, side effects, and follow‐up care. This article will provide an understanding of the selection criteria for treatment. The rewards of saving the breast should encourage the teaching and practice of monthly breast self‐examination to enhance early detection of a possible malignancy. Ninety percent of all breast cancers are discovered by women themselves. Seventy percent of lumps found are benign.*
