The nurse at the bedside is the person who can best identify the problems related to patient care. He or she is the only nurse who can implement significant research findings as an integral part of the care provided. Burn unit problems are innumerable, but with careful planning and expert assistance, nearly any nursing problem can be converted into a research question. This article describes the nursing research program initiated three years ago at Stanford University Hospital.

Although teaching has been a valuable component of the nursing profession, it confers upon the profession a new role and leadership function along with a method of teaching/learning evaluation for which nurses have not always been prepared. 摘要:
Knowledge of the principles and procedures of burn care assists the burn nurse in giving quality patient care and increases job satisfaction. This information should be presented to the nurse during the orientation period. The orientation classes are most effective if presented by the multidisciplinary members of the burn team. Simulation laboratories help to bridge the gap between the classroom and the clinical area. Staff nurse preceptors facilitate the orientee's entry into the clinical area and assist the new nurse in further acquisition of skills. Orientation to the burn intensive care unit should be delayed until basic burn nursing skills are mastered. After orientation the nurse possesses a basic knowledge base on which to build. Ongoing educational activities are needed to develop clinical competence and ensure better patient care.

Improved ways of orienting nurses to burn nursing are needed due to high turnover rates, employment of many nurses new to burn care, the variety of skills required for competency, the cost, and the time that learning requires at the expense of patient care hours. A competency-based orientation program was implemented in a burn center to meet goals including cost effectiveness, use of principles of adult learning, reduction of teacher time in group and one-toone teaching, use of staff nurse preceptors, and objective evaluation. The steps undertaken to develop the program included identifying critical and frequently used competencies essential to orientation; developing tools to evaluate entry knowledge and skills and newly acquired competencies; and providing efficient, effective learning options. A training program for staff nurse preceptors was also implemented. Results of pre- and post-tests on burn nursing content and evaluations of the program by orientees indicate that the goals have been met.

As the only burn facility in the state, our institution felt the obligation to provide an education for the community hospitals that would help them to differentiate those patients who should be treated in a burn facility from those in community hospitals whose condition did not require transfer to a burn facility. A goal was to form an open communication line between the community hospital and the burn facility that would ultimately result in improved patient care as well as decreased mortality and morbidity
