For many years, burn professionals have attempted to assess the outcomes of different types of burn injury and the factors that are related to good patterns of coping with the aftermath of thermal injury. Most writers have attempted to use objective criteria such as return to work or preexisting psychologic problems (e.g., alcoholism) in determining the success of rehabilitation, but much controversy over the forms of assessment persists. It is agreed that antisocial personality, organic brain syndromes, and lack of social support all undermine good recovery for patients with burns. The authors have attempted to look at the subjective side of the patient's adjustment by providing representative examples of several types of burn adjustment in terms of personality features, all of which would tend to complement other approaches. The use of denial, the ways in which hostility is managed by the patient, and how he or she uses key persons in the environment are examined. The cognitive, emotional (affective), and behavioral styles of patients are examined as part of this pilot study of cluster patterns or types of adjustment.

摘要:
Psychological techniques can serve as useful adjuncts to opioid drugs in the control of pain from severe burns. The lack of research on the use of such interventions may reflect our failure to realize how often we apply psychological principles in everyday management of burn pain. Psychological techniques with burn pain can be divided into cognitive (avoidant and reappraisal), preparatory (sensory and procedural), behavioral (respondent and operant), and hypnotherapy categories. The manner in which each of these types of interventions can be applied to patients in the burn unit is discussed.

Improved survival rates for patients with major burn injuries and the consistent finding of significant long-term psychologic disability among survivors of burn trauma call for a redefinition of the role of the psychiatric consultant in the care of patients with burns. In addition to the traditional functions of diagnosis and treatment of discrete psychiatric disorders in patients with burns, this expanded role includes assisting the patient's normal process of psychologic adaptation after injury, assessing and managing burn pain, and facilitating communication among all members of the burn team. The functions of the psychiatrist are most effectively carried out when the psychiatrist is able to participate on a regular basis in the care of every patient as a member of the burn team.

This article is written from the dual perspective of a child psychiatrist, consultant to a burn unit, who also happens to have suffered burns to his hands and face as the result of a car accident in 1976. One of its central themes is that burns are different from other surgical conditions. The role of the child psychiatrist as a consultant to a pediatric burn unit is explored and illustrated with clinical vignettes.

Burn injuries and subsequent disfigurement may place children at high risk for psychologic dysfunction. If medical professionals are to aid the child in achieving psychologic resolution and acceptance of the injury, they must be aware of the stage of body-image perception that the child is experiencing. This study was designed to determine differences in body image between children with burns and children without burns through the use of human figure drawings. The experimental group consisted of 32 children with burns, aged 5 to 15 years, studied at approximately 5 years after burn injury. The control group consisted of 32 children without burns, aged 6 to 15 years. Subjects in the burn group were matched to control subjects by age and sex. There were no significant differences in body image in relation to any of the demographic measures except age. Age was inversely related to body image both in patients with burns and in control subjects.

The patient with facial and hand scars that show clearly is forced to deal with his or her disfigurement, to grieve the loss, and to adapt to the new image. Patients who have body scars that are hidden by clothes and who fear exposure may never deal with the reality of their damaged selves and thus suffer from low self-esteem and accompanying depression and anxiety. Young adolescent women in particular are concerned about a pleasing appearance as they deal with fears about dating and sexual relationships. It is particularly difficult for them to be open and seek counsel on this subject. This article demonstrates a need for the burn unit therapist to be creative in providing psychotherapeutic intervention for the patient with hidden scars, especially the young woman with breast scars. It also shows that group therapy can play an important role in providing this necessary support.

Although survival of older burn victims has improved significantly in recent years, controversy remains over the care that should be given to this group and the functional recovery of survivors. To assess the impact of burn injury on lifestyle in the elderly, we conducted a mail-in survey among surviving patients ages 45 years or older who were treated in our burn center from 1978 to the middle of 1987. The survey asked questions about living arrangements, personal care, family relations, social life, work status, and outlook on the future. Of 168 surveys mailed, 109 were completed and returned (64.9%). Responders consisted of 82 men and 27 women with a mean age of 57.8 years (range, 45 to 92 years). Ninety-seven percent of patients were able to return home after discharge, but 27% of the oldest patients (ages gt;75 years) required permanent placement in extended care facilities. With increased age there was an increased tendency toward dependence and the need for assistance in daily living; half of the oldest patients required assistance in daily living. Even so, most of the patients in this survey remained independent and maintained a positive outlook on the future. These results appear to justify a policy of aggressive treatment for elderly burn victims.
