This article reviews precancerous lesions of the uterine cervix and focuses on personal factors now believed to influence risk. The review was conducted for the purpose of stimulating an awareness of risk factors and encouraging nurses to conduct research studies and to design appropriate nursing therapies, particularly for teenagers, who are developing precancerous lesions in increasing numbers.

A 4-month study was conducted at the University of Maryland Pediatric Hematology/Oncology Clinic to determine the incidence of oral complications as well as the value of early dental intervention as part of the support therapy for pediatric oncology patients. This study is important because of the limited number of reports that currently describe oral assessment and management of oral complications in these patients. Initial oral examination and subsequent evaluations were conducted over the 4-month period. Thirty-six pediatric cancer patients with leukemia or solid tumors who were newly diagnosed or receiving maintenance therapy for their malignancy during this period were evaluated. The relationship between the diagnosis of cancer, therapy, type of oral complication, and response to dental intervention was studied. Sixteen patients (44%) experienced a total of 21 oral complications requiring immediate dental intervention during their cancer therapy. Complications included acute and chronic xerostomia, mucositis, oral infection during tooth eruption, and acute pulpitis. Children with a history of routine dental care prior to their cancer therapy experienced fewer overall oral complications. The findings demonstrate that further comprehensive prospective investigations are necessary to enhance our knowledge to manage oral complications in this critically ill group of patients.

Surgical treatment of carcinomas and other malignancies in the head and neck region often require extensive surgery. These surgeries necessitate the development of various types of reconstructive flaps. Myocutaneous flaps are a relatively new reconstructive technique being used after radical head and neck surgery. These flaps utilize the axial blood supply that runs deep to the muscle masses as well as the cutaneous and subcutaneous tissue. By including the muscle when transferring the skin, the blood supply is transferred as well and allows a much greater range of rotation. Preoperative assessment and preparation of the patient directly impact the outcome of this surgery, especially in terms of nutritional management, oral hygiene, and assessment and correction of deficiencies in the pulmonary and hematopoietic systems. Also, many independent nursing implications and interventions are related to care of the myocutaneous flap. These include assessment, the promotion and maintenance of circulation, and prevention of complications such as infection and carotid artery rupture.

Myelodysplastic syndromes (MDS) are a group of disorders of the blood or bone marrow that frequently indicate an incomplete or early manifestation of acute leukemia. Peripheral blood cytopenias and abnormal hematopoiesis characterize the clinical picture. Approximately 25–50% of persons with MDS develop acute nonlymphocytic leukemia (ANLL), which is why the term “preleukemia” is often used to describe these hematologic disorders. The mean survival time is approximately 2 years. The pathophysiologic changes related to ineffective hematopoiesis may result in multiple clinical problems. Nursing care is focused on (a) identification of patient/family teaching needs; (b) assessment, prevention, and treatment of complications related to anemia, thrombocytopenia, and granulocytopenia; and (c) providing psychosocial support to the client/family throughout the course of the illness.

The concept of nursing diagnosis has recently become a topic of major importance in nursing. Incorporation of well-developed nursing diagnoses into oncology nursing practice can assure comprehensive, yet individualized care for the oncology patient. According to a review of literature, oncology nurses have developed the Outcome Standards, assessment tools, and care plans that can provide the framework from which oncology nursing diagnoses can be developed. Each of the nursing diagnoses should be analyzed and evaluated to determine their relevance and reflect the complexity of the problems experienced by cancer patients. The ultimate benefit is improved cancer care.
