Summary.The purpose of the study was to compare the anticaries effects of two dentifrices containing sodium fluoride with fluoride concentrations of 250 ppm and 1450 ppm on the primary dentition of kindergarten children. A total of 319 children participated in the study (155 in the low‐fluoride and 164 in the high‐fluoride group) and brushed their teeth daily under supervision in their kindergartens. The children were examined by one dentist trained according to the procedures of O'Mullaneet al., and carious lesions were recorded at the cavitation level according to the WHO criteria. At the final examination after 22 months, the groups comprised 83 (low‐fluoride group) and 89 (high‐fluoride group) children. The differences in meandmfsanddmftincrements between the two groups were not statistically significant. In both groups about 57% of the children remained caries free. Excluding these children from the analyses, the meandmfsincrement was significantly lower in the high‐fluoride group, but not the meandmftincrement. The meandmfsincrement excluding occlusal surfaces was significantly lower in the high‐fluoride group.Résumé.Un comparison de l'effet anti‐caries de deux dentifrices contenant 250 ppm ou 1450 ppm fluorure sur la dentition temporaireLe propos de cette étude est de comparer l'effet anti‐caries de deux dentifrices contenant du fluorure de sodium à 250 ppm et 1450 ppm sur la dentition temporaire d'enfants d'écoles maternelles. Un groupe de 319 enfants a été étudié (155 pour le dentifrice à faible teneur et 164 à teneur élevée en Fluor). Ils se brossaient les dents deux fois par jour à l'école sous surveillance. Les enfants ont été examinés par un dentiste averti des procédures de O'Mullaneet al.Les lésions carieuses ont été notées d'après les niveaux de cavitation agréés par l'OMS. Die Kinder wurden von einer Zahnärztin (ABSC) welche den Methoden von O'Mullaneet al.vertraut war, untersucht. Kariöse Läsionen wurden nach den WHO‐Kriterien regristriert. Bei der Enduntersuchung nach 22 Monate sind noch 83 Kinder in der niedrigfluoridierte Gruppe und 89 in hochfluoridierte Gruppe geblieben. Die Unterschiede der durchschnittlichendmfsunddmftder Gruppen waren nicht statistisch signifikant. In beiden Gruppen waren etwa 57% der Kinder Karies frei. Un total de 39 niños participaron en el estudio (155 en el grupo de 250 ppm y 164 en el de 1450 ppm). Los niños se cepillaron los dientes bajo supervisión en sus kindergartens. Los niños fueron examinados por un odontólogo entrenado de acuerdo a los procedimientos de O'Mullane et al y las caries fueron anotades a nivel de cavitación de acuerdo a los criterios de la OMS. Al final del examen a los 22 meses, los grupos comprendían 83 (250 ppm) y 89 (1450 ppm) niños. Summary.The aim of the study was to determine the prevalence of iron‐deficiency anaemia in children less than 6 years of age who attended for outpatient general anaesthesia for dental extractions. Pre‐ or peri‐operative capillary blood samples were taken from 109 children (70 white Caucasian and 39 ethnic‐minority children). The haemoglobin concentration (Hb), mean cell volume (MCV) and red cell porphyrin (RCP) value were determined for each subject. 19% of the children were found to be anaemic (Hb<11 g/dl) and there was no statistically significant difference for mean Hb or prevalence of anaemia between white Caucasian and ethnic‐minority children. A significantly greater proportion of the ethnic‐minority children were iron deficient, as indicated by low MCV and high RCP. There were no statistically significant differences between anaemic and non‐anaemic subjects with regard to sex or social class, but a significantly greater mean number of extractions were performed in the anaemic children.Résumé.Anémie ferriprive chez de jeunes enfants ayant eu des extractions sous anesthésie généraleLe propos de cette étude est de déterminer la prévalence de l'anémie ferriprive chez des enfants de moins de 6 ans ayant eu des extractions sous anesthésie générale