With an increasing number of women surviving breast cancer beyond treatment, the focus of care has shifted from the acute treatment-related side effects to long-term effects associated with changes in quality of life (QOL). Part I of this article described the impact of breast cancer on the domains of physical and social well-being of 21 long-term survivors of breast cancer through qualitative analysis. Part II explores the impact of breast cancer on the domains of psychological and spiritual well-being.

The aim of this study was to describe nurses' perception of hope among cancer patients in palliative care. Interviews, conducted with nine skilled oncology nurses, were subsequently analyzed using a phenomenological-hermeneutic approach inspired by Ricoeur. The findings revealed five subthemes: inner strength and energy, significant events, support from relatives and/or a familiar environment, confidence in treatment, and nursing actions and treatment. Two themes emerged: internal and external factors related to hope and the relationship between patients and nurses. Consideration of the themes disclosed in this study will enable nurses to broaden the content of the phenomenon of hope. This, in turn, might also help nurses to select strategies in palliative care that will foster or restore hope in patients with cancer.

Fatigue is reported to be a significant and distressing problem for people receiving chemotherapy, but the phenomenon is poorly understood and little is known about the factors influencing it. Nurses need to understand the dimensions of fatigue in order to provide effective help for individuals with cancer who experience it. This article describes a study that employed a daily diary with the aim of prospectively charting the onset, pattern, duration, intensity, and distress associated with fatigue in 109 patients receiving chemotherapy. The diary comprised four visual analogue scales measuring selected dimensions of fatigue: extent of fatigue, distress caused by fatigue, the influence of fatigue on the ability to engage in social activities, and the impact of fatigue on workrelated activities. The patients' diaries produced detailed time series of data that captured the dynamics of their fatigue. Analyses of these data revealed the patterns of fatigue after the administration of chemotherapy, which appear strongly related to both the timing of treatment and the manner in which cytotoxic agents are administered. Furthermore, Kruskal-Wallis tests performed to compare fatigue among subgroups of patients revealed that it is statistically associated with particular types of cancer, specific chemotherapy regimens, and certain methods of drug administration. Fatigue varied throughout the day, more frequently occurring in the afternoon and early evening. These insights, gained during this study about the likely pattern of fatigue in the period after the administration of chemotherapy, and the potential benefits of maintaining a fatigue diary could be utilized by nurses engaged in the care of chemotherapy patients.

In the mid-1980s, a clear connection was made between infection by the human papillomavirus (HPV) and cervical cancer. This association led to the generation of a large body of literature about the diagnosis, testing, treatment, transmission and, virological life span of HPV-related neoplasms and cancers. Because of advances in virology, molecular biology, and biochemistry, more is known about HPV and its effects on cervical neoplasms than ever before. Because deaths from cervical cancer, while less common in the United States and western Europe, account for 20% of cancer deaths in women worldwide(1), accurate knowledge of the disease and its prevention is critical for nurses working with patients both with preinvasive and invasive disease.

Little is known of the experiences of oncology patients who abandon biomedicine to use alternate therapies. This ethnography provides insights into the experiences of eight estranged informants. Six main themes emerging from the analysis are (a) expressed stress: emotions such as anger and fear, (b) taking control, (c) belief in a cure,(d) social group association, (e) cost considerations, and(f) mystical insights into health care. Conducted over 3 years, the study includes a network sample and longitudinal case study with one key witness. Individual interviews and one focus group were included. Insights are provided for health professionals who may encounter biomedical abandonment.

Pain is difficult to measure, regardless of etiology. Furthermore, the measurement of pain is complicated by variation in the use of visual analogue scales as well as other pain assessment measures. This brief article reviews situations contributing to confusion regarding pain intensity measures and argues for consistency in its measurement with adults not experiencing cognitive disorders in the clinical setting.

The challenges to nurses working in the rapidly changing milieu of radiotherapy in cancer care are great. New treatment regimens continually require reexamination of old assumptions about patients' needs and communication between colleagues. In order to continue the development of the nursing care in the radiation therapy department, the purpose of this study was to assess the nursing problems experienced in the care of the cancer patients and the nursing problems experienced in work with other professionals from the nurse's perspective. Eighteen of the 38 nurses in the radiation therapy department were invited to participate in the study. The mean age of the nurses was 39 years(range 25-57 years). The study was carried out by using a three-phase, structured, communication-process method according to the Delphi technique. The nurses reported"poor follow-up of patients who have completed radiation therapy," closely followed by "finding time to document the nursing care" and "finding time to treat the scheduled patients" as the most problematic areas of importance for the development of nursing care. The nursing problems considered to be the most difficult in the work with other professionals were "lack of communication," "lack of knowledge of our work and competence," and "lack of comprehension of each other's professions." With the present trend in the health care system toward smaller financial resources, it is necessary to find new ways of organizing nursing care in order to find time to take care of the patient's problems during and after the radiation therapy. This study constituted the base for organizing a nursing care clinic in the department with sufficient time to assess and monitor side effects, patient education, sexual counselling, and coping assistance.
