Triglyceride lowering is the most consistent effect offish oils, having been observed in Greenland Eskimos and subsequently in controlled clinical trials. Although total cholesterol levels are not altered, LDL levels may rise, especially in hypertriglyceridemic patients. The persistence of the hypotriglyceridemic effect has been documented in two recent trials following large numbers of patients for 6-9 months. These studies also provided evidence that the LDL raising effect may not persist. The safety of fish oils was also supported in these studies because problems with excessive bleeding and worsening glycemic control did not materialize. However, in one of these studies fish oils proved ineffective in slowing the rate of restenosis after coronary angioplasty.

Oxidative modification of low density lipoprotein is influenced by dietary poly unsaturates. Omega-6 poly unsatured fatty acids enhance the susceptibility of low density lipoprotein to oxidation compared with monoenes. Most studies on omega-3 fatty acids also exhibit increased peroxidation of low density lipoprotein, although these data are more conflicting. Future studies should focus on additional information concerning dietary intake of antioxidants, fatty acids and lipid peroxides, as well as on the importance of low density lipoprotein oxidation

Platelets take part in haemostasis and thrombosis, and studies have been carried out to try to understand how dietary fatty acids could reduce platelet activation and thus the risk of cardiovascular disease. Unfortunately, many of these studies had serious methodological flaws, and the shortcomings in their study designs are probably the main reason for contradictory results in humans. The evidence concerning linoleic acid is not consistent, but intervention studies show increased platelet aggregation to various agonists after highlinoleic- acid diets. On the other hand, intake of alpha-linoleic acid either has no effect or leads to decreased platelet aggregation when compared with linoleic acid. High intake of long-chain n-3 fatty acids of fish or fish oils seems to usually decrease platelet aggregation. To date, there have not been many studies on the effect on platelet aggregation of small or reasonable amounts of n-3 fatty acids.

Fish oil diets, rich in n-3 polyunsaturated fatty acids, are considered to have an antithrombotic effect. Both platelets and endothelial cells play a crucial role in the regulation of thrombosis and haemostasis. There is substantial evidence that, in these cells, fish oil-derived polyunsaturated fatty acids can replace the n-6 polyunsaturated fatty acid, arachidonic acid, in the membrane phospholipids and can modify those cellular reactions in which the latter fatty acid participates. However, it now appears that dietary n-3 polyunsaturated fatty acids are less potent than, for example, aspirin in modifying the activation properties of these cell systems. This suggests the possible involvement of additional cells or factors in mediating the antithrombotic potential of fish oil fatty acids.