ambulatoire. Des prélévements sanguins pré et post opératoires ont été recueillis chez 109 enfants (70 enfants caucasiens blancs et 39 de minorités ethniques). La concentration en hémoglobine (hb), le volume cellulaire moyen (vcm) et des hématies porphyrines (hp) ont été mesurés chez chaque sujet. Dixneuf pour cent des enfants étaient anémiques (hb inférieur à 11 g/dl). Il n'y avait pas de différence statistiquement significative pour le taux moyen de hb ou la prévalence d'anémie entre les enfants caucasiens blancs et de minorités ethniques. Une proportion significativement plus importante d'enfants de minorités ethniques étaient déficients en fer comme le démontre le VCM faible et le HP élevé. Kapilläre Blutproben wurden Prä oder perioperatif von 109 Kindern entnommen (von 70 weissen und von 39 farbigen). Die hämoglobinkonzentration (Hb) das durchschnittliche Zellvolumen (MCV) und der Porphyrinwert der roten Zellen (RCP) wurden für jedes Kind bestimmt. 19% der Kinder war anämisch (Hb<11 g/dl). Es wurde kein statistischer signifikanter Unterschied gefunden zwischen den weissen und den farbigen Kindern bezüglich der Hb und der Anämie, aber ein signifikanter grösserer Anteil der farbigen Kinder hatten einen Eisenmangel wie der niedriger MC V und der höheren RCP zeigten. Es war kein signifikanter Unterschied zwischen den anämischen und nicht anämischen Kindern was das Geschlecht oder den sozialen Stand betrifft, aber eine signifikante grössere Anzahl von Extraktion wurde bei anämischen Kindern durchgefürht.Resumen.Anemia por deficiencia de hierro en niños requiriendo extracciones bajo anestesia generalEl objeto de este estudio fue determinar la prevalencia de anemia por deficiencia de hierro en niños menores de 6 años de edad que recibieron extracciones dentales bajo anestesia general. Muestras de sangre capilar obtenidas pre y postoperatoriamente fueron obtenidas en 109 niños (70 blancos caucásicos y 39 de una minoría étnica). La concentración de hemoglobina (Hb), volumen medio celular (VMC) y porfirina en los glóbulos rojos (PGR) fueron determinados en cada niño. El 19% de los niños presentaron anemia (Hb<11 g/dl) y no hubo diferencia significativa entre la media Hb y la prevalencia de anemia entre los niños caucásicos y los de la minoria étnica. Una proporción significativamente mayor de la minoria étnica presentaron anemia por deficiencia de hierro evidente con un bajo VMC y un alto PGR. Summary.The purposes of the study were threefold: to compare salivary levels of mutans streptococci andLactobacillusin 140 5‐year‐old children from two ethnic groups, to correlate caries experience of each group with bacterial counts, and to determine levels of infectivity which could indicate high caries activity in young children. Pakistani‐Muslim and white Caucasian children were paired, matched for age, gender and caries experience. There were no significant differences in mutans streptococci orLactobacilluslevels between the two ethnic groups. However, strong correlations were found between caries experience and levels of both mutans streptococci andLactobacillusin each ethnic group. Furthermore, mutans streptococci andLactobacilluslevels correlated strongly with one another. For detection of high caries activity, the optimum screening levels of bacteria were>105cfu/ml for mutans streptococci (sensitivity 78% and specificity 86%) and>104cfu/ml forLactobacillus(sensitivity 82% and specificity 89%).Résumé.Microflora bactérienne et expérience carieuse chez les enfants de deux groupes ethniquesLe propos de cette étude est triple: comparer le taux de streptrocoques mutans et de lactobacilles chez 145 enfants de deux groupes ethniques, corréler l'expérience carieuse de chaque groupe avec le comptage bactérien et déterminer le niveau infecteux qui pourrait indiquer une activité carieuse élevée chez de jeunes enfants. Les enfants pakistanais musulmans et caucasiens blancs ont été appariés, assortis par âge, sexe et expérience carieuse. Il n'y avait pas de différence significative