Prophylactic methotrexate therapy given to 33 cases of hydatidiform mole reduced the incidence of malignant trophoblastic disease from 17.3 per cent to 6.1 per cent and prevented the development of choriocarcinoma without producing any mortality. Toxic effects were more common with oral as compared with intramuscular administration. Two cases of hyperpyrexia are described, and one was further complicated by endotoxin shock. The number of courses of methotrexate given varied from one to five courses, depending on the pattern of H.C.G. excretion. Chemotherapy was discontinued when urinary gonadotrophin fell below 100 m.u.u. per 24 hours.

Mixed gonadal dysgenesis is a condition characterised by presence of a testis on one side and a streak gonad on the other. Two such patients with the condition are described, one with a predominantly female external genitalia and the other with predominantly male external genitalia. The former was a mosaic of XO deleted Y, while the latter was XY. The majority of individuals with this syndrome have ambiguous external genitalia but with predominantly female genital duct system, and a chromosome constitution of XO / XY. The literature is reviewed and the relationship of the condition with other syndromes of sex abnormalities discussed.

A report of 15 cases of gonadal dysgenesis (Turner's syndrome) is presented.Thirteen cases belonged to the short‐stature group and two in the normal‐stature group.All had primary amenorrhoea, elevated gonadotrophin, infantile genital organs and no secondary sex characteristics. The gonads were visualised in eight. In the short‐stature group typical gonadal dysgenesis was found in four and small ovaries with undeveloped follicles were found in two. Both patients in the normal‐stature group had gonadal dysgenesis proven by peritoneoscopy and exploratory laparotomy. For the short‐stature group 83% were chromatin negative and six were shown to have 45 XO karyotype. Both normal stature patients were chromatin positive and one had a 44 XX karyotype. A webbed neck was present in five, anomalies of the kidneys in two and colour blindness in one of the short‐stature group. No somatic anomaly was detected in the normal‐stature group. A method of management of 400 cases of severe pregnancy toxaemia with benzodiazepines (chlordiazepoxide and diazepam) for sedation, and an active approach to the obstetric management is described. Adequate sedation was achieved in 88 per cent of the cases, and not a single patient developed an eclamptic fit once treatment was instituted. A gross perinatal mortality rate of 10.5 per cent was obtained, with a corrected perinatal mortality rate of 8.7 per cent (after excluding 43 cases which were included in the series post‐partum, and 12 cases with the foetuses already dead on admission.) In patients who were admitted in labour or whose pregnancies were terminated on admission the perinatal mortality rate was 4.8 per cent to 1.1 per cent. But in the group where the pregnancy was conserved the perinatal mortality was as high as 24.4 per cent, rising to an even higher rate of 40.8 per cent in those whose toxaemic state was not well controlled. A maternal mortality rate of 1.25 per cent was recorded (5 deaths). In this series of Uterine Cervical Cancers 1955‐1958, the five‐year survival rate for adenocarcinotna was lower than that for epidermoid carcinoma. The survival rate was about equal in both when the lymph nodes were free of tumour cells whereas in the presence of lymph node metastasis the survival rate for adenocarcinoma was much lower. The incidence of lymph node metastases was also higher with adenocarcinoma. From a review of the literature and our personal experience the authors feel that the present forms of treatment are ineffective for adenocarcinoma of the cervix and propose that future treatment should be: Surgery followed by chemotherapy for adenocarcinoma and chemotherapy and/or irradiation followed by surgery for small cell carcin

The present status of irradiation therapy in Japan for cervical cancer of the uterus and the several concomitant problems are discussed in this paper. Recently, our radiotherapeutic techniques have markedly advanced; the introduction of telecobalt, linac and betatron as well as the improved techniques helped to increase the salvage rate. However, there are increased risks of severe side effects from such intensified external irradiation, so that much more accurate determination of the irradiation field, dose and technique would be required. The present paper deals with the intracavitary irradiation method, afterloading, remote control insertion of the micro source, and the measurement of radiation dose by IBM. The author's views on the following items are also expressed: a future prospect on the selective tissue irradiation method, the estimation and artificial alteration of radiosensitivity (for instance, irradiation under hyperbaric oxygen), the criteria for selecting a therapeutic method, and judgement concerning the necessity of postoperative irradiation.
