The initial purpose of this survey was to determine the prevalence of clean tracheotomy care (cleaning and suctioning) aseptic technique as compared to sterile technique. Based on a serendipitous finding of a prior telephone poll to head and neck nurses and physicians, an additional purpose was to assess the degree of inconsistency regarding employment of the terms “clean and sterile” among these health care providers. Questionnaires were sent to 386 members of two head and neck surgeon societies, hospital-based head and neck nurses, and to department chiefs of cancer care centers. From a one-time mail only, 189 responses (48.96%) representing 142 hospitals from 37 states were reviewed; 162 of these responses were categorized into six groups. In collapsing these groups, 16.7% of the respondents report using clean suctioning, clean cleaning, and teach patients clean technique; 27.8% use sterile, and 55.6% use some combination of the above. According to the definitions proposed, procedural methods described did not match the appropriate aseptic technique selected in 37.33% of the responses. Conclusions reached were that much confusion in terminology exists in relation to aseptic tracheotomy care principles, and that clean tracheotomy care technique is becoming more in vogue, not only as a reported type, but as implied from the qualifying statements. Because of the exhibited confusion in terminology between clean and sterile principles obtained from this survey data, one would question the violation of aseptic principles in practice. Ultimately what may be practiced is clean technique, regardless of what it is called.

Few instruments that measure separately the patient's perception of nausea and vomiting are reported in the literature. The pattern of occurrence and the amount of distress caused by each of these common symptoms is unknown for the individual chemotherapeutic agents and drug combinations. Two instruments, the adapted form of the McCorkle and Young symptom distress scale (ASDS) and the index of nausea and vomiting (INV). were developed to measure both nausea and vomiting. These tools were administered to 32 cancer patients who were beginning an initial chemotherapy regime and 72 well citizens in a pilot study designed to develop and test the reliability and validity of these measures. Data are presented that show the tools are reliable and that they validly measure post-treatment nausea and vomiting in chemotherapy patients. This pilot effort is important to the future study of patterns and effective nursing interventions to control the problems of both nausea and vomiting.

Approximately half of all cancer patients receive radiation therapy sometime during the course of illness. In combination with early diagnosis, radiotherapy offers a good probability of increased survival rate. Because external radiation therapy is usually provided on an outpatient basis, individuals receiving this mode of treatment are responsible for their own self-care.The purpose of this study was to identify therapeutic self-care demands as expressed by outpatients receiving radiation therapy. Radiation therapy, physiology, Orem's concept of self-care, and Magoon's constructivist methodology were synthesized as a foundation for the study. A descriptive survey research design was used. An open-ended interview technique, constructed and conducted by the researcher, was utilized to collect the data. Thirty participants were asked to verbally express changes, if any, which occurred in his or her life since the radiation treatments began. The data were collected by the use of a tape recorder, transcribed, and categorized according to the six universal self-care requisites identified by Orem. In order to establish reliability, peers of the researcher who were familiar with the self-care theory categorized the data also.

Fifty-nine patients were evaluated for nutritional changes while receiving a 28-day course of recombinant leukocyte A interferon in an escalating dose, phase I study, on a three times weekly schedule. Side effects included, but were not limited to: fatigue (70%), anorexia (59%), nausea (32%), vomiting (18%), and diarrhea (10%), and are the subject of this report. Weight, serum albumin, and performance status were all significantly lower (p< 0.0001) on day 28 when compared to baseline, and decreases in weight were dose-dependent (p< 0.0001). Recombinant leukocyte A interferon appears to have an adverse effect on patients' nutritional status as measured by these parameters.