entre le taux de streptocoques mutans et de lactobacilles entre les deux groupes ethniques. Mais des corrélations fortes entre l'expérience carieuse et le niveau de streptocoques mutans et de lactobacilles dans chaque groupe ethnique. De plus, le taux de streptocoques mutans et de lactobacilles était corrélé l'un avec l'autre. Niños pakistanies‐musulmanes y blancos caucásicos fueron apareados y distribuidos por edad, sexo y prevalencia de caries. No hubo diferencia significativa en los niveles de estreptococo mutans y lactobacilo entre los dos grupos étnicos. Summary.Forty children (20 boys and 20 girls) aged 8–16 years with no previous experience of dental visits were included in the study. Three psychological variables were determined: anxiety (State‐Trait Anxiety Inventory of Children), personality (Eysenck Personality Questionnaire‐Junior) and intelligence (Wechsler Intelligence Scale for Children). The relationship between personality and intelligence factors and the levels of anxiety at the beginning of the first dental visit (before treatment) and at the end of the third visit (after treatment) were determined. There was no significant relationship between levels of dental anxiety and age. Dental anxiety both before and after treatment was higher among the girls than among the boys. Children with high intelligence quotients showed less dental anxiety at their first dental visit. There was no relationship between the children's anxiety and their own personality variables.Résumé.La peur du dentiste chez l'enfant: influence de l'intelligence et de la personnalitéQuarante enfants (20 garçons et vingt filles) âgés de 8–16 ans, qui n'avaient jamais eu de soins dentaires, ont été étudiés. Trois variables psychologiques ont été déterminées à l'aide de tests: l'anxiété (inventaire des symptômes d'anxiété chez l'enfant), la personnalité (questionnaire d'Eysenck de personnalité pour jeunes) et intelligence (échelle d'intelligence de Wechsler pour enfants). Les relations entre la personnalité et l'intelligence et le niveau d'anxiété, au début de la première consultation (avant traitement) et à la fin de la troisième consultation (après traitement) on été notées. Il n'y avait pas de relation significative entre le niveau d’ anxiété et l'âge. l'anxiété dentaire, avant et après traitement, était plus élevée chez les filles que chez les garçons. Les enfants, dont le quotient intellectuel était élevé, souffraient moins d'anxiété dentaire lors de la première consultation. Il n'y avait pas de relation significative entre l'anxiété des enfants et les variables de leur personnalité.Zusammenfassung.Kinder Angst vor den Zahnartzt: Der Einfluss der Intelligenz und PersonalitätAnhand von allgemein anerkannten Instrumenten wurden Kinder (20 Jungen und 20 Mädchen) im Alter zwischen 8 und 16 Jahren untersucht, die zuvor noch keine zahnärztliche Praxis besucht hatten. Es wurden die folgenden psychologischen Variablen analisiert: Ängstlichkeit (Test STAIC), Persönlichkeit (Test EPQ‐J) und Intelligenz (Test WISC). Zwischen diesen psychologischen Factoren und der Ausprägung der Ängstlichkeit wurde eine Beziehung vor und nach unterschiedliechen zahnärztlichen Behandlungen hergestellt. Es besteht keine Beziehung zwischen der Ausprägung der Ängstliichekeit und dem Alter. Offensichtlich geben Kinder mit hohem Intelligenzkoefizient weniger Ängstlichkeit bei ihrem ersten zahnärztlichen Besuch Zu erkennen als diejenigen mit niedrigem Koefizient. Die Ängslichkeit vor dem Zahnarzt ist in der Gruppe der Mädchen höher ausgeprägt als bei den Jungen. Es kann keine Beziehung hergestellt werden zwischen der Ängstglichkeit der Kinder und der Variablen, die die eigene Persönlichkeit bestimmen.Resumen.Ansiedad dental en niños: influencia de los factores personalidad e inteligenciaSe estudiaron 40 niños (20 varones y 20 hembras) de 8–16 años de adad sin visitas previas al consultorio dental. Se analizaron las siguientes variables psicológicas: ansiedad (test STAIC: State‐Trait Anxiety Inventory of