This paper reviews the recently published literature on obesity, body fat distribution and lipid metabolism. Studies have emphasized the importance of visceral fat as a correlate of alterations in plasma lipid transport observed in abdominal obesity. Some of the mechanisms in the relation of fat distribution to lipid metabolism have been identified, but additional studies are needed.

The relationships between diet and cardiovascular disease are reviewed on the basis of the most important papers published during the past year. The review is divided into three parts: in the first, the relationships between dietary patterns and incidence of cardiovascular disease are reported; the second deals with the possibility of reversing even severe atherosclerosis by dietary change, and in the third section we consider the mechanisms by which diet may influence atherosclerosis.

Water-soluble non-starch polysaccharides (NSP) lower plasma cholesterol both as isolates and in whole foods, for example oat bran. Their exact mechanism of action is uncertain, but probably involves enhanced steroid excretion and altered fat and cholesterol absorption. NSP protect against cardiovascular disease, but this effect may be modified by other factors such as antihypertensive therapy.

The mechanism of action and clinical efficacy of lovastatin, simvastatin and pravastatin as hypolipidemic agents is reviewed. All three drugs act primarily in the liver and reduce plasma concentrations of low-density lipoprotein and very-low-density lipoprotein remnants by 30–50%. Serious side effects involving the liver or muscle are uncommon; the risk of myopathy is, however, increased when lovastatin is combined with gemfibrozil and this combination is not recommended.

This review covers significant work on drugs working on the intestine for the year up to June 1990. Developments in bile acid sequestrants and related drugs are discussed. Some novel properties of β-hydroxy-β-methylglutaryl coenzyme A reductase inhibitors are considered. Finally, the role of acyl coenzyme A: cholesterol acyltransferase, cholesteryl ester hydrolase and compounds affecting these enzymes in intestinal cholesterol absorption are assessed.

Based on recent evidence, oxidized low-density lipoprotein (LDL) is a strong candidate for anin vivoatherogenic lipoprotein. Probucol, a drug widely used as a hypolipidemic agent, has an anti-atherosclerotic action which can be attributed to its antioxidant effect. Results from studies using probucol have suggested that oxidized LDL have an important role in atherosclerosis. In this review the effects of probucol on oxidized lipoproteins are discussed with particular reference to oxidized LDL.

Fibric acid derivatives are an important and powerful group of lipid-lowering drugs which have found a key place in our lipid-regulating formulary. We review recent research on aspects of the mechanism of fibrate action, novel applications of established fibrates, development of new drugs and further evaluation of existing compounds.

Nicotinic acid is effective in all forms in hyperlipoproteinemia. Its primary mode of action is to inhibit lipolysis in adipose tissue. Consequently, a reduction in plasma triglyceride, low-density lipoprotein and very-low-density lipoprotein cholesterol and lipoprotein (a) levels as well as a rise in high-density lipoprotein cholesterol are observed. Derivatives, for example acipimox, are free of the major side effects of nicotinic acid, but are apparently less powerful.
