There is accumulating evidence that oxidatively modified low-density lipoproteins have a role in the pathogenesis of atherosclerosis. The susceptibility of low-density lipoprotein to oxidative stress depends on the balance between its content of polyunsaturated fatty acids and antioxidants, in particular vitamin E. Both these nutrients are derived entirely from dietary sources.

Dietary studies have shown that not all individuals respond to the same extent to changes in dietary fat and cholesterol. This variability in response can be reproduced over prolonged periods, suggesting the involvement of genetic factors. Recent studies have demonstrated that polymorphism of apolipoprotein genes can influence an individual's response to dietary manipulation.

摘要:
This review presents a discussion of potential mechanisms by which dietary polyunsaturated fats (including both n-3 and n-6 fatty acids) lower cholesterol concentrations in plasma lipoproteins. The prevailing evidence suggests that the decrease in plasma low-density lipoprotein cholesterol induced by polyunsaturated fat is a result of modified secretion of cholesteryl ester (in the case of n-6 fatty acids) and triglyceride (in the case of n-3 fatty acids) and increased low-density lipoprotein receptor function. These changes protect against atherosclerosis in spite of the concomitant reduction in high-density lipoprotein cholesterol concentrations which appear to be mediated through decreased production and increased catabolism of apolipoprotein A-l, the major apolipoprotein of the high-density lipoprotein.

HMG CoA (β-hydroxy-β-methylglutaryl coenzyme A) reductase inhibitors are very effective in lowering total and low-density lipoprotein cholesterol. The incidence of adverse effects is very low with elevated transaminase levels and myopathy being of greatest concern. Administration of these drugs in combination with colestipol has produced a reduction in coronary artery stenosis and cardiovascular events.

Recent reanalyses of trial data have defined a small subgroup of patients which benefitted most from gemfibrozil treatment. New data have been presented indicating that fibrates influence hemostatic factors favorably and that some may improve glucose tolerance. These data and their clinical implications are summarized.

Recent studies on the effects of hypolipidemic drugs in the intestine are reviewed, with particular emphasis on trials in man. Developments in bile acid sequestrants and the possibility of drug interaction with the active uptake of bile acids from the intestine are discussed. Attempts to lower plasma cholesterol through inhibition of cholesterol absorption from the intestine have continued but a major breakthrough is still awaited.

The three established methods for low-density lipoprotein (LDL) apheresis, LDL immunoabsorption, dextran sulfate cellulose-LDL apheresis and heparin-induced extracorporeal LDL precipitation (HELP)-LDL apheresis, are equally safe and effective. Hemorrheological parameters are also improved by these procedures, notably by HELP-LDL apheresis which also eliminates fibrinogen from plasma. Preliminary data suggest that LDL apheresis may be an important tool for primary and secondary prevention in selected patients with a particularly high risk for coronary heart disease
