Advances in medical treatment have enabled limb salvage procedures to be evaluated as a preferred mode of treatment for many children with osteogenic sarcoma of an extremity. Chemotherapy, surgery, and at some institutions, radiation therapy all play a major role in the limb salvage effort. Specific criteria for limb salvage candidacy have recently been established and are discussed in this article. Multiple orthopedic surgical procedures and a variety of grafts and prosthetic devices are utilized in the actual salvage attempt. These procedures are presented to provide the nurse with an adequate knowledge base to support optimal nursing care of these patients. Complications of the limb salvage procedure are discussed along with specific preoperative, postoperative, and rehabilitative nursing interventions. The psychosocial and educational needs of these patients and their families are evaluated and presented, paying particular attention to the adolescent patient.

The purpose of this study was to explore the changes in family relationships when a cancer nurse's parent has cancer. The theoretical framework used in structuring the exploration was based on family crisis models by Hansen. Hill, and Burr. Using a cross-sectional approach, a convenience sample of 27 nurse/daughters completed an extensive four-part questionnaire that sought information concerning the nurse/daughter, the parent with cancer, the nurse/daughter's perceptions of usual family relationships prior to the cancer diagnosis, and perceptions about changes in role relationships since the parent's diagnosis. Study findings suggest that nurses as family members could be a hidden resource in the health care system in terms of providing advice, facilitating family communication, and coordinating care if the potential for role conflict and unrealistic expectations can be adequately addressed.

Hyperglycemia and glycosuria are known to occur in pediatric patients diagnosed with acute lymphocytic leukemia and treated with L-asparaginase and prednisone for induction therapy. Although these complications are uncommon, they can have a major impact on the treatment of the patient. Nurses providing care for these patients have a vital role in the detection, management, and education of the patient and family. This article provides current information on etiology, risk factors, and patient management of drug-induced diabetes for pediatric patients with leukemia. Early diagnosis, management, and extensive education promote control of this secondary disease and prevent further complications.

Until recently, peripheral neuropathy (PN) has been a relatively uncommon side effect of cisplatin therapy. However, with the administration of high-dose cisplatin (DDP) (40 mg/m2daily X 5 days), PN symptoms have emerged as a dose-limiting toxicity. Aaronson's et al. assessment of quality of life (QOL) and Diener's concept of subjective well-being (SWB) were used as a framework for this study. The following questions were examined: What is the effect of PN on QOL and SWB? Do PN symptoms from cisplatin decrease with time? What independent variables most predict variability in PN symptoms? Three tools were used: Diener's et al. (11) satisfaction with life scale (SWLS), Aaronson's et al. QOL questionnaire, and eight items assessing PN symptoms, which were developed by the researchers for this study. Patient data were obtained by phone and personal interviews. All patients (n = 30) were free of cancer, and had received more than 720 mg of DDP for testicular and ovarian cancer. Most had completed treatment more than 1 year ago; mean age, 33 years. Results show that PN symptoms are significantly correlated (p < 0.05, Pearson's correlation coefficient) with increase in fatigue, malaise, and psychological distress and with decrease in sense of well-being, satisfaction with life, activities of daily living, and ability to work. Time from end of treatment correlated negatively (r = −0.48, p < 0.05) with PN symptoms, suggesting a decrease in PN symptoms over time. By using stepwise regression analysis, only age, among the selected variables, predicted variability of PN symptoms (r2= 0.29, p < 0.01). The study lends direction for nursing education, practice, and research.

Germ cell tumours, although relatively uncommon, are the most common malignancies amongst men aged 20–39. They are important because they affect men in their most reproductive years. The last decade has seen dramatic improvements in survival rates and so there is increasing emphasis on the quality of survival. This study looked at the impact of testicular loss upon the sexuality of men suffering from testicular cancer. Sixteen men with germ cell tumours of the testis, diagnosed within 2 years of the study, were compared with five men who had also undergone unilateral orchidectomy in the same time period, but for reasons other than cancer. They were also compared with 10 men in the same age group who had no history of testicular problems. A questionnaire modified from the Derogatis Sexual Functioning Inventory was used. No significant differences were identified between the three groups. However, there were individuals in all groups who had obvious sexual problems. In the germ cell tumour group, there were significant differences between retrospective scores for sexual intercourse, masturbation and sexual drive and the postoperative scores. This showed a perceived reduction in sexual activity postoperatively for the germ cell tumour group.

Antineoplastic agents can be carcinogenic, mutagenic, and in some instances teratogenic. The purpose of this study was to quantify urinary mutagenicity of seven nurses who administer antineoplastic agents. In addition, an attempt was made to control for extraneous variables, such as various dietary mutagenic substances, including coffee, alcohol, barbecued meat, etc., and various environmental substances, such as paint fumes, fireplace smoke, gas fumes, cigarette smoke, etc., which may have effected the degree of mutagenic activity in the urine. Other studies controlled for a few of these substances. This study, however, attempted to control for most of the dietary substances that are currently known to be mutagenic as well as many potentially mutagenic environmental substances. There was a statistically significant decrease in mutagenicity after diet modification and a statistically significant increase after antineoplastic handling. One subject taking Dilantin exhibited significant mutagenic ratios in all three samples and was removed from the analyses. Three of the remaining seven samples collected after antineoplastic administration exhibited mutagenicity. This study provided the baseline data for further research currently being conducted in the area of occupational exposure to antineoplastic agents.

Breast self-examination (BSE) is recommended for breast cancer screening, but most women do not perform BSE regularly. Greater accuracy of measures of beliefs and attitudes about BSE is essential to clarify the relationships between these variables and the frequency of BSE performance. The purpose of this research was to assess the reliability and validity of selected measures of beliefs and attitudes about BSE. Sixty-four women attending health programs in community settings responded to a questionnaire about selected beliefs, attitudes, and frequency of BSE. Internally consistent scales were derived (Cronbach alpha coefficients = 0.65–0.89) for remembering, competence, comfort, interference, and efficacy related to BSE. Moderate to strong and significant relationships were observed (Spearman rank correlation coefficients = 0.35–0.58; p < 0.01) between some of these scales and frequency of BSE. From this study, a set of reliable scales of selected beliefs and attitudes about BSE with support for content and criterion-related validity are offered. Directions for future research to increase the understanding of BSE performance and to guide effective nursing interventions to promote BSE performance are identified.

Representative Warren Magnuson, for whom the Clinical Center research facility at the National Institutes of Health (NIH) was named, once said “Health is the wealth of the nation.” That quotation is actually chiseled in the cornerstone of the Clinical Center. In many ways this guiding principle has endured at NIH over the past century. As part of the centennial celebration of the NIH and the 50th anniversary of the National Cancer Institute (NCI), the authors recount some of the history of both the NIH and the NCI. The research conducted at the NIH and NCI has had an impact upon the oncology nurses' responsibilities and need for professional growth. This article discusses some of the available programs, resources, and opportunities available from many NCI divisions and the NIH Clinical Center.