Children), personalidad (test EPQ‐J: Eysenck Personality Questionnaire‐Junior), e inteligencia (test de WISC: Wechsler Intelligence Scale for Children). No se detectó relación entre estos factores psicológicos y los niveles de ansiedad y la edad. La ansiedad fue mayor en las niñas que en los niños. Summary.A report is presented of a prospective study into the oral manifestations of liver disease in children. Fifty‐five children who had received a liver transplant were examined; their ages ranged from 8 months to 16 years 2 months, and 37 of them were under 5 years of age. The following information was noted: the teeth that were erupted, the presence and severity of enamel hypoplasia and of gingival hyperplasia, and the presence of intrinsic discoloration. Results showed that seven of the 17 children under 3 years of age had delayed eruption of teeth. Six children had enamel hypoplasia, three in permanent incisors and three in primary incisors. Intrinsic discoloration was found in the primary dentition of 25 children (23 of whom had biliary atresia prior to receiving the transplant) and in one child's permanent incisors. Thirty‐four children had gingival hyperplasia. There was a significant inverse relationship between the length of time since transplantation and the severity of gingival hyperplasia. Analysis of variance failed to show any association between serum cyclosporin levels and the severity of gingival hyperplasia. There was no significant relationship between current nifedipine medication and the presence of gingival hyperplasia.Résumé.Manifestations orales chez des enfants hépatotransplantésUne étude prospective des manifestations orales chez des enfants souffrant de maladie hépatique a été entreprise chez cinquante cinq enfants ayant reçu un transplant hépatique. Leur âge s'étalait de 8 mois à 16 ans 2 mois et 37 d'entre eux avaient moins de trois ans. Les informations suivantes ont été notées: les dents ayant fait leur éruption, la présence et la sévérité des hypoplasies, des hyperplasies gingivales, des décolorations intrinséques. Les résultats ont montré que 7 des 17 enfants âgés de moins de 3 ans avaient des retards d'éruption. Six enfants avaient des hypoplasies de l'émail, trois sur les incisives permanentes et trois sur les incisives temporaires. Des décolorations intrinséques ont été retrouvées dans la dentition temporaire de 25 enfants (23 d'entre‐eux avaient eu une atrésie biliaire avant la transplantation), et sur les incisives permanentes d'un enfant. Trente enfants avaient une hyperplasie gingivale. Il y avait une relation significative inversée entre l'ancienneté de la transplantation hépatique et la sévérité de l'hyperplasie. l'analyse de variance n'a montré d'association entre le taux de cyclosporine dans le sang et la sévérité de l'hyperplasie gingivale. Das Alter bewegte sich zwischen 8 Monaten bis 16 Jahre und 2 Monate. 37 davon waren weniger als 3 Jahre alt. Die folgenden Befunde wurden registriert: der Stand des Zahndurchbruches, das Vorhandensein und der Schweregrad von Schmelzhypoplasien, eventuele Verfárbungen und Gingivahyperplaisen. Man fand dass 7 der 37 Kinder unter 3 Jahren einen verzögerten Zahndurchbruch aufwiesen, ebenso hatten sie Hypoplasien, bei 3 Kindern in den bleibenden Frontzähnen und bei 3 in den Milchfrontzähnen. Verfárbungen wurden an den Milchzähnen von 25 Kindern und bei einem Kind an den bleibenden Frontzähnen gefunden. Von den Kindern mit betroffenen Milchzähnen litten 23 vor der Transplantation an einer Gallenatresie. Bel 30 Kindern wurde eine Gingivahyperplasie diagnostiziert. Eine signifikante umgekehrten Relation, wurde festgestellt, zwischen der Zeitspanne seit der Transplantation und dem Schweregrad der Gingivalen Hyperplasie. In einer weiteren Analyse fand man keine Beziehung zwischen dem Zyklosporinspiegel im Serum und dem Schweregrad der Gingivahyperplasie. Es war keine signifikante Beziehung zwischen der laufenden Nifedipine Medikation und der Gingivahyperplasie festzustellen.Resumen.Manifestaciones