Meta-analysis has been applied to dietary intervention studies to increase sample size but the conclusions are limited by the unusual nature of the study populations. Recent angiographic studies suggest benefits of diet on coronary artery patency.

摘要:
Hypercholesterolemia represents a key factor in the etiology of atherosclerosis. A sizeable reduction in the consumption of dietary saturated fatty acids and cholesterol is needed, therefore, in order to decrease plasma cholesterol levels in individuals and in populations at high risk. In people who are not overweight, calories from saturated fatty acids need to be replaced with other sources of energy. In this case, monounsaturated fatty acids might represent a reasonable alternative to polyunsaturated fatty acids and starch, in view of their favourable effects on plasma lipoprotein concentrations and other factors presumed to be etiologically linked to atherosclerosis.

The advent of increasingly powerful and effective lipid-lowering medication has raised the issue of the usefulness of continuing with diet in therapeutic lipid-lowering regimes. A number of recent studies indicate a beneficial effect from the use of a lipid-lowering diet in conjunction with lipid-lowering drugs in the treatment of hyperlipidaemia.

摘要:
An apparent protective effect of light-to-moderate alcohol consumption on the risk of coronary heart disease has been documented in numerous ecological, case-control and cohort studies. The findings are consistent and there are several biological effects of alcohol which could explain the findings. It has been argued, however, that the observed association is an artefact due to either the misclassification of ex-drinkers as never-drinkers or to confounding factors. A series of recent studies have addressed these concerns and examined the possible biological mechanisms involved. These studies add support to the hypothesis that the inverse association between light-to-moderate alcohol consumption and coronary disease risk is causal. Increasing consumption offsets the benefits for coronary heart disease with adverse health effects: epidemiological evidence indicates that safe limits are up to two to three drinks a day for men and one to two drinks a day for women.

n-3 polyunsaturated fatty acids (PUFAs) possess several effects that may potentially reduce the risk of ischaemic heart disease. The results from clinical trials with n-3 PUFAs give rise to some optimism, and the possible value of n-3 PUFAs in the prevention and treatment of ischaemic heart disease should be further investigated.

The evidence that hypertriglyceridemia is a risk factor for the premature development of coronary artery disease in certain populations remains strong. The two most effective classes of triglyceride-lowering drugs are nicotinic acid and its derivatives and the fibrates. The present review discusses recent work in which the effect of these drugs on the concentrations and metabolism of lipoproteins in human patients has been evaluated.

Evidence for the efficacy and safety of the β-hydroxy-β-methylglutaryl coenzyme A (HMG CoA) reductase inhibitors in the treatment of patients with primary hypercholesterolemia continues to accumulate. In addition, these drugs may have a role in the treatment of other forms of hyperlipidemia. Hepatotoxicity is the most common side effect. Primary and secondary intervention studies are currently under way to investigate the effects of HMC CoA reductase inhibitors on cardiovascular morbidity and mortality.

Cholesterol reduction has been shown to be of benefit for both primary and secondary prevention of coronary disease in middle-aged men. There is, however, much less data evaluating drug efficacy in the elderly for either lipid lowering or coronary heart disease (CHD) prevention, and controversies of total mortality and cost-effectiveness in the 1990s have further dampened enthusiasm about generalizing results on drug treatment for the elderly. For CHD prevention, lipid-lowering drug therapy in elderly patients with established CHD may be beneficial, but primary prevention efforts demand stringent criteria for patient selection to ensure likelihood of benefit while minimizing risk. The existence of nuances of drug metabolism indicate that the elderly can achieve similar lipid lowering using smaller doses.
