Thirteen out‐patients suffering from chronic reversible bronchial obstruction were treated on two separate days with two doses of terbutaline aerosol, administered with a 750 ml spacer interposed between the actuator and the mouth, in order to evaluate whether clinical responses showed a dose dependency during a 4‐h period. On each day the two different doses (65 μg versus 250 μg) were inhaled three times at 15‐min intervals. Fifteen minutes after the first dose there was a significant difference in FEV1increase between the small dose and the normal dose, FEV1 being higher after 250 μg, but no significant differences in increase after the 2nd and 3rd inhalations. This study indicates that the bronchodilating effect of terbutaline administered via the spacer is dose‐dependent at the first inhalation, and that succeeding inhalations show no dose‐resp

Seven asthmatic patients were studied in a single‐blind randomized, crossover study after oral administration of 20 mg nifedipine or placebo Four increasing doses of i.v. terbutaline were then given with 30 min interval. The study was concluded with inhalation of five terbutaline puffs. FFAV1measurements 30 min after intake of nifedipine did not show any difference compared to placebo. During the terbutaline treatment, however, there was a more pronounced bronchodilation after nifedipine than after placebo (P<0.051. Terbualine‐induced skeletal muscle tremor was similar after nifedipine and placebo pretreatments. After nifedipine intake there was a decrease of diastolic blood pressure and a reflexogenic tachycardia. The possibility of autoimmune type I reactions to cellular constituents was investigated in 22 patients with ulcerative colitis, 12 with Crohn's disease, and in 22 healthy volunteers Nuclear components and colon mucosa fragments were tested as potential antigens by the basophil histamine release technique One of 12 patients with ulcerative colitis responded to sonicated leukocyte nuclei and one of 12 patients with Crohn's disease responded to both nuclei and RNA. The influence of sodium aurothiomalate on the secretion of immunoglobulins by normal human lymphocytes in vitro was investigated by means of a reverse hemolytic plaque forming cell (PFC assay, Aurothiomalate inhibited the FFC response induced by pokeweed mitogen (PWM) and by Epstein‐Barr virus (EBV) in a dose dependent manner. The inhibition was irreversible, as pre‐incubation for 2 h with the drug followed by extensive washing and further culture in gold salt–free medium still caused an inhibition of the PFC response to PWM and to EBV. Cell proliferation was not significantly affected, suggesting that the inhibition of PFC formation was not due to cytotoxicity. Pre–incubation of monocytes/macrophages (Mø's), T lymphocytes and B lymphocytes with the gold compound prior to culture with PWM showed that M0's and B cells were highly sensitive, whereas T lymphocytes where resistant to the drug. Repeated conjunctiva] provocation tests ICPT) were done in 20 children with rhino‐conjunctivitis due to birch pollen allergy. Compared with the first open challenge, three successive blinded CPTs were reproducible within an allergen strength difference of one 10‐potency in 92 of the tests. Based on the data, a power function was constructed, making it possible to determine the number of patients needed to discriminate CRT sensitivity of a given magnitude between two populations. During the study period specific serum‐IgE increased in only three of 19 patients, indicating that stimulation of IgE antibody production is not a common consequence of repeated tests. In a double blind study, alcohol/onion extract (5% ethanol) was injected simultaneously with 20 IU and 200 IU rabbit anti‐human‐IgE intradermally in 12 adult volunteers (6 atopics, 6 non‐atopics). Diameters of wheals and flares were measured 10 mm after and compared with control sites challenged with 20 IL1 and 200 IL1 anti‐IgE in a 5% ethanol solution. The skin sites were then treated epidermally with 45% alcohol/onion extract and 45% ethanol under occlusion. Diameters of late cutaneous reactions were measured hourly. Oedema formation was clinically estimated according to an arbitrary scale and skin thickness measured with a calliper. In the onion‐treated skin sites the wheal areas were significantly reduced (20 IU: control: 108 ± 53 mm2: onion 69 ± 42 mm2, P<0.05;, 200 IL anti‐lgE: control: 152 ±, 25 mm2, onion: 138 ±, 26 mm2, P<0.02). The oedema formation during the late phase skin reaction was markedh depressed (P<0.005 at 2 h, P<0.01 at 4 and 6 h, P<0.02 at 8 h).