orales en niños con transplantes del higadoSe presenta un reporte de un estudio prospectivo sobre las manifestaciones orales en niños que han recibido transplantes del hígado. Se examinó un total de 55 niños (de 8 meses a 16 años y 2 meses de edad; 37 de elos menores de 3 años de edad) que habían recibido transplante del higado. La siguiente información fue anotado: dientes erupcionados, presencia y severidad de hipoplasia del esmalte y de hiperplasia gingival, y la presencia de pigmentaciones intrínsecas. Los resultados mostraron que 7 de los 17 niños menores de 3 años de edad presentaron erupción dental retardada. Seis de los niños presentaron hipoplasia del esmalte, tres en los incisivos permanentes y tres en los incisivos temporales. Pigmentaciones intrínsecas fueron observadas en la dentición temporal de 25 niños y en los incisivos permanentes de un niño (23 de ellos tenían atresia billar antes de recibir el implante). Treinta niños presenraron hiperplasia gingival. No hubo relación inversamente proporcional entre el tiempo transcurrido despues del transplante y la severidad de la hiperplasia gingival. Summary.Differential diagnosis and treatment modalities of giant cell granulomas have been a controversial matter since Jaffe first distinguished true giant cell tumours (TGCG) from central giant cell granulomas (CGCG). CGCG is a slowly growing and painless tumour usually treated by simple curettage but denotes a strong tendency to recurrence. A case report is presented to describe conservative therapy of CGCG in the maxilla of a young child: surgical enucleation of the tumour at the age of 11 years, though successful, was followed by progressive impaction of the maxillary right permanent canine. Orthodontic treatment aligned the maxillary teeth to accommodate autotransplantation of the impacted tooth. Clinical and radiographic evaluation of the patient 5 years after the initial surgical treatment showed good bony and periodontal healing around the transplanted canine and no recurrence of the CGCG. As the literature shows that distinction of the two tumours is difficult even by histological means, various clinico‐pathologic factors involved in prognostic evaluation are discussed.Résumé.Traitement conservateur chirurgical et orthodontique d'un granulome à cellules géantes chez l'enfantLe diagnostic différentiel et les différentes modalités thérapeutiques du granulome à cellules géantes sont l'objet de discussions depuis la distinction, établie par Jaffé en 1953, entre granulome central à cellules géantes et tumeur vraie à cellules géantes. Le granulome central à cellules géantes est une tumeur d'évolution lente et asymptômatique, généralement traitée par simple curetage bien que manifestant une forte tendance à récidiver. Un cas de traitement conservateur de GCCG chez un jeune enfant est présenté. A la suite de l'exérése chirurgicale de la tumeur à l'âge de 11 ans, la canine supérieure droite s'est progressivement incluse. Un traitement orthodontique a permis l'alignement des dents de l'arcade supérieure en laissant la place nécessaire pour l'autotransplantation de la canine incluse. l'examen clinique et radiologique, cinq ans après le curetage chirurgical initial, montre la parfaite incorporation tant osseuse que parodontale du transplant sans récidive du GCCG. De nombreaux auteurs jugent néanmoins très difficile de poser un diagnostic différentiel précis entre ces deux pathologies, même à l'aide d'un examen histologique. Cette incertitude pouvant amener à un traitement chirurgical par exérèse compléte souvant invalidant, les différents éléments pronostiques liés à cette pathologie sont discutés.Zusammenfassung.Konservative, chirurgische und orthodontische Behandlung eines Falles mit RiesenzellgranulomaDie Differentialdiagnose und die Behandlungsmöglichkeiten von Riesenzellgranuloma sind kontrovers, seit Jaffe als erster zwischen einem echten Riesenzelltumor (TGCG) und einem zentralem Riesenzelltumor (CGCG) unterschied. Der Letzte ist ein langsam schmerzlos wachsender Tumor, welcher in der Regel mittels einfache Kurretage entfernt wird, aber eine erhöhte Tendenz von Rezidiven aufweist. Ein Fall wird beschrieben von einem 11 jährigem Kind bei welchem ein CGCG in der maxilla konservativ ausgeschält wurde. Obwohl der Eingriff sichtlich erfolgreich war wurde eine Retention des oberen rechten bleibenden Eckzahnes festgestellt. Eine orthodontische Behandlung der oberen Zähne ermöglichte die Einreihung des autotransplantierten retinierten Zahnes. Klinische und röntgologische Beurteilung des Patienten 5 jahre nach dem primären Eingiff zeigten gut knöchernde und peridontale Heilung des transplantierten Eckzahnes, und keine Rezidiv des Tumors. In der Literatur wird darauf hingewiesen dass das Unterscheiden zwischen den beiden Tumoren, sogar mit histologischen Mitteln, sehr schwierig ist. Verschiedne klinische pathologische Faktoren in Bezug der prognostischen Beurteilung werden diskutiert.Resumen.Tratamiento quirúrgico y ortodóntico conservador de un niño con granuloma de célas gigantesEl diagnóstico diferencial y modalidades de tratamiento de los granulomas de células gigantes han sido controversiales desde que Jaffe distinguió por primera vez los verdaderos tumores de células gigantes (VTCG) de los granulomas centrales de células gigantes (GCCG). El (GCCG es un tumor de crecimento lento e indoloro usualmente tratado con un simple curetage, pero tiene una gran predilección a recurrir. Se reporta el caso de un tratamiento conservador para el GCCG en el maxilar superior de un niño joven Aún cuando la enucleación quirúrgica del tumor a los 11 años de edad tuvo éxito, una progresiva impactación del canino superior derecho fue evidente. El tratamiento ortodóntico alienó los dientes superiores para acomodar al canino que fue autotransplantado. El examen clinico y radiográfico 5 años despues transplante mostró una buena cicatrización ósea y alveolar alrededor del canino transplantado sin recurrencia del GCCG. Summary.A case of bilateral ‘double teeth’ is described. The patient, aged 8 at initial presentation, was unhappy with the appearance of his two abnormally large upper central incisor teeth. Endodontic treatment was carried out on both teeth followed by surgical splitting and the removal of half of each so as to leave two reasonably‐shaped central incisors. Comprehensive orthodontic treatment was then carried out to align the teeth and reduce the overjet, following which the central incisors exhibited no increased mobility or any periodontal problems.Résumé.Traitement chirurgical et orthodontique de dents géminées bilatérales: étude d'un casUn cas de dents bigéminées bilatérales est décrit. Le patient âgé de huit ans, lors de la première consultation, se plaignait de l'aspect de ses deux incisives centrales supérieures, anormalement larges. Un traitement endodontique a été réalisé suivi d'une suppression chirurgicale de la moitié de chaque incisive pour obtenir un aspect normal. Un traitement orthodontique a permis l'alignement des dents et la suppression de l'overjet. Les incisives ne montraient aucune mobilité accrue, ni problémes paradontaux après ces traitements.Zusammenfassung.Eine kombinierte chirurgische und orthodontische Behandlungsmethode von bilateralen Doppelzähnen: Ein FallreportEin Fall von bilateralen Doppelzähnen wird beschrieben. Ein Patient 8 Jahre alt war unglücklich mit dem Aussehen seiner Zwei zentralen übergrossen Schneidezähnen. Eine Wurzelbehandlung an beiden Zähnen wurde durchgeführt, dann eine chirurgische Trennung und die Entfernung deren Hälften, so das 2 relativ normal grosse zentrale Schneidezähne übrig blieben. Anschliessend wurden mittels einer orthodontischen Behandlung die Zähne eingereiht, und der Overjet reduziert, mit dem Resultat das die behandelten Schneidezähne keinerlei erhönte Beweglichtkeit oder periodontale Propleme aufwiesen.Resumen.Tratamiento quirúrgico‐ortodóntico combinado en dientes dobles bilatérales: reporte de un casoSe describe un caso de ‘diente doble’.
